Impact of Continuous Veno-Venous Hemodiafiltration on Thyroid Homeostasis in Critically Ill Patients

Background: Patients in Intensive Care Units (ICUs) often develop non-thyroidal illness syndrome. Potentially, thyroid hormones may be removed during continuous veno-venous hemodiafiltration (CVVHDF), as their molecular size is smaller than the filter pores’ cutoff. The study’s main aim was to assess whether the serum concentration of thyroid hormones changes over time during CVVHDF. Methods: This was a prospective observational trial that included 30 patients treated in an ICU. All patients developed acute kidney injury (AKI) and had clinical indications for implementation of CVVHDF. Blood samples were collected before initiation of CVVHDF and at 1, 2, 3, 6, 9 and 12 days after. The last sample was collected three days after CVVHDF withdrawal. Thyroid function was evaluated by determining the serum concentration of TSH, thyrotropin-releasing hormone (TRH), free triiodothyronine (fT3), free thyroxine (fT4), total triiodothyronine (tT3), total thyroxine (tT4) and reverse triiodothyronine (rT3). We additionally calculated the total activity of peripheral deiodinases (GD) using a mathematical model. Results: TRH and TSH levels remained mostly within normal ranges. fT4 and tT4 were in normal range or slightly below. In contrast, fT3 and tT3 were undetectably low in most patients throughout. Reverse T3 levels remained within normal limits. There were no statistically significant changes in any thyroid hormone levels over the CVVHDF treatment period. The calculated peripheral GD activity was lower than normal, but importantly, it did not change significantly over time. Conclusions: Thyroid hormones are not lost due to hemodiafiltration. Decreased deiodinases activity is responsible for alterations in serum concentrations of thyroid hormones in patients during CVVHDF. Full article

Objectives: Saccharomyces boulardii CNCM I-745, a probiotic yeast, is effectively used for the treatment of acute diarrhea as well as for the prevention and treatment of traveller‘s diarrhea and diarrhea under tube feeding. The underlying mechanisms are not fully elucidated. Both antitoxic and regulatory effects on the intestinal barrier, mediated either by the yeast or yeast-derived substrates, have been discussed. Methods: To examine the effects of Saccharomyces boulardii released substrates (S.b.S) on gastrointestinal (GI) barrier function, a murine small intestinal organoid cell model under stress was used. Stress was induced by lipopolysaccharide (LPS) exposure or withdrawal of growth factors from cell culture medium (GFRed). Stressed organoids were treated with S.b.S (200 µg/mL), and markers of GI barrier and inflammatory response were assessed. Results: GFRed-induced stress was characterized by disturbances in selected tight junction (TJ) (p < 0.05), adherent junction (AJ) (p < 0.001), and mucin (Muc) formation (p < 0.01), measured by gene expressions, whereby additional S.b.S treatment was found to reverse these effects by increasing Muc2 (from 0.22 to 0.97-fold change, p < 0.05), Occludin (Ocln) (from 0.37 to 3.5-fold change, p < 0.0001), and Claudin (Cldn)7 expression (from 0.13 ± 0.066-fold change, p < 0.05) and by decreasing Muc1, Cldn2, Cldn5, and junctional adhesion molecule A (JAM-A) expression (all p < 0.01). Further, S.b.S normalized expression of nucleotide binding oligomerization domain (Nod)2- (from 44.5 to 0.51, p < 0.0001) and matrix metalloproteinase (Mmp)7-dependent activation (from 28.3 to 0.02875 ± 0.0044 ** p < 0.01) of antimicrobial peptide defense and reduced the expression of several inflammatory markers, such as myeloid differentiation primary response 88 (Myd88) (p < 0.01), tumor necrosis factor α (Tnfα) (p < 0.01), interleukin (IL)-6 (p < 0.01), and IL-1β (p < 0.001). Conclusions: Our data provide new insights into the molecular mechanisms by which Saccharomyces boulardii CNCM I-745-derived secretome attenuates inflammatory responses and restores GI barrier function in small intestinal organoids. Drought stress is a major environmental constraint that significantly reduces wheat productivity worldwide. In this study, seventeen wheat genotypes were evaluated under well-watered and drought-stressed conditions across two consecutive years (2023–2024) in a controlled greenhouse experiment. Twenty morphological and agronomic traits were recorded, and their responses to prolonged water limitation were assessed using multivariate statistical methods, including three-way ANOVA, principal component analysis (PCA), and cluster analysis. Drought stress significantly decreased all traits except the harvest index (HI), with the most severe reductions observed in traits related to secondary spikes (e.g., grain weight reduced by 95%). The ANOVA results confirmed significant genotype × treatment (G × T) interactions for key agronomic traits, with the strongest effect observed for total grain weight (F = 7064.30, p < 0.001). A PCA reduced the 20 original variables to five principal components, explaining 87.2% of the total variance. These components reflected distinct trait groups associated with productivity, spike architecture, and development in phenology. Cluster analysis based on PCA scores grouped genotypes into three clusters with contrasting drought response profiles. A yield-based evaluation confirmed