Validation of KIDMED 2.0 PL—Mediterranean Diet Quality Index for Polish Children and Adolescents

Background: The Mediterranean diet is widely recognised for its health benefits and remains a key reference point in shaping dietary guidelines across populations. Despite its growing international relevance, there is a lack of validated tools assessing Mediterranean diet adherence among children and adolescents in Central and Eastern Europe. Methods: The present study aimed to adapt and validate the KIDMED 2.0 questionnaire for use in Polish children and adolescents aged 10 to 18 years (KIDMED 2.0 PL). The adaptation process involved forward–backward translation, expert consultations, and pilot testing to ensure linguistic and cultural relevance. A total of 102 participants completed the questionnaire twice over a two-week interval, and anthropometric data were collected. Results: The KIDMED 2.0 PL demonstrated high test–retest reliability (Spearman’s ρ = 0.876; p < 0.001) and strong criterion validity, with a significant negative correlation between KIDMED scores and BMI centile (ρ = −0.854; p < 0.001). Children with normal weight showed the highest adherence to the Mediterranean diet, while scores were significantly lower in overweight and obese participants. Item-level analysis indicated that fruit and vegetable consumption was relatively frequent, whereas intake of legumes, whole grains, and extra virgin olive oil remained low. Conclusions: The KIDMED 2.0 PL is a valid and reliable tool for evaluating diet quality and Mediterranean dietary adherence in the Polish pediatric population. Full article

Background: Maintaining thermal homeostasis is a basic function of the human body. This homeostasis depends largely on the body’s nutritional status and other conditions related to it. Aim: The present study investigated the impact of 8 days of water-only fasting (8DW-F) on selected features of thermal homeostasis, taking into account somatic, metabolic, and circulatory changes in middle-aged men. Methods: A total of 13 healthy men took part in the experiment. Volunteers were examined twice: after a mixed diet (C) and after using 8DW-F. At baseline, the following were recorded: body mass (BM), body fat (FM), fat-free mass (FFM), and total water (TBW), along with basal metabolic rate (BMR) and body surface area (BSA). Then, after 30 min of sitting under thermoneutral conditions, the following measurements were taken: eardrum temperature (Ti), skin temperatures (Tsk), heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP), oxygen uptake (VO2), and respiratory exchange ratio (RER). The following were then calculated: average body (MTB) and skin temperature (MTsk), resting metabolic rate (RMR), body to skin temperature gradient (g), and whole-body thermal conductivity (C). Results: The results showed that 8DW-F cause a significant reduction in most somatic variables as well as SBP and BMR (p < 0.001), RMR (p < 0.05) with no changes in Ti, MTsk, MTB, or C and g (p = 0.09). There were also significant correlations between Δ MTB × Δ BMR (p < 0.05) and Δ RMR × Δ VO2 (p < 0.001). Moreover, changes in the C range correlated with Δ RMR (p < 0.005) and Δ DBP (p < 0.05). Conclusions: 8DW-F reduced resting metabolic heat production in the studied men, but sufficient heat conservation ensured that thermal homeostasis was maintained under thermally neutral conditions. While the Ankom RF system facilitates efficient high-throughput in vitro fermentation studies, its high cost and limited flexibility constrain its broader applicability. To address these limitations, we developed and validated a low-cost, modular gas monitoring system (FerME), assembled from commercially available components. To evaluate its performance and reproducibility relative to the Ankom RF system (Ankom Technology, Macedon, NY, USA), in vitro rumen fermentation experiments were conducted under strictly controlled and identical conditions. Whole rumen contents were collected approximately 2 h post-feeding from individual mid- or late-lactation dairy cows and immediately transported to the laboratory. Each fermenter received 50 mL of processed rumen fluid, 100 mL of anaerobically prepared artificial saliva buffer, and 1.2 g of the donor cow’s diet. Bottles were sealed with the respective system’s pressure sensors, flushed with CO2, and incubated in a 50 L water bath maintained at 39 °C. FerME (New Taipei City, Taiwan) and Ankom RF fermenters were placed side-by-side to ensure uniform thermal conditions. To assess the effect of filter bag use, an additional trial employed Ankom F57 filter bags (Ankom Technology, Macedon, NY, USA; 25 μm pore size). Trial 1 revealed no significant differences in cumulative