Special Issue Information

Dear Colleagues,

Asthma is a heterogeneous chronic airway disease characterized by complex interactions between genetic susceptibility, environmental exposures, and immune dysregulation. Despite major advances in clinical management, significant gaps remain in understanding the molecular mechanisms underlying disease onset, progression, severity, and treatment response. This Special Issue aims to provide an updated and comprehensive overview of the molecular pathways and biomarkers involved in asthma, with particular emphasis on epigenetic regulation, microRNAs, immune cell modulation, and inflammatory signaling networks. Special attention will be given to microRNA-mediated regulatory circuits acting across different cellular compartments, including airway epithelium, immune cells, and extracellular vesicles, as well as their potential role as biomarkers. 

We welcome original research articles, high-quality reviews, and case series addressing mechanistic insights from experimental, translational, and clinical studies. Topics of interest include, but are not limited to, the role of microRNAs and other non-coding RNAs in airway inflammation and remodeling; immune-endotype characterization; biomarkers for diagnosis, prognosis, and therapeutic stratification; gene–environment interactions related to occupational and environmental exposures, air pollution, and oxidative stress; and emerging targets for personalized and precision medicine in asthma. Well-characterized clinical case series highlighting rare phenotypes, complex disease presentations, or translational biomarker findings are particularly encouraged. By integrating molecular biology, immunology, environmental medicine, and clinical perspectives, this Special Issue seeks to advance current knowledge and promote innovative approaches for improved disease phenotyping and management.

Bronchial asthma is a common chronic respiratory disease with a complex etiology, wherein the interaction between genetic and environmental factors plays a critical role in its pathogenesis. The β-2 adrenergic receptor gene (ADRB2) is pivotal in regulating airway smooth muscle relaxation. Its genetic polymorphisms have been extensively studied and are closely associated with asthma susceptibility, clinical phenotypes, and drug responses. Recently, the relationship between major single-nucleotide polymorphisms (SNPs) of the ADRB2 gene and bronchodilator efficacy, alongside its transcriptional regulation and epigenetic modification mechanisms, has been progressively elucidated. Furthermore, ADRB2 genotype-guided therapeutic strategies have shown potential clinical and economic value in selected studies, but they remain exploratory and have not yet been incorporated into routine guideline-based asthma management algorithms. This review summarizes recent advances in the role of the ADRB2 gene concerning genetic susceptibility, disease phenotyping, environmental interactions, and immune regulation in asthma, aiming to provide theoretical support and clinical guidance for precision diagnosis and treatment.
