Dear Colleagues,
Non-operating room anesthesia (NORA) has emerged as a rapidly expanding and integral component of modern anesthesiology practice. With the increasing volume and complexity of procedures performed outside the traditional operating room—including endoscopy suites, interventional radiology, cardiac catheterization laboratories, and office-based settings—there is a growing need for adaptable, safe, and efficient anesthesia care tailored to diverse environments. These locations often present unique challenges, including variable infrastructure, limited resources, and evolving procedural demands.
Recent advances in pharmacology and monitoring have significantly enhanced the safety and feasibility of NORA. Short-acting anesthetic agents, improved sedation strategies, and the use of drugs such as remimazolam are facilitating faster recovery and improved patient throughput. Capnography, processed EEG, and noninvasive hemodynamic monitoring have become increasingly important in detecting early physiologic compromise, particularly in deeply sedated, non-intubated patients. Additionally, airway management strategies in remote locations continue to evolve, emphasizing preparedness, portability, and rapid response.
The adoption of standardized protocols, including pre-procedural risk stratification and post-procedural recovery pathways, has contributed to improved patient outcomes. Closed-loop systems and computer-assisted drug delivery, though still developing, hold promise in optimizing sedation and minimizing provider variability. Furthermore, the emphasis on patient-centered care, including comfort, satisfaction, and minimal disruption to daily life, aligns closely with the goals of NORA practice.
Despite these advances, NORA presents ongoing challenges related to patient selection, monitoring limitations, and environmental constraints. Continued research is essential to better define the best practices, enhance safety frameworks, and integrate technological innovations across varied procedural settings.
In this Special Issue, Journal of Clinical Medicine (JCM) invites submissions of original research, reviews, and expert perspectives focused on non-operating room anesthesia, including sedation techniques, monitoring, safety, and outcomes across diverse clinical environments.

Manuscripts should be submitted online at www.mdpi.com by registering and logging in to this website. Once you are registered, click here to go to the submission form. Manuscripts can be submitted until the deadline. All submissions that pass pre-check are peer-reviewed. Accepted papers will be published continuously in the journal (as soon as accepted) and will be listed together on the special issue website. Research articles, review articles as well as short communications are invited. For planned papers, a title and short abstract (about 250 words) can be sent to the Editorial Office for assessment.
Submitted manuscripts should not have been published previously, nor be under consideration for publication elsewhere (except conference proceedings papers). All manuscripts are thoroughly refereed through a single-anonymized peer-review process. Journal of Clinical Medicine is an international peer-reviewed open access semimonthly journal published by MDPI.
