Abstract

Background/Objectives: Non-invasive telemonitoring may support heart-failure management, but the nursing activities embedded within complex programmes are not always clearly distinguished from technological and multidisciplinary components. This systematised review synthesised contemporary evidence on clinical outcomes, self-care, health education, nursing roles and continuity of care. Methods: We searched PubMed, CINAHL, Scopus, SciELO, Dialnet and LILACS for original studies published between January 2020 and June 2026 in English, Spanish or Portuguese. Findings were synthesised narratively and appraised descriptively using Joanna Briggs Institute tools. Independent duplicate screening, extraction and appraisal were not undertaken. Results: From 232 identified records, 23 publications representing 21 independent studies or parent trial populations were included. Evidence was heterogeneous: some structured programmes reported fewer heart-failure-related hospitalisations or worsening events, while others showed neutral or inconsistent clinical effects. Nursing activities included symptom assessment, alert interpretation, health education, self-care support, structured communication and care coordination. Their independent effects could not be isolated from broader multidisciplinary interventions. Conclusions: Non-invasive telemonitoring may support selected patients when integrated into responsive clinical pathways, but its effectiveness should not be generalised across settings. This review clarifies the nursing care processes involved rather than establishing a pooled effect of telemonitoring. The review was retrospectively registered with the Open Science Framework (https://osf.io/r42cm/ (accessed on 18 October 2026)).