the cluster structure, distinguishing genotypes with a stable performance (average yield loss ~58%) from highly sensitive ones (~70% loss). Overall, the findings demonstrate that drought tolerance in wheat is governed by complex trait interactions. Integrating a trait-based multivariate analysis with a yield stability assessment enables the identification of genotypes with superior adaptation to water-limited environments, providing an excellent genotype background for future breeding efforts. The coastal developments in the Middle East put low priority on tsunami risk assessment due to the rare occurrence and absence of genuine tsunami track records on the coastline in the past. Tsunami-vulnerable coasts, including the east coast of the UAE, need to prepare for, and pay attention to, the impact of future tsunamis due to increased earthquake activity in the region. This study investigated the tsunami characteristics of the nearshore from hypothetical tsunami conditions by applications of numerical modeling and Artificial Neural Network (ANN) methods. The modeling results showed that the maximum tsunami depth at the shore was highest in Khor Fakkan and Mirbih for the given tsunami boundary conditions, while the tsunami withdrawal was greater on the southern bathymetry compared to that on the northern bathymetry when the tsunami period increased. ANN results confirmed that the still sea depth and seabed slope were more important than the tsunami period when predicting the maximum tsunami depth at the shore. Full article

A new series of 6-arylaminoflavones was synthesized via the Buchwald–Hartwig cross-coupling reaction, aiming to functionalize the flavone core efficiently. Reaction optimization revealed that Pd2(dba)3/XantPhos with Cs2CO3 in toluene provided the best yields, with isolated yields ranging from 8% to 95%, depending on the arylamine structure. Steric hindrance and electron-withdrawing groups at the arylamine ring impacted the reaction outcomes. Cytotoxicity assays in different human cancer cell lines indicated that substitution patterns at both the arylamine and B-rings strongly impacted biological activity. In particular, compounds bearing a 3,4-dimethoxy substitution at the B-ring and a trifluoromethyl (13c) or chlorine (13g) group at the aniline moiety exhibited enhanced cytotoxicity. These findings provide insights into the structure–activity relationship of 6-arylaminoflavones while contributing to the development of synthetic methodologies for functionalized flavones. This article examines the concept of ming 命 (mandate/command or fate/destiny) in the Analects, Mencius, and Zhuangzi, exploring its relationship to tian 天 (Heaven). Across these works, ming retains an intrinsic connection to tian—an inviolable cosmic force beyond human control. All three texts exhibit profound reverence and submission to tian, acknowledging the boundary between human control and cosmic inevitability, yet, at the same time, advocating active alignment with tian’s ordained patterns. In the Analects, a central tension emerges between tian’s teleological purpose—centered on preserving human culture and ethical cultivation—and the seemingly arbitrary fluctuations of individual fate, particularly regarding lifespan and personal fulfillment. This tension persists in the Mencius, articulated as a conflict between the political disorder of Mencius’ contemporary era and tian’s normative moral order. The Zhuangzi, by contrast, resolves this tension through advocating for withdrawal from the political life, as well as a radical reinterpretation of tian. Stripping tian off the Confucian moral–cultural imperatives, the text deconstructs dichotomies like life and death, championing inner equanimity via flowing with the cosmic transformation. This study investigates ground deformations in the southeastern Po Plain (northern Italy), focusing on the Bologna area—a densely populated region affected by natural and anthropogenic subsidence. Ground deformations in the area result from geological processes (e.g., sediment compaction and tectonic activity) and human activities (e.g., ground water production and underground gas storage—UGS). We apply a multidisciplinary approach integrating subsurface geology, ground water production, advanced differential interferometry synthetic aperture radar—DInSAR, gas storage data, and land use information to characterize and analyze the spatial and temporal variations in vertical ground deformations. Seasonal and trend decomposition using loess (STL) and cluster analysis techniques are applied to historical DInSAR vertical time series, targeting three representatives areas close to the city of Bologna. The main contribution of the study is the attempt to correlate the lateral extension of ground water bodies with seasonal ground deformations and water production data; the results are validated via knowledge of the geological characteristics of the uppermost part of the Po Plain area. Distinct seasonal patterns are identified and correlated with ground water production withdrawal and UGS operations. The results highlight the influence of superficial aquifer characteristics—particularly the geometry, lateral extent, and hydraulic properties of sedimentary bodies—on the ground movements behavior. This case study outlines an effective multidisciplinary approach for subsidence