Cover Story: There are two different types of myelodysplastic neoplasms (MDSs) associated with the deletion of the chromosome 5q region—isolated 5 q deletion (MDS-iso5q) and 5q deletion—observed in association with complex chromosomal abnormalities (MDS-CK). MDS-iso5q usually originates from 5q del as a primary pathogenic event, while MDS-CK usually originates from TP53 mutations as a primary pathogenic event, followed by del5q and other chromosomal abnormalities in the context of generalized chromosomal instability. MDS-iso5q has a better prognosis than MDS-CK. Both MDS-iso5q and MDS-CK are heterogenous at the molecular level, and the characterization of their molecular heterogeneity is fundamental to defining an optimal therapeutic strategy.
