Fibromyalgia syndrome – a disorder of stress coping Setting out from a view of fibromyalgia as a rheumatological disease, it is shown, on the basis of a careful compilation of neurobiological, psychologicodevelopmental and clinical research results that this is a disorder of coping with stress and a pathogenetic model is developed. Against this background diagnostic subgroups are differentiated and the resultant consequences for causative treatment outlined. Full article

Facts and rules for an accurate neuro-otological diagnosis The successful road to neuro-otology starts with an appreciation of phenomenology, in particular eye movements. Some physiological facts and “laws” concerning the vestibulo-ocular reflex (VOR) constitute the basis for accurate neuro-otological diagnosis. These include the frequency-dependence of the VOR, the concept of vestibular resting rate, the push-pull configuration of the semicircular canals, Ewald’s second law, Alexander’s law, and the inherent imbalance of vertical eye position. Only when one is able to distinguish normal from disordered ocular motor behaviour, and understands the underlying physiological mechanisms, is it possible to identify the affected neuro anatomical structures and establish accurate diagnoses. Psychotherapy in case of dizziness: explanatory model and case report The traditional discrimination between organic and psychogenic dizziness fails to capture the complex relationships between neurologic, otologic, and psychiatric factors in patients suffering from long-standing dizziness. After an introduction, this case report presents and discusses the cognitive-behavioural treatment of a 29-year-old patient with frequent dizziness attacks without avoidance behaviour. The diagnostic model was based on both somatic and psychic components. This enabled the patient, who kept assuming an overlooked organic pathology, to understand the potential helpfulness of a psychotherapeutic treatment. The treatment plan was derived from an individual case formulation in order to consider the potential causal, developmental, and maintenance factors of the symptomatology. Full article

Insomnia Sleep disorders are among the most common symptoms in the general population. According to international classification schemes, in clinical practice they are divided into insomnia, hypersomnia, sleep-wake rhythm disorders and parasomnia, whereby insomnia occurs most frequently. In the event of insomnia occurring, the first step always involves cause-oriented investigation and treatment. In order to investigate the causes, the “5 P” rule can be followed: “Physical, Pharmacological, Physiological, Psychological, Psychiatric”. From a quantitative perspective, restless legs syndrome and sleep apnoea, above all, are of consequence among the somatic causes. The taking of sleep-disturbing substances, stimulants in particular, must also be taken into account especially. In the therapeutic schedule, non-pharmacological treatments such as a sleep consultation, investigation, and relaxation techniques are the top priority after cause-oriented investigation and treatment. To this one can add specific psychotherapeutic techniques, among which behavioural therapy-oriented processes, above all, have shown good acceptance and scientific evidence. In pharmacotherapy, phytotherapeutics can also be prescribed prior to the use of actual hypnotics. Full article

Tiredness/sleepiness: practical diagnosis In the present review article the authors describe their personal experience with patients referred to a tertiary sleep centre for tiredness or sleepiness. The paper is a non-systematic narrative review. After discussing the important differentiation of sleepiness, tiredness and fatigue, the clinical and paraclinical diagnostic processes in sleepy or tired patients are described, with a major emphasis on the clinical characteristics of the presenting symptom and its course during the day. Causes of sleepiness, tiredness or fatigue of social, medical or psychiatric origin are delineated and relevant subjective and objective tests were explained. Full article
