Special Issue Information

Dear Colleagues,

Pediatric dentistry has undergone a significant transformation, shifting its focus from traditional restorative and surgical interventions to a minimally invasive, preventive, and evidence-based approach. The primary goal is to manage and control dental caries, which remains the most prevalent chronic disease in children globally, while minimizing patient discomfort and anxiety. Recent advancements leverage innovative technologies to preserve healthy tooth structure, promote remineralization, and improve long-term oral health outcomes. Crucial developments include novel caries-arresting agents, regenerative endodontics, and cutting-edge behavior management techniques. This evolution is driven by the commitment to provide children with gentler, more efficient, and personalized dental care, thereby enhancing the overall experience and ensuring a positive relationship with dentistry from a young age.

Background and History 
Historically, pediatric dental care was dominated by the “drill and fill” philosophy for managing carious lesions. The late 20th century brought the pivotal understanding of the caries process as a dynamic continuum of demineralization and remineralization, rather than a simple infection, which spurred the move toward early diagnosis and aggressive prevention. The development of adhesive materials allowed for more conservative restorations. More recently, the field has championed Minimally Invasive Dentistry (MID), prioritizing non-restorative treatments (NRCTs) such as Silver Diamine Fluoride (SDF). The push for less traumatic care is also shaped by ethical considerations and regulatory efforts to reduce the use of general anesthesia and pharmacological sedation in routine pediatric procedures.

Aim and Scope of the Special Issue
The aim of this Special Issue is to synthesize and disseminate the most current clinical and translational research driving the modern landscape of pediatric dentistry. We seek to provide a comprehensive overview of the technological and pharmacological innovations that enable practitioners to address the challenges of pediatric oral health more effectively and compassionately. The scope is broad, covering everything from personalized prevention protocols and advanced caries risk assessment to the latest in pulp regeneration techniques, dental trauma management, and the integration of digital dentistry (AI, 3D printing, teledentistry) into clinical practice. The Issue intends to be a foundational resource for implementing contemporary, child-centric care models.

Cutting-Edge Research
Current cutting-edge research is focused on several transformative areas:
--Non-Restorative Caries Treatment (NRCT): Extensive investigation into Silver Diamine Fluoride (SDF) for its high efficacy in arresting active carious lesions in primary teeth with minimal need for drilling.
--Regenerative Endodontics: Exploring the potential of dental stem cells for vital pulp therapy, aiming for tissue regeneration rather than merely repair.
--Digital and Laser Dentistry: The application of laser technology (e.g., Erbium lasers) for painless cavity preparation and soft tissue surgery, and the use of 3D printing and intraoral scanners for precise, customized appliances and treatment planning.
--Advanced Anxiety Reduction: Studies on the effectiveness of Virtual Reality (VR) and Augmented Reality (AR) as distraction techniques to reduce dental anxiety and improve behavior management without heavy pharmacological intervention.

What Kind of Papers We Are Soliciting
We invite high-quality original research and reviews in the following specific areas:
--Randomized Controlled Trials comparing the long-term clinical and patient-reported outcomes of SDF versus traditional restorative approaches in early childhood caries (ECC).
--Systematic Reviews and Meta-Analyses on the success rates and material advancements in vital pulp therapies (e.g., pulpotomies using bioactive materials).
--Translational Studies evaluating the integration and impact of Artificial Intelligence (AI) models for automated caries detection and risk prediction in pediatric populations.
--Clinical Efficacy Studies assessing novel non-pharmacological methods (e.g., VR, Acupressure) for reducing dental anxiety and improving child cooperation.

Efficacy and Safety of Nitrous Oxide and Midazolam Sedation Protocols in Pediatric Dentistry: A Multicenter Clinical Retrospective Study

Aim: This two-center retrospective observational study evaluated whether outpatient conscious sedation protocols based on titrated nitrous oxide/oxygen, midazolam, or their combination enabled complex dental and minor oral surgical procedures to be completed in uncooperative pediatric and special needs patients, without escalation to deep intravenous sedation or general anesthesia. Materials and Methods: Forty pediatric and special needs patients treated at the Conscious Sedation Units of Fondazione IRCCS Ca’ Granda Ospedale Maggiore Policlinico (Milan, Italy) and Policlinico of Bari (Bari, Italy) were included. Overall, 60 complex dental or minor oral surgical procedures were performed. Procedures were classified according to the sedation protocol administered: Protocol A, nitrous oxide/oxygen inhalation sedation alone; Protocol B, midazolam alone; and Protocol C, combined midazolam and nitrous oxide/oxygen sedation. Vital signs were monitored before, during, and after treatment. The primary outcome was successful procedural completion, defined as completion of the planned procedure without premature termination, escalation to deep intravenous sedation or general anesthesia, or the need for unplanned airway or emergency medical intervention. Results: Forty patients underwent 60 procedures. The mean age was 8.7 ± 2.3 years (range, 4–14 years). Protocol A was used in 24 procedures (40%), Protocol B in 6 procedures (10%), and Protocol C in 30 procedures (50%). All procedures were completed successfully, with no conversion to general anesthesia, no airway crises, and no medical emergencies. Transient elevations in heart rate and blood pressure during local anesthesia occurred in 11/60 procedures (18.3%), and mild self-limiting nausea or fatigue occurred in 5/60 procedures (8.3%). Conclusions: The evaluated protocols were associated with successful procedural completion and few recorded adverse events in this selected cohort. Combined behavioral and pharmacological management was associated with completion of the planned procedures without conversion to deep intravenous sedation or general anesthesia. Comparative studies are required to determine whether these protocols reduce general-anesthesia utilization. Full article

Background: Dental anxiety remains a prevalent and persistent challenge in pediatric dentistry, significantly affecting children’s cooperation, treatment outcomes, and long-term oral health behaviors. Despite advances in minimally invasive care, anxiety continues to act as a barrier to effective clinical management. This narrative review aims to synthesize current evidence on validated assessment tools for pediatric dental anxiety and to examine contemporary non-pharmacological management strategies applicable in routine clinical practice. Methods: A structured literature search was conducted in major electronic databases to identify relevant studies, systematic reviews, and clinical guidelines addressing dental anxiety assessment and behavioral management in children. Particular emphasis was placed on validated anxiety scales, communication strategies, environmental adaptations, and emerging digital interventions such as immersive distraction technologies. Results: Multiple validated instruments are available to assess pediatric dental anxiety; however, their applicability varies by age, cognitive development, and clinical context. Non-pharmacological approaches including tell–show–do, modeling, parental guidance, audiovisual distraction, and virtual reality-based techniques demonstrate consistent effectiveness in reducing anxiety and improving behavioral cooperation. Recent trends emphasize multimodal, patient-centered strategies integrating communication, environmental modification, and digital tools. Conclusions: Structured anxiety assessment combined with contemporary multimodal management strategies can enhance clinical efficiency, improve child cooperation, and promote positive dental experiences. The integration of emerging digital technologies represents a promising advancement in pediatric anxiety management and supports a more individualized approach to care. Furthermore, a structured multimodal clinical framework is proposed to facilitate chairside decision-making and practical implementation. Full article
