Development and Construct Validation of a Food-Based Diet Quality Score Using Image-Based Food Records

Background/Objectives: Diet quality indices (DQIs) are commonly used to evaluate the alignment of dietary intake with national dietary guidelines. However, few methods have been developed to apply DQIs to image-based dietary assessment tools. This study aimed to develop a novel food-based Diet Quality Score (DQS) and examine its construct validity in Australian adults aged 18–65 years living with a higher weight (BMI 30–45 kg/m2), using the validated Healthy Eating Index for Australians 2013 (HEIFA-2013) for comparison. Methods: This cross-sectional study assessed dietary intake in 260 adults (mean age 48 ± 12 years; BMI 35.0 ± 4.2) collected over 4 days using the mobile Food Record (mFR™) and scored using the HEIFA-2013 and DQS (both scored out of 100). Descriptive statistics summarised participant characteristics with DQS and HEIFA-2013 scores. Construct validity was examined by assessing the associations between sociodemographic, health behaviour, anthropometric, and clinical blood marker variables and DQS and HEIFA-2013 scores. Multivariable binary logistic regression identified variables associated with being in the lowest diet quality tertile for both DQIs. Results: The mean scores were 47.4 ± 8.7 for the DQS and 52.0 ± 8.6 for HEIFA-2013. Both scores were positively associated with age (DQS: r = 0.216, p < 0.001; HEIFA-2013: r = 0.265, p < 0.001) and attention paid to the health aspects of their diet (DQS: p < 0.023; HEIFA-2013: p < 0.010). Increasing age (OR = 0.958, 95% CI: 0.932–0.985, p = 0.003) and moderate versus low physical activity (OR = 0.40, 95% CI: 0.21–0.80, p = 0.009) decreased odds of being in the lowest diet quality tertile for both DQI scores. Conclusions: The DQS demonstrated acceptable construct validity, providing a valid method for using image-based dietary assessments to evaluate diet quality in Australian populations. Full article

Objectives: In Japan, smartphone applications are increasingly used for dietary recording in healthcare settings. This study aimed to examine the agreement between energy and nutrient intake estimates obtained using an artificial intelligence (AI)-based dietary recording application and those obtained using the weighed dietary record (WDR). Methods: The AI-based dietary recording method (FoodLog Athl method) was compared with WDR. Thirty-six university students (35 women and 1 man) simultaneously recorded their dietary intake using FoodLog Athl (FLA) and the WDR for 10 consecutive days. Energy and nutrient intakes were estimated using each method, and correlations and agreement between the two methods were evaluated. Results: Significant positive correlations were observed between the two methods for energy and most nutrients, except for iron, vitamin B1, and sodium chloride equivalent (p < 0.01). Compared with the WDR, the FLA method showed systematic overestimation of energy and major macronutrients (protein, fat, and carbohydrate) and underestimation of total dietary fiber. Bland–Altman analysis indicated fixed bias and relatively wide limits of agreement for several nutrients. Conclusions: The FLA method demonstrated moderate agreement with the WDR, with systematic bias observed for selected nutrients. These findings suggest that the application may be useful for monitoring overall dietary trends or relative intake over time, but caution is warranted when precise individual-level nutrient quantification is required. Professional review by registered dietitians may help improve estimation accuracy and reduce bias. Full article

Background/Objectives: Excessive social media usage in the current times and high rates of food advertising can impact the health status of individuals by increasing food cues related to perceived hunger and, thus, dietary behaviour. This study examined the association between social media usage patterns, food-related advertising, and dietary choices among Gen Z individuals. Methods: A cross-sectional study was carried out amongst 314 young adults between the ages of 18 and 25 in Surat city, Gujarat. Data was collected for social media usage, the most used platforms, preferred content, and eating patterns. Anthropometric measurements (height and weight) were also recorded. Perceived hunger responses to 12 social media-based food images were assessed using a ten-point Visual Analogue Scale (VAS). Statistical analyses were performed using SPSS (version 26.0), with the significance level set at p < 0.05. Results: YouTube and Instagram were the most used social media apps. There were no significant differences observed between the BMI of participants using social media for 2 h a day and those using it 3+ hours a day. However, a significant association between the BMI of those who viewed advertisements for ready-to-eat foods (p = 0.004) and the BMI of those who viewed advertisements for food delivery platforms (p = 0.001) was seen. A significant difference between usage of Pinterest (p = 0.02), Instagram (p = 0.047), and BMI was also found. Conclusions: Social media marketing and food content are shaping the dietary choices of young adults, and more studies need to be conducted in Pan India to understand the reasons. Such evidence will be crucial for guiding nutrition policies, digital marketing regulations, and youth-focused awareness programmes. Full article