gas production, volatile fatty acids (VFAs), NH3-N, or pH between systems (p > 0.05). However, the use of filter bags reduced gas output and increased propionate concentrations (p < 0.05). Trial 2, which employed filter bags in both systems, confirmed comparable results, with the FerME system demonstrating improved precision (CV: 4.8% vs. 13.2%). Gas composition (CH4 + CO2: 76–82%) and fermentation parameters remained consistent across systems (p > 0.05). Importantly, with 12 pressure sensors, the total cost of FerME was about half that of the Ankom RF system. Collectively, these findings demonstrate that FerME is a reliable, low-cost alternative for real-time rumen fermentation monitoring and could be suitable for studies in animal nutrition, methane mitigation, and related applications. Autism spectrum disorder (ASD) is a complex neurodevelopmental condition with early-life origins. Maternal obesity has been associated with increased ASD risk, yet the mechanisms and timing of susceptibility remain unclear. Using a mouse model combining in vitro fertilization (IVF) and embryo transfer, we separated the effects of pre-conception and gestational obesity. We found that maternal high fat diet (HFD) exposure prior to conception alone was sufficient to induce ASD-like behaviors in male offspring—including altered vocalizations, reduced sociability, and increased repetitive grooming—without anxiety-related changes. These phenotypes were absent in female offspring and those exposed only during gestation. Cortical transcriptome analysis revealed dysregulation and isoform shifts in genes implicated in ASD, including Homer1 and Zswim6. Whole-genome bisulfite sequencing of hippocampal tissue showed hypomethylation of an alternative Homer1 promoter, correlating with increased expression of the short isoform Homer1a, which is known to disrupt synaptic scaffolding. This pattern was specific to mice with ASD-like behaviors. Our findings show that pre-conceptional maternal obesity can lead to lasting, isoform-specific transcriptomic and epigenetic changes in the offspring’s brain. These results underscore the importance of maternal health before pregnancy as a critical and modifiable factor in ASD risk. Full article

Background: The Mediterranean-DASH Intervention for Neurodegenerative Delay (MIND) was a 3-year, multicenter, randomized controlled trial to test the effects of the MIND diet on cognitive decline in 604 individuals at risk for Alzheimer’s dementia. Here, we describe the genotyping, imputation, and quality control (QC) procedures for the genetic data of trial participants. Methods: DNA was extracted from either whole blood or serum, and genotyping was performed using the Infinium Global Diversity Array. Established sample and SNP QC procedures were applied to the genotyping data, followed by imputation using the 1000 Genomes Phase 3 v5 reference panel. Results: Significant study-site, specimen type, and batch effects were observed. A total of 494 individuals of inferred European ancestry and 58 individuals of inferred African ancestry were included in the final imputed dataset. Evaluation of the imputed APOE genotype against gold-standard sequencing data showed high concordance (98.2%). We replicated several known genetic associations identified from previous genome-wide association studies, including SNPs previously linked to adiponectin (rs16861209, p = 1.5 × 10−5), alpha-linolenic acid (rs174547, p = 1.3 × 10−7), and alpha-tocopherol (rs964184, p = 0.003). Conclusions: This dataset represents the first genetic resource derived from a dietary intervention trial focused on cognitive outcomes. It enables investigation of genetic contributions to variability in cognitive response to the MIND diet and supports integrative analyses with other omics data types to elucidate the biological mechanisms underlying cognitive decline. These efforts may ultimately inform precision nutrition strategies to promote cognitive health. Full article

Nephrogenic diabetes insipidus (NDI) is a rare hereditary disorder characterized by renal resistance to arginine vasopressin (AVP), resulting in the kidneys’ inability to concentrate urine. Approximately 90% of NDI cases follow an X-linked inheritance pattern and are associated with pathogenic variants in the AVPR2 gene, which encodes the vasopressin receptor type 2. The remaining 10% are attributed to mutations in the AQP2 gene, which encodes aquaporin-2, and may follow either autosomal dominant or recessive inheritance patterns. We present the case of a male infant, younger than nine months of age, who was clinically diagnosed with NDI at six months. The patient presented recurrent episodes of polydipsia, polyuria, dehydration, hypernatremia, and