Evaluation of Myocardial Protection in Prolonged Aortic Cross-Clamp Times: Del Nido and HTK Cardioplegia in Adult Cardiac Surgery

Background and Objectives: Effective myocardial protection is essential for successful cardiac surgery outcomes, especially in complex and prolonged procedures. To this end, Del Nido (DN) and histidine-tryptophan-ketoglutarate (HTK) cardioplegia solutions are widely used; however, their comparative efficacy in adult surgeries with prolonged aortic cross-clamp (ACC) times remains unclear. This study aimed to compare the efficacy and safety of DN and HTK for myocardial protection during prolonged ACC times in adult cardiac surgery and to define clinically relevant thresholds. Materials and Methods: This retrospective study included a total of 320 adult patients who underwent cardiac surgery under cardiopulmonary bypass (CPB) with an aortic cross-clamp time ≥ 90 min. Data were collected from the medical records of elective adult cardiac surgery cases performed at a single center between 2019 and 2025. Patients were categorized into two groups based on the type of cardioplegia received: Del Nido (n = 160) and HTK (n = 160). The groups were compared using 1:1 propensity score matching. Clinical and biochemical outcomes—including troponin I (TnI), CK-MB, lactate levels, incidence of low cardiac output syndrome (LCOS), and need for mechanical circulatory support—were analyzed between the two cardioplegia groups. Subgroup analyses were performed according to ACC duration (90–120, 120–150, 150–180 and >180 min). The predictive threshold of ACC duration for each complication was determined by ROC analysis, followed by the analysis of independent predictors of each endpoint by multivariate logistic regression. Results: Intraoperative cardioplegia volume and transfusion requirements were lower in the DN group (p < 0.05). HTK was associated with lower TnI levels and less intra-aortic balloon pump (IABP) requirement at ACC times exceeding 180 min. Markers of myocardial injury were lower in patients with an ACC duration of 120–150 min in favor of HTK. The propensity for ventricular fibrillation after ACC was significantly lower in the DN group. Significantly lower postoperative sodium levels were observed in the HTK group. Prolonged ACC duration was an independent risk factor for LCOS (odds ratio [OR]: 1.023, p < 0.001), VIS > 15 (OR, 1.015; p < 0.001), IABP requirement (OR: 1.020, p = 0.002), and early mortality (OR: 1.016, p = 0.048). Postoperative ejection fraction (EF), troponin I, and CK-MB levels were associated with the development of LCOS and a VIS > 15. Furthermore, according to ROC analysis, HTK cardioplegia was able to tolerate ACC for up to a longer duration in terms of certain complications, suggesting a higher physiological tolerance to ischemia. Conclusions: ACC duration is a strong predictor of major adverse outcomes in adult cardiac surgeries. Although DN cardioplegia is effective and economically advantageous for shorter procedures, HTK may provide superior myocardial protection in operations with long ACC duration. This study supports the need to individualize cardioplegia choice according to ACC duration. Further prospective studies are needed to establish standard dosing protocols and to optimize cardioplegia selection according to surgical duration and complexity. Full article

Objectives: Diabetes Mellitus involves demanding challenges that interfere with family functioning and routines. In turn, family and social context impacts individual glycemic control. This study aims to identify this recursive interplay, the mutual influences of family systems and diabetes management. Design: Data was collected through a cross-sectional design comparing patients, aged 22–55, with and without metabolic control. Methods: Participants filled out a set of self-report measures of sociodemographic, clinical and family systems assessment. Patients (91) were also invited to describe their perception about disease management interference regarding family functioning. We first examined the extent to which family variables grouped dataset to determine if there were similarities and dissimilarities that fit with our initial diabetic groups’ classification. Results: Cluster analysis results identify a two-cluster solution validating initial classification of two groups of patients: 49 with metabolic control (MC) and 42 without metabolic control (NoMC). Independent sample tests suggested statistically significant differences between groups in family subscales- family difficulties and family communication (p < 0.05). Binary logistic regression shed light on predictors of explained variance to no metabolic control, in four models: Sociodemographic, Clinical data, SCORE-15/Congruence Scale and Eating Behavior. Furthermore, groups differ on family support, level and sources of family conflict caused by diabetes management issues. Considering only patients who co-habit with a partner for more than one year (N = 44), NoMC patients score lower on marital functioning in all categories (p < 0.05). Discussion: Family-Chronic illness interaction plays a significant role in a patient’s adherence to treatment. This study highlights the Standards of Medical Care for Diabetes, considering caregivers and family members on diabetes care. Full article