Background/Objectives: Coupling mobile health (mHealth) technology with community-based nutrition programs may enhance diet quality in adolescents. This pilot study evaluated the feasibility, acceptability, and preliminary efficacy of integrating PortionSize Ed (PSEd), an image-assisted dietary assessment and education app, into the six-week Hawaii Food and Lifeskills for Youth (HI-FLY) curriculum delivered via Supplemental Nutrition Assistance Program Education (SNAP-Ed). Methods: Adolescents (grades 6–8) from two classrooms were cluster-randomized into HI-FLY or HI-FLY + PSEd. Both groups received HI-FLY and completed Youth Questionnaires (YQ) and food records (written or app-based) at Weeks 0 and 7. Feasibility and acceptability were assessed via enrollment, attrition, and User Satisfaction Surveys (USS). Diet quality was measured using Healthy Eating Index-2020 (HEI-2020) scores and analyzed via mixed-effects models. Results: Of 50 students, 42 (84%) enrolled and attrition was minimal (2.4%). The sample was 49% female and 85% at least part Native Hawaiian or Pacific Islander (NHPI). PSEd was acceptable, with average USS scores above the scale midpoint. No significant HEI-2020 changes were observed, though YQ responses indicated improvements in sugary drink intake (p = 0.03) and use of nutrition labels in HI-FLY + PSEd (p = 0.0007). Conclusions: Integrating PSEd into SNAP-Ed was feasible, acceptable, and demonstrated potential healthy behavior change among predominantly NHPI youth in Hawaii. Full article

Background: The rising prevalence of metabolic-dysfunction-associated steatotic liver disease (MASLD) is a significant health challenge, and the consumption of ultra-processed foods (UPFs) could play a key role. Aim: The aim is assess the impact of UPF consumption changes on the development and progression of MASLD in adults. Design: This is a longitudinal study to assess how changes in UPF consumption affect liver fat and MASLD parameters over 6 months in 70 participants. Methods: Dietary intake was assessed using a validated food frequency questionnaire, and foods were classified according to the NOVA system. Participants were divided into three groups based on UPF consumption changes: maximum (T1), medium (T2), and minimum reduction (T3). Fatty liver parameters were assessed with magnetic resonance imaging and ultrasonography. Mediterranean diet (Med-diet) adherence and sociodemographic parameters were also recorded. The General Linear Model was used to determine relationships between UPF consumption, fatty liver disease parameters, and diet. Results: Participants in T1 experienced a 7.7% reduction in intrahepatic fat content (IFC) compared to 2.6% in T3. T1 showed increased Med-diet adherence and decreased meat and sweets consumption. The energy intake decreased by 605.3 kcal/day in T1, while T3 showed an increase of 209.5 kcal/day. Conclusions: Reducing UPF consumption leads to a decrease in IFC, associated with high Med-diet adherence and low calorie intake. Adopting these dietary patterns aligns with global sustainability goals and could further benefit MASLD patients by addressing environmental challenges alongside improving liver health. Full article

Background/Objectives: Chronic severe neuropathic pain (NP) affects one in 10 individuals over the age of 30 in North America. Standard pharmacological interventions are associated with significant side effects and have limited effectiveness. Diets seeking to improve physiological health, support gut barrier integrity, and decrease systemic inflammation have recently emerged as powerful tools conferring neuroprotective and anti-inflammatory effects, potentially reducing the overall morbidity and mortality of multiple neurological and metabolic diseases. This systematic review aimed to synthesize the literature around NP outcomes following dietary interventions compared to routine standard of care. Methods: Following PRISMA guidelines, an initial search yielded 15,387 records after deduplication. Six interventional trials specifically assessing dietary interventions for neuropathic pain were included and analyzed. The dietary lifestyle interventions included low-fat plant-based, plant-based fasting-mimicking, low-calorie, potassium-reduced, gluten-free, and intermittent high-protein/Mediterranean diets. Results: The included studies described some statistically significant improvements in pain severity on objective quantitative sensory testing, electrophysiology, imaging, and subjective questionnaires. The overall risk of bias was moderate, with only one trial demonstrating a low risk of bias across all assessed domains. No serious adverse events were identified, and dietary interventions were generally well tolerated. Conclusions: The data collected and synthesized in this systematic review indicate that dietary lifestyle interventions may offer a low-risk, low-cost, low-tech option for chronic neuropathic pain management, potentially improving quality of life and reducing overall morbidity. However, given substantial variability across studies and a moderate risk of bias, further research is warranted to substantiate these findings. Full article