persistently low urine osmolality. Despite adjustments in pharmacologic treatment and strict monitoring of urinary output, the clinical response remained suboptimal. Given the lack of improvement and the radiological finding of an absent posterior pituitary (neurohypophysis), the possibility of coexistent central diabetes insipidus (CDI) was raised, prompting a therapeutic trial with desmopressin. Nevertheless, in the absence of clinical improvement, desmopressin was discontinued. The patient’s management was continued with hydrochlorothiazide, ibuprofen, and a high-calorie diet restricted in sodium and protein, resulting in progressive clinical stabilization. Whole-exome sequencing identified a novel homozygous missense variant in the AQP2 gene (c.398T > A; p.Val133Glu), classified as likely pathogenic according to the American College of Medical Genetics and Genomics (ACMG) criteria: PM2 (absent from population databases), PP2 (missense variant in a gene with a low rate of benign missense variation), and PP3 (multiple lines of computational evidence supporting a deleterious effect)]. NDI is typically diagnosed during early infancy due to the early onset of symptoms and the potential for severe complications if left untreated. In this case, although initial clinical suspicion included concomitant CDI, the timely initiation of supportive management and the subsequent incorporation of molecular diagnostics facilitated a definitive diagnosis. The identification of a previously unreported homozygous variant in AQP2 contributed to diagnostic confirmation and therapeutic decision-making. The diagnosis and comprehensive management of NDI within the context of polyuria-polydipsia syndrome necessitates a multidisciplinary approach, integrating clinical evaluation with advanced molecular diagnostics. The novel AQP2 c.398T > A (p.Val133Glu) variant described herein was associated with early and severe clinical manifestations, underscoring the importance of genetic testing in atypical or treatment-refractory presentations of diabetes insipidus. Mycotoxin Assessment in Minimally Processed Traditional Ecuadorian Foods

Nowadays, there is special interest in promoting the consumption of ancestral crops and minimally processed foods with high nutritional value. However, besides nutritional issues, safety assessments must be addressed. This study aimed to evaluate mycotoxin contamination in five minimally processed traditional Ecuadorian foods: ochratoxin A (OTA), fumonisin B1 (FB1), and aflatoxins (AFs) in brown rice, lupin, and quinoa; OTA, FB1, and deoxynivalenol (DON) in whole-wheat flour; and OTA and AFs in peanuts. Samples (45 samples of peanuts and whole-wheat flour, 47 of brown rice, 46 of quinoa, and 36 of lupin) were collected from local markets and supermarkets in the three most populated cities in Ecuador. Mycotoxins were determined by RP-HPLC with fluorescence and detection. Results were compared with the maximum permitted levels (MPLs) of European Regulation 2023/915/EC. Overall contamination reached up to 59.8% of the analyzed samples (38.4% with one mycotoxin and 21.5% with co-occurrence). OTA was the most prevalent mycotoxin (in 82.6% of quinoa, 76.7% of whole-wheat flour, 53.3% of peanuts, 48.6% of lupin, and 25.5% of brown rice), and a modest number of quinoa (17%) and lupin (5.7%) samples surpassed the MPLs. DON was found in 82.2% of whole-wheat flour (28.9% > MPL). FB1 was detected in above 25% of brown rice and whole-wheat flour and in 9% of the quinoa samples. FB1 levels were above the MPLs only for whole-wheat flour (17.8%). AFB1 and AFG1 showed similar prevalence (about 6.5 and 8.5%, respectively) in quinoa and rice and about 27% in peanuts. Overall, these findings underscore the importance of enhancing fungal control in the pre- and post-harvest stages of these foods, which are recognized for their high nutritional value and ancestral worth; consequently, the results present key issues related to healthy diet promotion and food sovereignty. This study provides compelling insights into mycotoxin occurrence in minimally processed Ecuadorian foods and highlights the need for further exposure assessments by combining population consumption data. The Atlantic Diet (AD) is based on traditional dietary patterns in Galicia (northwestern Spain) and northern Portugal and is known for its health benefits. The AD focuses on fresh, local, and seasonal foods, especially fish, seafood, vegetables, legumes, whole grains, fruit, olive oil, and a moderate consumption of wine. However, it has received less attention from researchers than other dietary patterns. The present study had two main objectives: (i) to evaluate the dietary habits of a