Cover Story: This study examined trends in bicycle-related incidence and mortality in Japan, a rapidly aging country where cycling is an essential mode of transportation, and where the proportion of crashes involving cyclists is rising. An analysis of administrative data identified decreasing incidence and mortality rates at a population level, but no significant change in lethality over the years. Special attention is given to the aging population, highlighting how demographic shifts may contribute to their heightened vulnerability. The article emphasizes the importance of mandatory helmet use based on the results of an analysis among hospitalized cyclists, and other targeted interventions, including re-education, especially among the older population, to mitigate risks and promote safer cycling practices in an aging society.
