Special Issue Information

Dear Colleagues,

We are pleased to announce a Special Issue dedicated to exploring the innovations and current challenges in facial plastic and reconstructive surgery. This issue aims to highlight the opportunities and challenges within this dynamic field, including innovative techniques, and the evolving landscape of patient care.

The current research in this field showcases the utilization of advanced imaging techniques, microsurgical precision, and tailored procedures to optimize outcomes. Yet, the field is still grappling with clinical complexities, such as achieving the perfect balance between personalization and maintaining safety and effectiveness.

We invite for submission original research articles and reviews on the following themes:

  • Recent advancements and technological innovations in facial plastic surgery;
  • Challenges in surgical techniques and outcomes;
  • Psychological and social implications of facial reconstructive procedures;
  • Patient safety and postoperative care;
  • Interdisciplinary approaches to facial reconstruction;
  • Minimally invasive procedures for facial treatments.

We invite clinicians, surgeons, and clinical researchers to contribute their original research and insights to advance our collective understanding of individualized plastic reconstructive surgery.

Objective Auricular Measurements and Patient Satisfaction Following Cartilage-Cutting Otoplasty Using Fascioperichondrial Flaps

Background/Objectives: Prominent ears, affecting approximately 5% of the population, can cause significant psychosocial distress without functional impairment. Methods: This retrospective single-center study evaluated a cartilage-cutting otoplasty technique augmented by three triangular fascioperichondrial flaps in 40 patients (80 ears), aged 6–37 years (mean age: 13 years), treated between 2022 and 2024 at Târgu Mureș County Emergency Hospital. Results: Preoperative helical–mastoid distances (HMD) were elevated, superior: 21 mm, middle: 22 mm, inferior: 21 mm; concho-mastoid angle (CMA) averaged ~50°. Surgery involved postauricular skin excision, selective conchal cartilage resection, and flap fixation to the mastoid periosteum for stabilization. Measurements at immediate postoperative and 6-week follow-up showed significant reductions: superior HMD by 10 mm (immediate) and 9 mm (6 weeks), middle by 9 mm and 8 mm, inferior by 4 mm and 4 mm, CMA by 27° and 26° (all p < 0.001). Stability between postoperative and 6-week intervals was confirmed by ANOVA (no significant differences). Patient satisfaction was excellent: 75% very satisfied and 22% satisfied (97% overall positive), with no correlation with age or sex. Conclusions: This hybrid approach offers reliable correction, natural contours, and low postoperative complications, representing a feasible option in otoplasty. From Contouring to Rejuvenation: A Nationwide Big-Data Analysis of Hyaluronic Acid Injection Trends in Japan

Background: Hyaluronic acid (HA) injections have become a cornerstone of minimally invasive aesthetic medicine. While the demand for these procedures continues to grow globally, large-scale longitudinal analyses of patient demographics and specific injection site trends remain limited, particularly in Asian populations. Existing data in Japan are largely confined to aggregate procedure numbers. This study aimed to elucidate the transition in patient demographics and site-specific treatment trends using a nationwide big-data approach. Methods: This retrospective study analyzed 299,413 treatment sessions (417,590 injection sites) from patients who underwent facial HA injections at 110 clinics across Japan between October 2020 and December 2024. Data were analyzed by year, patient age, and injection site to evaluate demographic shifts and treatment patterns. Results: The annual number of treatment sessions increased steadily during the study period. A significant demographic shift was observed: while patients in their 20s were predominant in 2020–2022, the proportion of patients aged ≥ 40 years increased markedly from 2023 onward, accounting for more than half of all cases (63.7% in 2024). Treatment preferences varied distinctly by age; younger patients favored localized contouring (e.g., pretarsal fullness, chin), whereas older patients required multi-site rejuvenation. By 2024, the orbital rim became the most frequently treated site (22.6%). Statistical analysis confirmed that age was a significant predictor for multi-site treatments (p < 0.001). Conclusions: This large-scale analysis reveals a clear transition in the Japanese aesthetic market from contour enhancement in younger demographics to anatomy-based rejuvenation in middle-aged and older populations.