Technology-assisted dietary assessment has the potential to improve the accuracy of self-reported dietary intake. This study evaluates MealScan3D (MS3D), a mobile device-based food recording system, which uses three-dimensional images to obtain food volumes and an application to capture algorithm-driven food intake data. Participants (n = 179) were randomly assigned and trained to record three meals using either MS3D or a written food record (WFR). Generous amounts of standardized meals were provided, and participants self-selected portions for each food. The weights of provided and uneaten/leftover foods were used to determine true intake. For total energy intake (three meals combined), validity (Pearson correlation) was significantly higher for MS3D vs. the WFR (p < 0.001); when interpreted as the percentage of variance in energy intake explained, MS3D explained 84.6% of true variance, a 25.3% absolute and 42.6% relative increase over the 59.3% explained by the WFR. For 9 of 15 individual foods, the Pearson correlations between true and reported portion size estimates were significantly larger for MS3D than the WFR. Bias was smaller (intercepts were closer to the means) for 9 of 15 foods and the regression coefficients for 10 of 15 foods were significantly closer to 1.0 in the MS3D arm. MS3D is feasible for dietary assessment and may provide improvements in accuracy compared to WFRs. Full article

Background: Accurate estimation of dietary intake is challenging. However, whilst some progress has been made in high-income countries, low- and middle-income countries (LMICs) remain behind, contributing to critical nutritional data gaps. This study aimed to validate an objective, passive image-based dietary intake assessment method against weighed food records in London, UK, for onward deployment to LMICs. Methods: Wearable camera devices were used to capture food intake on eating occasions in 18 adults and 17 children of Ghanaian and Kenyan origin living in London. Participants were provided pre-weighed meals of Ghanaian and Kenyan cuisine and camera devices to automatically capture images of the eating occasions. Food images were assessed for portion size, energy, nutrient intake, and the relative validity of the method compared to the weighed food records. Results: The Pearson and Intraclass correlation coefficients of estimates of intakes of food, energy, and 19 nutrients ranged from 0.60 to 0.95 and 0.67 to 0.90, respectively. Bland–Altman analysis showed good agreement between the image-based method and the weighed food record. Under-estimation of dietary intake by the image-based method ranged from 4 to 23%. Conclusions: Passive food image capture and analysis provides an objective assessment of dietary intake comparable to weighed food records. Full article

