Dear Colleagues,
The story of botulinum toxin in migraine care is one of clinical observation leading to scientific discovery. In the early 1990s, clinicians noted that patients receiving botulinum toxin for cosmetic indications often reported unexpected improvement in their headache symptoms. What began as anecdotal evidence evolved into a rigorous program of research that ultimately established botulinum toxin as the first non-oral preventive therapy approved for chronic migraine.
Since its approval, botulinum toxin has reshaped the preventive treatment landscape, offering an important alternative for patients who are unable to tolerate or do not respond adequately to conventional oral therapies. Its emergence has also deepened our understanding of migraine as a disorder of dysfunctional pain processing rather than solely a vascular phenomenon.
This Special Issue explores the evolution of neurotoxins in migraine treatment from early observations to contemporary clinical practice. We examine current insights into migraine pathophysiology and the mechanisms by which neurotoxins modulate peripheral and central pain pathways to produce sustained clinical benefit. In addition, we review the safety profile, durability of response, and long-term outcomes associated with botulinum toxin therapy in patients living with migraine.
