Background: Melatonin supplementation has been suggested as influencing migraine frequency, but the few published studies on this subject have arrived at differing conclusions. Case: A 37-year-old white male, who had experienced four to six attacks of migraine with aura yearly over the previous nineteen years, noticed repeated morning migraine attacks with aura 24 to 48 hours after the intake of melatonin (patient’s estimate of frequency was 80% of the events), which was being used as a sleep inducer at a dose of 3–6 mg. The intensity of pain was independent of duration and quality of sleep. Such attacks were considered to be provoked by the melatonin withdrawal. Conclusion: This case illustrates the complexity of the relationship between melatonin and migraines, and suggests that the selection of certain migraine phenotypes and the manipulation of the melatonin doses could be of value in further studies. Microbleeds are under intense scrutiny as predictors of intracerebral haemorrhage and as an in-vivo marker of cerebral amyloid angiopathy, a disease that is notoriously difficult to diagnose in a noninvasive way. Microbleeds are common in patients with haemorrhagic and ischaemic cerebrovascular disease. Cortical microbleeds are associated with cerebral amyloid angiopathy and microbleeds in deep cerebral structures and brainstem are thought to be one facet of cerebral small vessel disease. On magnetic resonance imaging a few diseases and artefacts may mimic microbleeds. In the current state of knowledge, the presence of a few microbleeds should probably not be considered a contraindication to thrombolysis or oral anticoagulation. Full article

Clinical, scanographic and histological aspects of cerebral tumours in children in Yaoundé, Cameroon Cerebral tumours in children occur in approximately 2.5 cases per 100 000 inhabitants every year in developed countries, however, they remain misunderstood in subSaharan Africa, where there is a problem with diagnosis and treatment. The objective of this retrospective work was to illustrate the clinical, scanographic and histological aspects of cerebral tumours in children in Yaoundé. Children who have had their cerebral tumours operated on were included, the age of whom varied between 0 and over 15 years. In all cases, the working diagnosis was made using scanography and the confirmed diagnosis was determined using histology. Cerebral tumours in children (42 cases) represented 35.29% of all cerebral tumours (119 cases). Intracranial hypertension represented the most frequent method of discovery (88.09%) followed by cerebellar syndrome (37.5%). The subtentorial position was the most frequent and represented 54.76% of cases. Cerebral tumours in children are frequent in Yaoundé. The subtentorial position was most frequently encountered. Astrocytoma was the most frequent type in the histology. The diagnosis essentially rests on computed tomography which enables the histological diagnosis to be targeted in 71.43% of cases and the tumour to be located. Full article

Pharmacologically supported relapse prevention of alcohol dependence with so-called anticraving substances is still a relatively new clinical field. Apart from disulfiram, which when alcohol is consumed results in massive intolerance reactions, for many years there have been no promising pharmacological approaches to improving prognosis in alcohol dependence. By now there has been a clearcut improvement in knowledge of the neurobiological basis of alcohol dependence and basic addiction mechanisms, such as, for example, craving. A solidly evidence-based finding by now is that acamprosat reduces the risk of relapse in alcohol dependence by acting via glutamatergic neurons with largely good tolerance. The effectiveness of the opioid antagonist naltrexon in the treatment of alcohol dependence is also solidly evidence-based. Naltrexon’s main action is to lower the positive-reinforcing effect of alcohol vectored by opiate receptors. New clinically relevant developments may be another opioid antagonist, nalmefen, and the use of depot-naltrexon, which could markedly improve the compliance problem. A number of investigations of the two substances have been published or are under evaluation. Other substances for which at least first research results are available are GABA-B receptor agonist baclofen, the antiepileptics topiramat and gabapentin, daidzen, the drug derived from Chinese medicine which is probably an alcohol dehydrogenase inhibitor, and the atypical neuroleptic quetiapin. Substances resulting in modulation of the stress system may also be of clinical interest. To date, biologically-oriented addiction research has failed to produce a “magic bullet” to improve the prognosis or reduce the risk of relapse in alcohol dependence, but the first evidence-based therapeutic approaches exist.