characterization providing critical insights for risk assessment and mitigation strategies, relevant for the future development of CO2 and hydrogen storage in depleted reservoirs and saline aquifers. Lung transplantation remains the standard of care for end-stage lung disease, yet a persistent gap exists between donor lung availability and growing clinical demand. Expanding the donor pool and optimising donor lung management are therefore critical priorities. However, no universally accepted management protocols are currently in place. This narrative review examines evidence-based strategies to improve lung utilisation across three donor categories: donors after brain death (DBD), controlled donors after circulatory death (cDCD), and uncontrolled donors after circulatory death (uDCD). A systematic literature search was conducted to identify interventions targeting lung preservation and function, including protective ventilation, recruitment manoeuvres, fluid and hormonal management, and ex vivo lung perfusion (EVLP). Distinct pathophysiological mechanisms—sympathetic storm and systemic inflammation in DBD, ischaemia–reperfusion injury in cDCD, and prolonged warm ischaemia in uDCD—necessitate tailored approaches to lung preservation. In DBD donors, early application of protective ventilation, bronchoscopy, and infection surveillance is essential. cDCD donors benefit from optimised pre- and post-withdrawal management to mitigate lung injury. uDCD donor lungs, uniquely vulnerable to ischaemia, require meticulous post-mortem evaluation and preservation using EVLP. Implementing structured, evidence-based lung management strategies can significantly enhance donor lung utilisation and expand the transplantable organ pool. The integration of such practices into clinical protocols is vital to addressing the global shortage of suitable lungs for transplantation. Full article

The optical floating zone technique was utilized to purify chromium and a single-phase chromium–silicon alloy in this work. The impurity content (carbon, nitrogen, and oxygen) can be reduced by decreasing the withdrawal speed of samples during the zone refining process, and the coarsening of grains was also observed. The effect of the impurities on mechanical properties was determined by hardness measurements at room temperature, and the hardness of both chromium and the chromium–silicon alloy decreased with lower concentrations of nitrogen and oxygen. In contrast, brittle material behavior is observed in samples prepared by arc melting process with higher concentrations of impurities. To use chromium–silicon alloys for future high-temperature applications, their brittle behavior must be improved, which can be achieved by reducing their carbon, nitrogen, and oxygen concentrations. In this study, three new terphenyl-substituted NPN ligands bearing pyridyl groups, two phosphonites and one diaminophosphine, were synthesized and fully characterized. Their coordination chemistry with copper(I) was investigated using CuBr and [Cu(NCMe)4]PF6 as metal precursors, affording six mononuclear Cu(I) complexes, which were characterized using NMR spectroscopy and, in selected cases, single-crystal X-ray diffraction (SCXRD) analysis. The NPN ligands adopt a κ3-coordination mode, stabilizing the copper centers in distorted tetrahedral geometries. The catalytic performance of these complexes in the S-arylation of thiols with aryl iodides was evaluated. Under optimized conditions, complexes 2a and 2b exhibited excellent activity and broad substrate scope, tolerating both electron-donating and electron-withdrawing groups, as well as sterically hindered and heteroaryl substrates. The methodology also proved effective for aliphatic thiols and demonstrated high chemoselectivity in the presence of potentially reactive functional groups. In contrast, aryl bromides and chlorides were poorly reactive under the same conditions. These findings highlight the potential of well-defined Cu(I)–NPN complexes as efficient and versatile precatalysts for C–S bond formation. Full article

In medical research, random censoring often occurs due to unforeseen subject withdrawals, whereas progressive censoring is intentionally applied to minimize time and resource requirements during experimentation. This work focuses on estimating the parameters of a two-parameter exponential distribution under a progressive Type-II random censoring scheme, which integrates both censoring strategies. The use of symmetric properties in failure and censoring time models, arising from a shared location parameter, facilitates a balanced and robust inferential framework. This symmetry ensures interpretational clarity and enhances the tractability of both frequentist and Bayesian methods. Maximum likelihood estimators (MLEs) are obtained, along with asymptotic confidence intervals. A Bayesian approach is also introduced, utilizing inverse gamma priors, and Gibbs sampling is implemented to derive Bayesian estimates. The effectiveness of the proposed methodologies was assessed through extensive Monte Carlo simulations and demonstrated using an actual dataset. Full article

