Segmentectomy and Lobectomy in Early-Stage Lung Cancer Management

Dear Colleagues,
The surgical management of early-stage non-small cell lung cancer (NSCLC) has long regarded lobectomy as the standard of care. However, segmentectomy—an anatomical sublobar resection that preserves more pulmonary parenchyma—has gained increasing attention as an oncologically acceptable alternative for select patients. Recent randomized trials have supported this shift in practice: the JCOG0802/WJOG4607L trial demonstrated the statistical superiority of segmentectomy over lobectomy in terms of overall survival, while the CALGB 140503 trial confirmed the non-inferiority of segmentectomy for disease-free survival in patients with peripheral stage IA tumors.
Segmentectomy offers the advantage of better postoperative pulmonary function while maintaining adequate oncologic control, particularly in small, peripherally located tumors. Conversely, lobectomy provides wider resection margins and facilitates more comprehensive lymph node dissection, reducing the risk of locoregional recurrence.
This Special Issue aims to explore contemporary evidence and evolving perspectives on segmentectomy and lobectomy for early-stage NSCLC. We welcome submissions that provide novel insights, refine patient selection strategies, evaluate perioperative and long-term outcomes, or challenge existing paradigms in thoracic oncology.
Scope of the Special Issue
Submissions may include but are not limited to
- Comparative studies of lobectomy vs. segmentectomy (oncologic and functional outcomes);
- Surgical techniques and innovations in anatomical segmentectomy;
- Imaging, AI, and preoperative planning for sublobar resection;
- Lymph node dissection strategies in limited resections;
- Risk stratification tools for surgical decision-making;
- Real-world data and guideline implementation.