Candidiasis arises from the proliferation of Candida species in the human body, especially in individuals with compromised immune systems. Efficient therapeutic management of candidiasis is often hampered by the limited availability of potent antifungal drugs and the emergence of drug-resistant strains. We have previously identified the N-[(4-sulfamoylphenyl)methyl][1,1′-biphenyl]-4-carboxamide to have fungistatic and fungicidal properties, likely due to the hydrophobic biphenyl–chemical features affecting the structural organization of Candida spp. cell membrane. Here, we designed and synthesized a novel series of twelve 5-arylfuran-2-carboxamide derivatives bearing a new hydrophobic tail as bioisosteric replacement of the diphenyl fragment. Its antifungal effectiveness against C. albicans, C. glabrata, and C. parapsilosis, including ATCC and clinically isolated strains, was assessed for all compounds. The most active compound was N-benzyl-5-(3,4-dichlorophenyl)furan-2-carboxamide (6), with fungistatic and fungicidal effects against C. glabrata and C. parapsilosis strains (MIC = 0.062–0.125 and 0.125–0.250 mg/mL, respectively). No synergistic effects were observed when combined with fluconazole. Interestingly, fluorescent microscopy analysis after staining with SYTO 9 and propidium iodide revealed that compound 6 affected the cell membrane integrity in C. albicans strain 16. Finally, carboxamide 6 exhibited a dose-dependent cytotoxicity on erythrocytes, based on assessing the LDH release. Full article

This paper focuses on the failure mechanism of a pre-stressed CFRP cantilever beam under laser ablation. During testing, a mass was applied to the CFRP cantilever beam to achieve a pre-stressed state, and the laser power densities varied from 500 to 1500 W·cm−2. Corresponding scanning electron microscope (SEM) tests were also performed on the ablation zone and fracture surface to analyze the failure mechanism. The results showed that the CFRP beam failed in compression at the bottom surface, which was due to a decrease in local stiffness and strength caused by heat softening, rather than by ablation damage on the top surface. The failure time decreased from 19.64 s to 6.52 s as the power density (500–1500 W·cm−2) and pre-stress loading (300–750 N·cm) increased, indicating that pre-stress loading has a more significant influence on the failure time of CFRP beams compared to power density. This study proposes an improved YOLO11n-seg instance segmentation model to address the limitations of existing models in accurately identifying mature blueberries in complex greenhouse environments. Current methods often lack sufficient accuracy when dealing with complex scenarios, such as fruit occlusion, lighting variations, and target overlap. To overcome these challenges, we developed a novel approach that integrates a Spatial–Channel Adaptive (SCA) attention mechanism and a Dual Attention Balancing (DAB) module. The SCA mechanism dynamically adjusts the receptive field through deformable convolutions and fuses multi-scale color features. This enhances the model’s ability to recognize occluded targets and improves its adaptability to variations in lighting. The DAB module combines channel–spatial attention and structural reparameterization techniques. This optimizes the YOLO11n structure and effectively suppresses background interference. Consequently, the model’s accuracy in recognizing fruit contours improves. Additionally, we introduce Normalized Wasserstein Distance (NWD) to replace the traditional intersection over union (IoU) metric and address bias issues that arise in dense small object matching. Experimental results demonstrate that the improved model significantly improves target detection accuracy, recall rate, and mAP@0.5, achieving increases of 1.8%, 1.5%, and 0.5%, respectively, over the baseline model. On our self-built greenhouse blueberry dataset, the mask segmentation accuracy, recall rate, and mAP@0.5 increased by 0.8%, 1.2%, and 0.1%, respectively. In tests across six complex scenarios, the improved model demonstrated greater robustness than mainstream models such as YOLOv8n-seg, YOLOv8n-seg-p6, and YOLOv9c-seg, especially in scenes with dense occlusions. The improvement in mAP@0.5 and F1 scores validates the effectiveness of combining attention mechanisms and multiple metric optimizations, for instance, segmentation tasks in complex agricultural scenes. SABE-YOLO: Structure-Aware and Boundary-Enhanced YOLO for Weld Seam Instance Segmentation