Antibiotic residues in poultry pose health and resistance risks, necessitating breed-specific WDTs. In this study, the residue elimination patterns of seven antibiotics in Taihang chicken tissues under free-range conditions were studied and the appropriate WDT was formulated. A total of 240 healthy Taihang chickens aged 100 days were randomly divided into 8 groups, each comprising 30 chickens. Chickens in groups 1 to 7 were administered oxytetracycline, chlortetracycline, erythromycin, tylosin, tylvalosin, lincomycin, and tiamulin, respectively. Regarding the administration method, we adopted the highest dose and maximum course of treatment recommended by the Veterinary Pharmacopoeia of the People’s Republic of China. Group 8 served as the control group. Muscle, sebum, liver, and kidney samples were collected at 4 h, 1 d, 2 d, 3 d, 5 d, 7 d, 10 d, 13 d, and 16 d after drug withdrawal. Our results demonstrated that the drug residues after drug withdrawal gradually decreased with the increase in drug withdrawal days, and the elimination rate in the early stage of drug withdrawal was significantly faster than that in the later stage. At 4 h after drug withdrawal, the drug residues in various tissues reached their highest values. In most cases, the drug concentrations in the kidney and liver were higher than those in the muscles and sebum; however, some drugs also exhibited concentration peaks in the sebum. On the first day of drug withdrawal, the amount of residues in various tissues decreased rapidly. In general, the elimination rate of various drugs in the muscles, liver, and kidneys is faster but slower in the sebum. Based on the WDT calculation software WT1.4, the recommended WDTs for oxytetracycline, chlortetracycline, erythromycin, tylosin, tylvalosin, lincomycin, and tiamulin chickens are 4 d, 5 d, 11 d, 8 d, 13 d, 13 d, and 7 d, respectively. Acne vulgaris is a multifactorial inflammatory skin disorder that significantly impairs quality of life and may signal underlying systemic dysfunction, particularly in adult women with treatment-resistant or atypical presentations. This case series presents three clinically and etiologically distinct examples of persistent acne in female patients, each associated with different contributing factors: long-term topical corticosteroid misuse, polycystic ovary syndrome (PCOS), and metabolic syndrome with autoimmune thyroiditis. All cases underwent comprehensive dermatologic evaluation, endocrine/metabolic assessments, and personalized therapeutic interventions, ranging from corticosteroid withdrawal and barrier repair to hormonal modulation and insulin-sensitizing therapy. Clinical progression was monitored for up to six months, revealing favorable responses in all cases, with substantial lesion clearance and improved skin quality. These real-world cases highlight the importance of an integrative, interdisciplinary diagnostic approach in refractory acne and support the need for individualized, long-term management strategies tailored to underlying systemic contributors. Background and Clinical Significance: Juvenile ossifying fibroma (JOF) is a rare, benign, but locally aggressive fibro-osseous neoplasm that primarily affects the craniofacial skeleton of children and adolescents. Early surgical intervention is often required due to the lesion’s rapid growth and potential for significant facial deformity. Long-term functional and esthetic rehabilitation following maxillary resection in early childhood remains a clinical challenge. Case Presentation: This case reports a unique long-term follow-up of a 22-year-old female patient who underwent partial maxillary resection at the age of five due to JOF. Initial reconstructive efforts failed, necessitating a removable prosthesis to restore function and appearance. The patient experienced persistent self-consciousness and social withdrawal during adolescence, attributed to altered facial esthetics and repeated surgical disappointment. Nevertheless, prosthetic rehabilitation significantly improved mastication, phonetics, facial symmetry, and psychological well-being. Conclusions: The enduring psychosocial and functional impact of early maxillary resection for JOF and the pivotal role of prosthodontic management in long term rehabilitation are highlighted. A multidisciplinary approach that includes psychological support is suggested. Four entire male sugar gliders (Petaurus breviceps) belonging to the same colony were presented for elective orchiectomy. After clinical examination, dexmedetomidine (120 μg/kg) in combination with ketamine (5 mg/kg) were administered subcutaneously (SC). Once righting and pedal withdrawal reflexes were lost, ringer lactate solution, enrofloxacin and meloxicam were administered SC and a bilateral intratesticular block with lidocaine 0.25% was performed. Heart, respiratory rates and pulse oximetry values were recorded every minute. Onset of sedation, additional use of isoflurane, duration of anaesthesia, duration of surgery, time of recovery after atipamezole administration, quality of recovery and time of food intake were recorded. Postoperative assessment (posture, level of activity, vocalisation, response to manipulation, attention to the surgical wound) was performed hourly until discharge, five hours after surgery. Dexmedetomidine in combination with ketamine provided adequate short-lasting anaesthesia for castration in 3 out of 4 sugar gliders. One sugar glider needed additional isoflurane administration to perform orchiectomy. No perioperative additional analgesia was needed in any sugar glider. Full article