Galician population in relation to the AD and (ii) to create a numerical index to measure adherence to the AD. In 2022, a validated food frequency questionnaire was administered to 500 healthy adults living in Galicia. The data on participants’ dietary habits showed notable deviations from the ideal AD, especially regarding consumption of fruits, grains, and seafood. However, an adequate intake of legumes and nuts was observed, along with a reduction in the consumption of processed foods (except among younger participants) relative to that revealed in previous surveys. To assess adherence to the diet, statistical and chemometric analyses were applied, leading to the development of a new index: the Atlantic Diet Scale (ADS). The ADS was compared with three existing tools and proved to be a simple, flexible, and effective method for assessing dietary adherence based on optimal intake levels across food groups. When applied to dietary data, the ADS yielded adherence levels similar to two of the three traditional methods, with some differences relative to the third. These findings highlight the need for standardized evaluation tools, including clear definitions of food groups and consistent scoring systems, to better assess and promote adherence to the Atlantic Diet. Objective: Uncovering the renoprotective and anti-inflammatory effects of atorvastatin treatment in diabetic-and-obese rats by employing traditional renal function indicators (urea and creatinine) and four prototypical cytokines (IL-1β, il-6, IL-17α, TNFα). Method: Twenty-eight male Wistar rats, aged 6 months, 350–400 g, were randomized into four groups. The first group, G-I, the denominated control, were fed standard chow over the whole course of the experiments. The rodents in G-II were exposed to a High-Fat Diet. The last two groups were exposed to Streptozotocin peritoneal injection (35 mg/kg of body weight). A short biochemical assessment was performed before diabetes model induction to ensure appropriate glucose metabolism before experiments. Following model induction, only rodents in group G-IV were gradually introduced to the same High-Fat Diet as received by G-II. Model confirmation 10 days after injections marked the start of statin treatment in group G-IV, by daily gavage of atorvastatin 20 mg/kg of body weight/day for 21 days. At the end of the experiments, the biochemical profile of interest comprised typical renal retention byproducts (urea and creatinine) and the inflammatory profile described using plasma levels of TNFα, IL-17α, IL-6, and IL-1β. Results: Treatment with Atorvastatin was associated with a statistically significant improvement in renal function in G-IV compared to untreated diabetic rodents in G-III. Changes in inflammatory activity showed partial association with statin therapy, TNFα and IL-17α mirroring the trend in urea and creatinine values. Conclusions: Our results indicate that atorvastatin treatment yields a myriad of pleiotropic activities, among which renal protection was clearly demonstrated in this model of diabetic-and-obese rodents. The statin impact on inflammation regulation may not be as clear-cut, but the potential synergy of renal function preservation and partial tapering of inflammatory activity requires further research in severely metabolically challenged models. Plastic manufacturing involves using compounds that could be considered endocrine disruptors. Consequently, concern about the effect of these particles on the hormonal regulation of various systems, including the hypothalamic–pituitary–thyroid axis, has been increasing. By applying the Amphibian Metamorphosis Assay (AMA), the effects of irregular polystyrene microplastics (PS) MPs on the thyroid gland of Xenopus laevis were investigated. The histological effects on other organs of tadpoles were also studied. Tadpoles were exposed to 500 and 50 µg of virgin PS MP particles, (200 µm range)/L, administered by diet for 21 days. PS dietary exposure revealed statistically significant effects for the snout to vent length and the whole body length apical endpoints on day 21. The histological survey of both treatment groups revealed no noteworthy effects on the thyroid gland, digestive tract, or kidneys, but slight modifications to the liver. Mild ultrastructural modifications were detected in tadpoles’ enterocytes and hepatocytes in both treatment groups, but were likely to be reversible. Overall, our results contrast with previous research results in which effects were observed, but using different types, concentrations, and numbers of MPs. All this suggests the need for standardized methods for the environmental risk assessment of MPs/NPs (nanoplastics). Concern about the risk of NPs seems to be greater, and more studies with NP particles should be conducted. Full article