Image-based dietary records have been validated as tools to evaluate dietary intake. However, to determine meal timing, previous studies have relied primarily on image-based smartphone applications without validation. Noteworthy, the validation process is necessary to determine how accurately a test method measures meal timing compared with a reference method over the same time period. Thus, we aimed to assess the relative validity and reliability of the Remind® app as an image-based method to assess dietary intake and meal timing. For this purpose, 71 young adults (aged 20–33 years, 81.7% women) were recruited for a 3-day cross-sectional study, where they completed a 3-day image-based record using the Remind app (test method) and a 3-day handwritten food record (reference method). The relative validity of the test method versus the reference method was assessed using multiple tests including Bland–Altman, % difference, paired t-test/Wilcoxon signed-rank test, Pearson/Spearman correlation coefficients, and cross-classification. We also evaluated the reliability of the test method using an intra-class correlation (ICC) coefficient. The results showed that, compared to the reference method, the relative validity of the test method was good for assessing energy and macronutrient intake, as well as meal timing. Meanwhile, the relative validity of the test method to assess micronutrient intake was poor (p < 0.05) for some micronutrients (iron, phosphorus, potassium, zinc, vitamins B1, B2, B3, B6, C, and E, and folates) and some food groups (cereals and grains, legumes, tubers, oils, and fats). Regarding the reliability of an image-based method to assess dietary intake and meal timing, results ranged from moderate to excellent (ICC 95% confidence interval [95% CI]: 0.50–1.00) for all nutrients, food groups (except oils and fats, which had low to moderate reliability), and meal timings. Thus, the results obtained in this study provide evidence of the relative validity and reliability of image-based methods to assess dietary intake (energy, macronutrients, and most food groups) and meal timing. These results open up a new framework for chrononutrition, as these methods improve the quality of the data collected and also reduce the burden on users to accurately estimate portion size and the timing of meals. To prevent or deal with chronic diseases, using a smart device, automatically classifying food categories, estimating food volume and nutrients, and recording dietary intake are considered challenges. In this work, a novel real-time vision-based method for solid-volume food instance segmentation and calorie estimation is utilized, based on Mask R-CNN. In order to address the proposed method in real life, distinguishing it from other methods which use 3D LiDARs or RGB-D cameras, this work applies RGB images to train the model and uses a simple monocular camera to test the result. Gimbap is selected as an example of solid-volume food to show the utilization of the proposed method. Firstly, in order to improve detection accuracy, the proposed labeling approach for the Gimbap image datasets is introduced, based on the posture of Gimbap in plates. Secondly, an optimized model to detect Gimbap is created by fine-tuning Mask R-CNN architecture. After training, the model reaches AP (0.5 IoU) of 88.13% for Gimbap1 and AP (0.5 IoU) of 82.72% for Gimbap2. mAP (0.5 IoU) of 85.43% is achieved. Thirdly, a novel calorie estimation approach is proposed, combining the calibration result and the Gimbap instance segmentation result. In the fourth section, it is also shown how to extend the calorie estimation approach to be used in any solid-volume food, such as pizza, cake, burger, fried shrimp, oranges, and donuts. Compared with other food calorie estimation methods based on Faster R-CNN, the proposed method uses mask information and considers unseen food. Therefore, the method in this paper outperforms the accuracy of food segmentation and calorie estimation. The effectiveness of the proposed approaches is proven. Full article

There are limited methods to assess how dietary patterns adhere to a healthy and sustainable diet. The aim of this study was to develop a theoretically derived Healthy and Sustainable Diet Index (HSDI). The HSDI uses 12 components within five categories related to environmental sustainability: animal-based foods, seasonal fruits and vegetables, ultra-processed energy-dense nutrient-poor foods, packaged foods and food waste. A maximum of 90 points indicates the highest adherence. The HSDI was applied to 4-day mobile food records (mFRTM) from 247 adults (18–30 years). The mean HSDI score was 42.7 (SD 9.3). Participants who ate meat were less likely to eat vegetables (p < 0.001) and those who ate non-animal protein foods were more likely to eat more fruit (p < 0.001), vegetables (p < 0.05), and milk, yoghurt and cheese (p < 0.05). After adjusting for age, sex and body mass index, multivariable regression found the strongest predictor of the likelihood of being in the lowest total HSDI score tertile were people who only took a bit of notice [OR (95%CI) 5.276 (1.775, 15.681) p < 0.005] or did not pay much/any attention to the health aspects of their diet [OR (95%CI) 8.308 (2.572, 26.836) p < 0.0001]. HSDI provides a new reference standard to assess adherence to a healthy and sustainable diet. Full article

The ability to comprehensively assess the diet of infants is essential for monitoring adequate growth; however, it is challenging to assess dietary intake with a high level of accuracy. Infants rely on surrogate reporting by caregivers. This study aimed to determine if surrogate reporters (e.g., caregivers) could use an image-based mobile food record adapted (baby mFR) to record infants’ eating occasions, and via caregiver feedback, could assess the usability and feasibility of the baby mFR in recording infants’ diets. This was a cross-sectional study in which surrogate reporters (e.g., caregivers) recorded all food and beverage intake (including human milk) of the infant over a 4-day period. Trained research staff evaluated all images submitted during data collection for different indicators of quality. All surrogate reporters were asked to complete a usability questionnaire at the end of the 4-day data collection period. Basic descriptive analyses were performed on the infants 3–12 months of age (n = 70). A total of 91% (n = 64) of surrogate reporters used the baby mFR to record their infants’ eating occasions. The mean number of images submitted daily per participant via the mFR was 4.2 (SD 0.2). A majority of submitted images contained the fiducial marker and the food and/or beverage was completely visible. The mFR was found to be easy to use; however, suggestions were provided to increase utility of the application such as the inclusion of a bottle button and reminders. An image-based dietary assessment method using a mobile app was found to be feasible for surrogate reporters to record an infant’s food and beverage intake throughout the day. Full article