Accurate weld seam recognition is essential in automated welding systems, as it directly affects path planning and welding quality. With the rapid advancement of industrial vision, weld seam instance segmentation has emerged as a prominent research focus in both academia and industry. However, existing approaches still face significant challenges in boundary perception and structural representation. Due to the inherently elongated shapes, complex geometries, and blurred edges of weld seams, current segmentation models often struggle to maintain high accuracy in practical applications. To address this issue, a novel structure-aware and boundary-enhanced YOLO (SABE-YOLO) is proposed for weld seam instance segmentation. First, a Structure-Aware Fusion Module (SAFM) is designed to enhance structural feature representation through strip pooling attention and element-wise multiplicative fusion, targeting the difficulty in extracting elongated and complex features. Second, a C2f-based Boundary-Enhanced Aggregation Module (C2f-BEAM) is constructed to improve edge feature sensitivity by integrating multi-scale boundary detail extraction, feature aggregation, and attention mechanisms. Finally, the inner minimum point distance-based intersection over union (Inner-MPDIoU) is introduced to improve localization accuracy for weld seam regions. Experimental results on the self-built weld seam image dataset show that SABE-YOLO outperforms YOLOv8n-Seg by 3 percentage points in the AP(50–95) metric, reaching 46.3%. Meanwhile, it maintains a low computational cost (18.3 GFLOPs) and a small number of parameters (6.6M), while achieving an inference speed of 127 FPS, demonstrating a favorable trade-off between segmentation accuracy and computational efficiency. The proposed method provides an effective solution for high-precision visual perception of complex weld seam structures and demonstrates strong potential for industrial application. Full article

Genetic Complexity in Spondyloarthritis: Contributions of HLA-B Alleles Beyond HLA-B*27 in Romanian Patients

This study examined the distribution and disease associations of non-HLA-B*27 HLA-B alleles in Romanian spondyloarthritis (SpA) patients, aiming to address the underrepresentation of Eastern European populations in immunogenetic research. Methods: We analyzed 263 HLA-B*27-negative patients from Northeastern Romania fulfilling ASAS criteria. HLA-B genotyping was performed at two-digit resolution, and allele distributions were compared with two Romanian HLA-B*27-negative control groups (n = 335 and n = 1705 cases), using chi-square testing and logistic regression. Compared to controls, HLA-B*47 (p = 0.0007) and HLA-B*54 (p = 0.0013) were significantly enriched, while HLA-B*40 was underrepresented (p = 0.0287). Notably, HLA-B*54 was observed exclusively in axial SpA. Within the cohort, both HLA-B*13 and HLA-B*57 alleles were associated with psoriasis, while HLA-B*37 and HLA-B*41 alleles were clustered within the reactive arthritis group. The HLA-B*35 and HLA-B*18 alleles were the most frequently observed alleles across most clinical phenotypes. When comparing the frequency of HLA-B associations, the most common genotypes among SpA patients were B*08-B*18, B*13-B*35, and B*35-B*51. Notably, B*08-B*18 was more frequent in patients with radiographic sacroiliitis grade ≥ 2, while B*35-B*51 was more frequent in those with confirmed systemic inflammation, as indicated by elevated CRP or ESR levels. Analysis of peptide-binding patterns revealed a cluster of risk alleles, HLA-B*08, B*18, B*35, B*40, and B*54, sharing similar features, distinct from the canonical profile of B*27. These findings highlight the contribution of non-B*27 HLA-B alleles to SpA susceptibility in an Eastern European population and support the notion that HLA-B*27-negative SpA may represent a distinct clinical and immunological entity, driven by alternative pathogenic mechanisms. They also emphasize the importance of population-specific immunogenetic profiling and support expanding genetic characterization in HLA-B*27-negative patients. Full article

The Impact of Storage Time and Reheating Method on the Quality of a Precooked Lamb-Based Dish

