Special Issue Information

Dear Colleagues,

Peripheral artery disease (PAD) is a common manifestation of systemic atherosclerosis and a complex multisystem disorder characterized by vascular, metabolic, and inflammatory dysfunction. Beyond limb ischemia, PAD is strongly associated with adverse cardiometabolic outcomes, including coronary artery disease, heart failure, diabetes mellitus, chronic kidney disease, and increased cardiovascular and all-cause mortality. However, the molecular and cellular mechanisms linking peripheral vascular pathology to systemic cardiometabolic complications remain incompletely understood.

This Special Issue aims to provide a mechanistically oriented overview of cardiovascular and metabolic complications associated with PAD, focusing on the molecular pathways and cellular processes that drive disease initiation, progression, and systemic involvement. Topics include endothelial dysfunction, inflammatory and immune signaling, oxidative stress, mitochondrial dysregulation, impaired angiogenesis, vascular remodeling, and skeletal muscle metabolic abnormalities. Particular emphasis is placed on interorgan crosstalk among the peripheral vasculature, heart, skeletal muscle, adipose tissue, kidney, and liver.

This Special Issue also highlights translational research bridging experimental and clinical studies, including biomarker discovery, multi-omics approaches, molecular imaging, and therapeutic strategies targeting inflammatory, metabolic, and redox pathways. By integrating basic and clinical molecular research, this collection aims to advance understanding of PAD as a systemic cardiometabolic disease and to promote innovative strategies to improve outcomes in this high-risk population
