Asthma and seasonal allergic rhinitis are common conditions affecting health, wellbeing, and work place productivity. Current pollen monitoring requires experienced technicians and has a lag of at least 24 h, which limits usefulness for patient management. We evaluated agreement of daily pollen concentrations from an automated real-time sampler, the Swisens Poleno Jupiter, with a traditional Hirst-type trap using technician counting over a single monitoring season in England. We used regression models to assess the impact of weather covariates on differences. Total pollen showed a strong correlation (r = 0.74) but moderate Intraclass Correlation (ICC = 0.52) between devices, due to concentrations (total and taxa level) being higher for the manual than the automated sampler. For individual pollen taxa, strong correlation and good agreement was observed for Pinus, Alnus and Fraxinus throughout the monitoring period. Strong correlation but low/moderate agreement was observed for Poaceae (grass), Betula, Quercus and Taxus/Cupressus. No correlation or agreement was observed for Corylus and Platanus. Temperature explained most of the variability between samplers. We conclude that daily total pollen can be tracked using an automated Poleno, as can grass within its flowering season, but Poleno recognition algorithms developed for mainland Europe need further refinements for UK species. Improving agreement in quantification is of particular importance given that current allergenic thresholds are based on manual readings. Full article

Pollen exposure drives allergic disease in millions of Americans, yet no standardized, publicly available national pollen dataset has existed until now. We describe the first nationally standardized and publicly available dataset of pollen season timing and airborne pollen concentrations. The data were derived from the National Allergy Bureau™ (NAB™), the only pollen and mold measuring network in the United States certified by the American Academy of Allergy, Asthma & Immunology (AAAAI), and curated, processed, and disseminated by the U.S. Centers for Disease Control and Prevention (CDC). The 2003–2024 dataset provides standardized measures of (1) taxa-specific historical average main pollen season (MPS) concentrations and timing (e.g., start dates, peak dates, end dates, season length); (2) taxa-specific yearly MPS concentrations and timing; (3) grouped weekly MPS concentrations, levels, and timing; and (4) grouped daily pollen levels and MPS timing. Pollen concentrations are reported as pollen grains per cubic meter (PPCM). MPS timing is computed using a 3-day consecutive method: season start occurs after the first occurrence of three consecutive days with concentrations > 1.0 PPCM; season peak is the day of maximum concentration; and season end occurs after the first occurrence of three consecutive days with concentrations < 1.0 PPCM after the peak. Historical average timing is calculated in a 365-day-of-year format and converted to calendar dates using 2024 as a reference year for display and consistency. By combining long-term data from monitoring sites across the country, this dataset shows how pollen levels vary over time and across geographic locations. This resource supports tracking pollen trends, linking pollen with weather and climate factors, and informing public health action, clinical care, and communication about population exposure and the impact to allergic diseases such as asthma and hay fever. Grass and birch pollen are major allergens in the United Kingdom (UK), responsible for seasonal respiratory diseases between late March and July. East Anglia is an under-represented region in pollen allergy research, while patterns of continuous days of high pollen levels have not been studied at all. Analysis of pollen statistics and trends in East Anglia addresses a regional gap for pollen exposure in the UK and assesses the intensity of the exposure. Trends and statistics for start, end, length, first high day (FH), number of high days (NH), seasonal pollen integral (SPIn) and number of high days occurring in a run together were presented. Birch pollen occurred from late March to late April, with little indication that onset, end or duration were changing temporally. Severity (SPIn) and the number of days in a run together have increased, in line with severity trends in nearby regions. Grass pollen occurred from late May until the third week in July, with almost no indication of changing trends in this region, apart from a likely earlier first high day. These results inform clinicians that the information and advice on when to treat hay fever symptoms and for how long should not change at the present time. Full article

Many viruses, bacteria, and pollen that cause diseases such as COVID-19 are inhaled by humans as aerosols. Therefore, wearing a mask to block pathogen-containing aerosols is crucial for disease prevention. However, current masks have a drawback—residual bacteria on the mask surface can become aerosolized again, spreading infections. To address this, a reusable mask incorporating the antibacterial properties of silver particles should be developed to suppress the immune response to pathogens and pollen that contact the mask surface. This study analyzed protein surface changes in pollen shells following electroless silver plating on polypropylene (PP) filters of KF94 masks using microcurrent. Pore density increased from 7.94% before microcurrent application to 14.8% and 16.9%, depending on the duration of exposure. These results suggest that microcurrents alter pollen surfaces and affect the proteins in pollen shells that trigger hay fever, confirming the potential for preventing pollen allergies. Full article

