Special Issue Information

Respiratory diseases have become a global health issue, due to the increasing exposure to environmental risk factors as well as to ageing of general population. The huge progresses in the field of respiratory medicine over the last decades have revolutionized diagnostic work-up and therapeutic management of most of lung diseases, extending the borders of pulmologists’ horizon and underlining the relevance of a multidisciplinary approach. For example, with reference to diffuse parenchymal lung diseases (DPLD), advances in comprehension of pathogenesis, that have led to approval of effective antifibrotic drugs, has coupled with exciting evolutions in technologies related to tissue sampling, no longer exclusive domain of thoracic surgeons. However, an accurate diagnosis in this context is a challenging process, as it requires an integrated multidisciplinary approach involving clinicians from various specialties.The main goals of this special issue are to update and summarize the latest knowledge, evidence and innovations in diagnostic approach and treatment of diffuse parenchymal lung diseases, pleural diseases, chronic obstructive lung diseases (COPD) with advanced emphysema and severe asthma, with particular attention to the role of multidisciplinary interactions. Leaders in the different fields will be invited to provide comprehensive reviews on these topics.

Pleural disease diagnostics represent a sprawling topic that has enjoyed a renaissance in recent years from humble beginnings. Whilst pleural patients are heterogeneous as a population and in the aetiology of the disease with which they present, we provide an overview of the typical diagnostic approach. Pleural fluid analysis is the cornerstone of the diagnostic pathway; however, it has many shortcomings. Strong cases have been made for more invasive upfront investigations, including image-guided biopsies or local anaesthetic thoracoscopy, in selected populations. Imaging can guide the diagnostic process as well as act as a vehicle to facilitate therapies, and this is never truer than with the recent advances in thoracic ultrasound. Full article