Ready-to-eat meat products face quality challenges during storage and reheating. This study aimed to (i) characterize the physicochemical/microbiological changes in stewed mutton during storage (4 °C/−18 °C, 0–28 days) and (ii) evaluate reheating methods (boiling vs. microwaving) on day-7 samples. The nutritional analysis confirmed moisture reduction (57.32 vs. 72.12 g/100 g)-concentrated protein/fat levels. Storage at −18 °C suppressed microbial growth (the total plate count (TPC), 3.73 vs. 4.80 log CFU/g at 28 days; p < 0.05) and lipid oxidation (thiobarbituric acid reactive substances (TBARS): 0.14 vs. 0.19 mg/kg) more effectively than storage at 4 °C. The total volatile basic nitrogen (TVB-N) kinetics projected a shelf life ≥90 days (4 °C) and ≥120 days (−18 °C). Microwave reheating after frozen storage (−18 °C) maximized the yield (86.21% vs. 75.90% boiling; p < 0.05) and preserved volatile profiles closest to those in the fresh samples (gas chromatography–mass spectrometry (GC-MS)/electronic nose). The combination of freezing storage and subsequent microwave reheating has been demonstrated to be an effective method for preserving the quality of a precooked lamb dish, thereby ensuring its nutritional value. Comparative In Vitro Analysis of Composite Resins Used in Clear Aligner Attachments

Background: Attachments are essential components in clear aligner therapy, enhancing retention and improving the predictability of tooth movements. Mechanical and wear properties of the composite resins used for attachment reproduction are critical to maintaining their integrity and shape over time. This study aimed to evaluate and compare the mechanical properties, thermal behavior, and wear performance of the hybrid composite Aligner Connect (AC) and the flowable resin (Connect Flow, CF). Methods: Twenty samples (ten AC and ten CF) were reproduced. All specimens underwent differential scanning calorimetry (DSC), combustion analysis, flat instrumented indentation, compression stress relaxation tests, and tribological analysis. A 3D wear profile reconstruction was performed to assess wear surfaces. Results: DSC and combustion analyses revealed distinct thermal transitions, with CF showing significantly lower Tg values (103.8 °C/81.4 °C) than AC (110.8 °C/89.6 °C) and lower residual mass after combustion (23% vs. 61%), reflecting reduced filler content and greater polymer mobility. AC exhibited superior mechanical properties, with higher maximum load (585.9 ± 22.36 N) and elastic modulus (231.5 ± 9.1 MPa) than CF (290.2 ± 5.52 N; 156 ± 10.5 MPa). Stress relaxation decrease was less pronounced in AC (18 ± 4%) than in CF (20 ± 4%). AC also showed a significantly higher friction coefficient (0.62 ± 0.060) than CF (0.55 ± 0.095), along with greater wear volume (0.012 ± 0.0055 mm3 vs. 0.0070 ± 0.0083 mm3) and maximum depth (36.88 ± 3.642 µm vs. 17.91 ± 3.387 µm). Surface roughness before wear was higher for AC (Ra, 0.577 ± 0.035 µm; Rt, 4.369 ± 0.521 µm) than for CF (Ra, 0.337 ± 0.070 µm; Rt, 2.862 ± 0.549 µm). After wear tests, roughness values converged (Ra, 0.247 ± 0.036 µm for AC; Ra, 0.236 ± 0.019 µm for CF) indicating smoothened and similar surfaces for both composites. Conclusions: The hybrid nanocomposite demonstrated greater properties in terms of stiffness, load-bearing capacity, and structural integrity when compared with flowable resin. Its use may ensure more durable attachment integrity and improved aligner–tooth interface performance over time. Full article