Acceptability of a Colorectal Cancer-Preventive Diet Promoting Red Meat Reduction and Increased Fiber and Micronutrient Intake: A Cross-Sectional Study in Romanian Adults

Background/Objectives: Colorectal cancer is a leading cause of cancer-related death worldwide, with rising incidence in younger adults. Unhealthy diets high in red and processed meat and low in fiber are key modifiable risk factors, highlighting the need for preventive nutritional strategies targeting CRC through dietary interventions. Methods: A one-day sample diet for colorectal cancer prevention, consisting of fiber-rich meals excluding red meat and incorporating whole grains, legumes, vegetables, fruits, nuts, and lean protein alternatives (such as fish and poultry), was developed. Its acceptability was assessed in a cross-sectional study using an online questionnaire among healthy Romanian adults aged 18–50, with a total of 395 included participants. Results: Of the 395 respondents meeting the inclusion criteria (aged 18–50, no cancer or chronic gastrointestinal disorders), 63.5% were females, predominantly urban (90.1%), and highly educated. Mean age was 32.4 years; mean BMI was 25.07 kg/m2. The proposed colorectal cancer-preventive diet was rated as “quite attractive” and “very attractive” by 74.9% of participants. All meals received high ratings, with dinner and the first snack being most favored. Most respondents (77.2%) found the diet satisfying and the satiety level and energy adequate, and 90.4% were willing to adopt it at least a few times per week. Financial accessibility was affirmed by 77.2% of the respondents. However, 61.8% reported difficulty eliminating red meat consumption. Female participants rated the diet significantly more attractive than males did (p = 0.041). Willingness to adopt the diet strongly correlated with higher acceptability (p < 0.0001), while BMI and education level showed no significant effect. Conclusions: The proposed colorectal cancer-preventive diet was well accepted by Romanian adults aged 18–50, with higher receptivity among women and those with higher education; willingness to adopt the diet at least a few days per week was high, especially among those psychologically ready for dietary change, while key barriers included red meat reduction and perceived cost, underscoring the need for gender-sensitive, culturally adapted interventions and further research on long-term adherence and clinical impact. Type 2 diabetes (T2D) represents a public health problem globally, with the highest prevalence reported among older adults. While an interplay of various determinants including genetic, epigenetic, environmental factors and unhealthy lifestyle, particularly diet, has been established to contribute to T2D development, emerging evidence supports the role of interactions between nutrients or dietary patterns and genes in the pathogenesis of this metabolic disorder. The amount, and especially the type of carbohydrates, in particular, have been correlated with the risk of non-communicable chronic disease and mortality. This narrative review aims to discuss the updated data on the complex and not fully elucidated relationship between carbohydrate–gene interactions and incidence of T2D, identifying the most susceptible genes able to modulate the dual association between carbohydrate intake and risk of developing T2D. The identification of genetic polymorphisms in response to this macronutrient represents a potentially powerful target to estimate individual risk and prevent the development of T2D in the context of personalized medicine. The postulation around novel foods potentially tailored to minimize the risks of developing T2D will pave the way for a new era into food research in relation to the safeguarding of well-being status in patients affected by, or at risk for, T2D. Dietary choices and patterns have enormous consequences along the lines of individual, community, and planetary health. Excess meat consumption has been linked to chronic disease risk, and at large scales, the underlying industries maintain a massive environmental footprint. For these reasons, public and planetary health experts are unified in emphasizing a whole or minimally processed plant-based diet. In response, the purveyors of ultra-processed foods have added “meat alternatives” to their ultra-processed commercial portfolios; multinational corporations have been joined by “start-ups” with new ultra-processed meat analogues. Here, in our Viewpoint, we revisit the 1990s food industry rhetoric and product innovation, a time in which multinational corporations pushed a great “low-fat transition.” We focus on the McLean Deluxe burger, a carrageenan-rich product introduced by the McDonald’s Corporation in 1991. Propelled by a marketing and media-driven fear of dietary fats, the lower-fat burger was presented with great fanfare. We reflect this history off the current “great protein transition,” a period once again rich in rhetoric, with similar displays of industry detachment from concerns about the health consequences of innovation. We scrutinize the safety of carrageenan and argue that the McLean burger should serve as a cautionary tale for planetary health and 21st century food innovation. Full article

