Morbid obesity, defined by a body mass index (BMI) ≥40 kg/m2 or ≥35 kg/m2 with obesity-related comorbidities, presents complex diagnostic challenges that demand a multidisciplinary approach. This Special Issue explores cutting-edge strategies for accurate assessment, emphasizing the integration of anthropometric, metabolic, imaging, and biochemical markers to refine phenotyping and risk stratification. Key themes include the limitations of BMI, the role of visceral adiposity imaging, genetic and epigenetic profiling, and the impact of comorbidities such as metabolic syndrome and obstructive sleep apnea. By bridging gaps between clinical evaluation and advanced diagnostics, this collection aims to enhance early identification and personalized management frameworks for high-risk populations.