Biothiols, including cysteine (Cys), homocysteine (Hcy), and glutathione (GSH), are crucial for physiological regulation and their imbalance poses severe health risks. Herein, we developed a pH-responsive liquid crystal (LC)-based sensing platform for detection of biothiols by doping 4-n-pentylbiphenyl-4-carboxylic acid (PBA) into 4-n-pentyl-4-cyanobiphenyl (5CB). Urease catalyzed urea hydrolysis to produce OH, triggering the deprotonation of PBA, thereby inducing a vertical alignment of LC molecules at the interface corresponding to dark optical appearances. Heavy metal ions (e.g., Hg2+) could inhibit urease activity, under which condition LC presents bright optical images and LC molecules maintain a state of tilted arrangement. However, biothiols competitively bind to Hg2+, the activity of urease is maintained which enables the occurrence of urea hydrolysis. This case triggers LC molecules to align in a vertical orientation, resulting in bright optical images. This pH-driven reorientation of LCs provides a visual readout (bright-to-dark transition) correlated with biothiol concentration. The detection limits of Cys/Hcy and GSH for the PBA-doped LC platform are 0.1 μM and 0.5 μM, respectively. Overall, this study provides a simple, label-free and low-cost strategy that has a broad application prospect for the detection of biothiols. Full article

In this study, we applied a new technique for vegetatively transplanting kelp Eisenia bicyclis to restore macroalgal habitats. We aimed to assess the restoration success of the E. bicyclis bed by comparing the carbon and nitrogen stable isotope ratios of macrobenthic consumers and their isotopic niches in artificial and control (barren ground) habitats. Except for the deposit feeding group, no significant differences were observed in isotopic values of the other feeding groups (suspension feeders, herbivores, omnivores, and carnivores) between the two sites. In contrast, our results showed wider isotopic niche indices for all feeding groups at the transplantation site compared to those at the control site, suggesting increased trophic diversity in the artificial habitat. Overall, these results indicate that the macroalgal bed created using the new method can play an ecological role in restoring functional properties of food web structures via trophic support of degraded coastal ecosystems. Urban forests are vital to cities because they provide a range of ecosystem services, including carbon (C) sequestration, air purification, and urban cooling. However, urban forestry also generates significant amounts of organic waste, such as grass clippings, pruned tree branches, and fallen tree leaves and woody debris that can contribute to greenhouse gas (GHG) emissions if not properly managed. In this study, we investigated the effect of wheat straw biochar (produced at 500 °C) on GHG emissions from two types of urban forestry waste: green waste (GW) and yard waste (YW), using a 100-day laboratory incubation experiment. Overall, GW released more CO2 than YW, but biochar addition reduced cumulative CO2 emissions by 9.8% in GW and by 17.6% in YW. However, biochar increased CH4 emissions from GW and reduced the CH4 sink strength of YW. Biochar also had contrasting effects on N2O emissions, increasing them by 94.3% in GW but decreasing them by 61.4% in YW. Consequently, the highest global warming potential was observed in biochar-amended GW (125.3 g CO2-eq kg−1). Our findings emphasize that the effect of biochar on GHG emissions varies with waste type and suggest that selecting appropriate biochar types is critical for mitigating GHG emissions from urban forestry waste. The aim was to investigate the genetic effects of a SNP located in the precursor region of gga-miR-3528. (1) Single-nucleotide polymorphisms within precursor regions of microRNAs play crucial biological roles. (2) Utilizing a Gushi–Anka F2 resource population (n = 860), we screened and validated miRNA SNPs. A SNP mutation in the miR-3528 precursor region was identified. Specific primers were designed to amplify the polymorphic fragment. Genotyping was performed for this individual SNP across the population, using the MassArray system. Association analyses were conducted between this SNP and chicken growth and body measurement traits, carcass traits, meat quality traits, and serum enzyme activities. (3) The rs14098602 (+12 bp A > G) was identified within the precursor region of gga-miR-3528. Significant associations (p < 0.05) were observed between this SNP and chicken growth traits (body weight at the age of 0 day, body weight at the age of 2 weeks, and body weight at the age of 4 weeks), carcass traits (evisceration weight), meat quality traits (subcutaneous fat rate and pectoral muscle density), and serum enzyme activities (total protein, albumin, globulin, cholinesterase, and lactate dehydrogenase). (4) These findings suggest that the polymorphism at rs14098602 may influence chicken growth, meat quality, and serum biochemical indices, through specific mechanisms. The gga-miR-3528 gene likely plays an important role in chicken development. Therefore, this SNP can serve as a molecular marker for genetic breeding and auxiliary selection of growth-related traits, facilitating the rapid establishment of elite chicken populations with superior genetic resources.