This study was performed to investigate the effects of dietary yeast culture (YC) supplementation on growth performance, digestive function, intestinal inflammatory response, and microbiota composition of largemouth bass Micropterus salmoides (LMB). Six diets were formulated with graded levels of YC (0, 5, 10, 15, 20, and 30 g/kg), referred to as CON, YC5, YC10, YC15, YC20, and YC30, respectively. Each diet was assigned to four replicate tanks of LMB juveniles (initial body weight 8.11 ± 0.05 g) with twenty fish per tank. After an 8-week feeding trial, final body weight and specific growth rate showed an increasing trend with 5~20 g/kg YC and reached a maximum at 15 g/kg YC. Feeding ratio decreased, but feed efficiency ratio (FER) improved in response to dietary YC inclusion, and FER was higher in the YC10 fish than in the YC5, YC20, and YC30 fish. Proximate composition (moisture, protein, and lipid) of the whole fish was not affected by dietary YC levels. The activities of intestinal lipase and trypsin were higher in the YC10 fish, while the relative expression of interleukin-8 (il-8) and il-1β was downregulated in the hindgut of the YC15 fish compared with the CON fish. Histological examination showed that the villus height of the midgut, together with goblet cell density of the foregut and midgut, was higher in the YC10 and YC30 fish than in the CON fish. 16S rRNA sequencing showed that Proteobacteria, Fusobacteria, and Firmicutes dominated the intestinal microbiota in LMB. The decrease in harmful Mycoplasma accounted for the dramatic change in Firmicutes abundance, while the increase in Cetobacterium (specifically C. somerae) accounted for the change in Fusobacteria abundance in the gut of the YC10 and YC30 fish compared with the CON fish. The increase in the beneficial Endozoicomonas was the main reason for the change in Proteobacteria abundance in the intestine of the YC30 fish as compared with the CON fish. Taken together, the alteration of intestinal microbiota composition contributed to the improved digestive function and feed utilization in LMB fed YC-supplemented diets. Based on growth performance, the optimal YC level in the diet for LMB was 15 g/kg. Full article

Background/Objectives: Apolipoprotein E receptor-2 (apoER2) exists in various alternatively spliced forms, including variants that express apoER2 with or without exon 19 in the cytoplasmic domain. This study compared vascular response to endothelial denudation, as well as diet-induced atherosclerotic and metabolic diseases, between genetically modified mice that exclusively expressed the apoER2 splice variant with or without exon 19 to determine the impact of apoER2 exon 19 motif in cardiometabolic disease modulation. Methods: Vascular response to injury was assessed by measuring neointima area of the carotid arteries after endothelial denudation. The genetically modified mice were also fed a high-fat high-cholesterol diet for 16 weeks for the determination of body weight gain, glucose and insulin levels, glucose tolerance and insulin secretion. Additionally, adipose tissue inflammation was assessed by analysis of adipose gene expression, and atherosclerosis was characterized by measuring fatty lesion size in the whole aorta, as well as in the aortic roots. Results: The results showed that whereas the expression of either splice variant is sufficient to impede denudation-induced fibrotic neointima formation and complex necrotic atherosclerotic lesions, the expression of the apoER2 splice variant containing exon 19 is necessary for the complete protection of injury-induced neointima formation in the vessel wall. However, exclusive expression of either apoER2 cytoplasmic splice variant does not influence the early phase of atherogenesis. Additionally, the exclusive expression of apoER2 without exon 19 promotes adipocyte inflammation and accelerates diet-induced insulin resistance and glucose intolerance. Conclusions: These results indicate that the apoER2 cytoplasmic variants have distinct and cell type-specific roles in influencing cardiometabolic disease development. Full article
