Special Issue Information

Dear Colleagues,

Over the last 3 decades, the field of interventional nephrology (IN) has evolved dramatically and became a recognized specialty after the inception of the American Society of Diagnostic and Interventional Nephrology (ASDIN) in 2000. This organization provided the necessary framework for standardized dialysis access procedures, their complications and coding, and the accreditation to the institutions involved in the practice and teaching of these procedures.

It is well acknowledged that dialysis access and its associated complications represent a major cause of morbidity, mortality, and increased cost in the end-stage kidney disease (ESKD) population. However, when performed by interventional nephrologists, the rate of hospitalizations, missed outpatient hemodialysis sessions, and access complications are shown to be decreased. To further improve the clinical outcomes of dialysis vascular access, the Kidney Disease Outcomes Quality Initiative (KDOQI) Vascular Access Guidelines caused a paradigm shift in managing dialysis access by introducing the ESKD Life Plan in 2019. The core concept of the ESKD Life plan was to create the right dialysis access in the right time for the right patient.

Recent years have witnessed remarkable technological and procedural advancements, including (but not limited to) the following:

  • Endovascular arteriovenous fistula (EndoAVF) creation, a minimally invasive alternative to surgical access;
  • Drug-coated balloons (DCBs) and advanced stenting techniques to address pathologic lesions and prolong access patency;
  • Innovative diagnostic tools and imaging modalities for early detection of access dysfunction;
  • Radiologic-guided peritoneal dialysis catheter placement, optimizing precision and outcomes.

The scope of this Issue in Diagnostics is meant to cover the major advancements in the field of interventional nephrology including an update on the 2019 KDOQI guidelines, the effectiveness of drug-coated balloons, the outcomes of stenting in dialysis access, the technological advancements in managing immature access, an update on EndoAVF outcomes, managing central venous occlusions, and peritoneal dialysis catheter radiologic insertion techniques.

We invite original research, reviews, and case studies that explore diagnostic algorithms, novel interventional techniques, and translational research to improve outcomes in ESKD patients.
