Cover Story: In this cover image, the skin microbiome is portrayed as both a trigger and a shield for itch. On the left, bacteria, fungi, and viruses release proteases, pore-forming peptides, and toxins that activate PARs, TRP channels, and MRGPRs on sensory nerves, leading to barrier disruption, type 2 inflammation, and the itch–scratch cycle. On the right, selected commensals such as Malassezia, Roseomonas mucosa, and Staphylococcus hominis support epidermal lipids, tight junctions, and antimicrobial peptides, thereby restoring barrier function and reducing neuro-immune activation. The lower panel highlights how integrating microbiome-directed strategies with IL-31 or IL-4/13 blockade may transform pruritus care—shifting from pathogen eradication alone to ecosystem-aware precision therapies. View this paper
