Special Issue Information

Dear Colleagues,

Orthopedic diseases represent a diverse and complex challenge in modern medicine, encompassing conditions ranging from degenerative joint diseases to complex musculoskeletal trauma and orthopedic infections. Recent advancements in biomaterials, regenerative medicine and personalized treatment strategies have revolutionized patient outcomes, yet many clinical dilemmas persist. The integration of cutting-edge technologies, including artificial intelligence, 3D printing and patient-specific implants, is reshaping this field, allowing for more precise, efficient and durable orthopedic interventions.

This Special Issue will showcase pioneering research and clinical studies addressing approaches in orthopedic disease management. Topics of interest include minimally invasive, biomechanics-driven implant design; the role of biologics in musculoskeletal repair and emerging strategies in infection control and osteoarticular reconstruction. Additionally, the impact of rehabilitation protocols and multidisciplinary collaborations on functional recovery will be explored.

By bringing together global experts in orthopedic research and practice, this Special Issue will provide a platform for groundbreaking discoveries and evidence-based approaches that will shape the future of orthopedic care. We welcome high-quality original research, systematic reviews and clinical studies that will contribute to the evolution of treatment paradigms and patient-centered orthopedic care.

Background: Dieterich disease, or avascular necrosis of the metacarpal head, is a rare condition predominantly affecting adolescents, with fewer than 60 reported cases. We report a rare presentation in a 59-year-old adult with multiple systemic comorbidities. Methods: A 59-year-old man with diabetes, hypertension, and chronic kidney disease presented with progressive right third metacarpophalangeal joint pain without prior trauma. Following radiographic and magnetic resonance imaging (MRI) evaluation, the patient underwent surgical curettage of the necrotic bone followed by autologous iliac cancellous bone grafting. Histopathological examination supported the diagnosis. Results: At the long-term nearly 5-year follow-up, the patient maintained complete symptom resolution, fully restored range of motion, and durable osseous graft integration without secondary collapse or osteonecrotic recurrence. Conclusions: Although most frequently described in adolescents, Dieterich disease should be considered in adults presenting with unexplained metacarpophalangeal joint pain, especially alongside vascular-compromising comorbidities. MRI serves as a valuable adjunct for early lesion detection when radiographs are inconclusive. In this patient, joint-preserving curettage with autologous bone grafting provided durable structural support and favorable long-term functional recovery. Full article