Due to global advances in technology, image-based food record methods have emerged as an alternative to traditional assessment methods. The use of image-based food records in low and lower-middle income countries such as Tanzania is limited, with countries still using traditional methods. The current study aimed to determine the feasibility of using a new voice and image-based dietary assessment system (VISIDA) in Dar es Salaam, Tanzania. This mixed-method study recruited 18 nutritionists as participants who collected image-based records of food and drinks they consumed using the VISIDA smartphone app. Participants viewed an online demonstration of the VISIDA web platform and the analysis process for intake data collected using the VISIDA app. Then, participants completed an online survey and were interviewed about the VISIDA app and web platform for food and nutrient intake analysis. The method was reported as being acceptable and was found to be easy to use, although technical challenges were experienced by some participants. Most participants indicated a willingness to use the VISIDA app again for one week or longer and were interested in using the VISIDA system in their current role. Participants acknowledged that the VISIDA web platform would simplify some aspects of their current job. Image-based food records could potentially be used in Tanzania to improve the assessment of dietary intake by nutritionists in urban areas. Participants recommended adding sound-on notifications, using the VISIDA app in both Apple and Android phones, enabling installation from the app store, and improving the quality of the fiducial markers. Public health efforts to reduce diet-related health disparities experienced by indigenous peoples could be enhanced by efforts to improve complementary infant feeding practices. The latter is possible through interventions informed by cultural determinants. This cross-sectional secondary analysis explored possible determinants of the complementary feeding practices of Native Hawaiian, Pacific Islander, and/or Filipino infants (NHPIF) in Hawaiʻi, ages 3–12 months. The objective was to determine the association between caregiver cultural identity and infant household membership with indicators of infant diet healthfulness. The cultural identities, infant household memberships, early infant feeding practices and additional demographic information (infant age and sex, household income) were assessed via an online questionnaire. Surrogate reporting of the infants’ diets over four days was evaluated using an image-based mobile food record (mFR). Data collected by the mFR were evaluated to derive the World Health Organization’s minimum dietary diversity (MDD) indicator and food group consumption. Data were summarized by descriptive statistics and analyzed using multivariate linear and logistic regressions. Seventy infant participants, ages 3–12 months, and their primary caregivers completed the study. Of these, there were 56 infant participants between the age of 6–12 months. Approximately 10% of infants, ages 6–12 months, met MDD for all four days. Meeting MDD and the number of food groups consumed were significantly associated with age. Caregiver cultural identity, infant household membership and infant sex had non-significant associations with indicators of infant diet quality. Findings inform the influences shaping dietary patterns of Native Hawaiian, Pacific Islander and Filipino infants in Hawaiʻi. Full article

The present study aimed to examine associations between body image and under-reporting in female Japanese university students enrolled in a nutrition degree program. A total of 100 participants (aged 18–29 years) completed (1) a self-administered questionnaire including the Ben-Tovim Walker Body Attitudes Questionnaire (BAQ), (2) a dietary assessment using a brief-type self-administered diet history questionnaire (BDHQ), (3) a physical activity assessment using Bouchard’s Physical Activity Record (BAR) and a tri-axial accelerometer, (4) detailed anthropometry, and (5) body composition assessment. Based on the energy intake to basal metabolic rate ratio (EI:BMR) and using a cut-off point of 1.35, 67% of participants were considered under-reporters (URs). While there was no between-group difference in BMI, URs had significantly (p < 0.05) greater percentage body fat (%BF) and trunk fat (%TF) compared with non-URs. Regression analyses indicated accuracy of body perception and a discrepancy between current and ideal weight were associated with EI:BMR, whereas the salience subscale of the BAQ was associated with reported EI. The study raises concerns regarding the validity of EI reported from young Japanese females as they are known to have a strong preoccupation with thinness, even with an acceptable BMI and health and nutritional knowledge.