Allergic rhinitis (hay fever) prevalence has increased in Australia. People with hay fever often experience many eye symptoms, especially itching. This study explores clinical correlations between tear IgE levels and ocular allergy signs in hay fever sufferers, focusing also on eyelid wiper friction damage from eye rubbing. In a cross-sectional study from November 2024 to January 2025, 16 individuals with self-reported hay fever and 17 healthy controls were recruited. Participants completed demographic and allergy-related questionnaires, including symptoms and quality of life assessments. Tear samples were analyzed for IgE and MMP-9 biomarkers. Ocular surface parameters-bulbar redness, palpebral roughness, and lid wiper epitheliopathy (LWE)-were graded. Corneal and conjunctival dendritic cells were also evaluated. Elevated tear IgE significantly correlated with self-reported hay fever, QUICK score, MiniRQLQ, eye rubbing frequency, and lower LWE grade. The hay fever group showed significantly higher LWE compared to healthy controls (p < 0.001), indicating frictional eyelid damage. ROC analysis of tear IgE yielded an AUC of 0.893 (cut-off 0.03 IU/mL; sensitivity 90%, specificity 85%). Tear IgE is a useful biomarker for ocular inflammation and may indicate friction-related eyelid damage in allergy sufferers. Incorporating LWE grading into clinical assessments of ocular allergy is recommended. Full article

Grass pollen allergies significantly contribute to atopic diseases such as asthma and allergic rhinitis, resulting in considerable healthcare burdens. Objective: In this study, molecular sensitization patterns to grass pollen in Swiss patients were addressed. The research utilized a retrospective cohort approach using ImmunoCAP™ ISAC testing from October 2015 to July 2020. Clinical histories, demographics, and skin prick test results were collected for analysis. The minimum patient age was 18 years and the average patient age was 41.3 years, with a female predominance (68.5%). In total, 4814 measurements were analyzed. Allergic rhinitis was the most common clinical symptom, followed by asthma and urticaria. A total of 1963 patients (40.8%) revealed sensitization to grass pollen. The most common sensitizations were found to the major allergens Phl p 1 (86%) and Phl p 5 (65%), but also to Phl p 4 (62%). Monosensitization was mostly found to allergens Phl p 1 (266/13.5%) and Phl p 4 (157/7.9%), and less so to Phl p 5 (33/1.7%). Notably, the Phl p 4-monosensitized subgroup showed only an 18% positivity rate in skin prick tests and presented mostly with urticaria. This study gives insights into the spectrum of grass pollen allergies in a Central European setting and underscores the possibly underestimated role of Phl p 4 among grass pollen allergens, especially in a subgroup that suffers mainly from seasonal urticaria. Monovalent sensitization to Phl p 4 can also cause seasonal rhinitis and might therefore be missed if only Phl p 1/p 5 are tested. A better understanding of sensitization patterns will further improve diagnosis and treatment options. Full article

Allergic rhinitis (AR) is a global health problem on the rise. More and more people are affected, and climate change is exacerbating this health problem in the long term. The quality of life of those affected is often severely compromised, and the financial burden on healthcare systems cannot be disregarded. Therefore, effective and safe medicines are needed to counteract this trend. P. hybridus (butterbur) leaf extract (Ze 339) displays a promising alternative to antihistamines in the treatment of AR symptoms. More than two decades after the first market launch it is now possible to draw a meaningful conclusion on its safety and efficacy. This review summarizes the available preclinical and clinical data, real-world data (RWD) as well as data from post-marketing pharmacovigilance monitoring about the herbal medicinal drug Ze 339. It focusses on the current knowledge about the mode of action as well as the evaluation of its efficacy and safety in the treatment of AR. Given its favourable safety profile and lack of sedative side effects, Ze 339 offers a valuable alternative to antihistamines and should therefore continue to be considered by medical practitioners for the treatment of allergic rhinitis symptoms. Vernal keratoconjunctivitis is a persistent allergic ocular disease predominantly mediated by the T-helper 2 lymphocyte-associated immune response. The standard therapeutic approaches for vernal keratoconjunctivitis include topical corticosteroids and immunosuppressive eye drops. However, managing vernal keratoconjunctivitis with only topical treatments becomes challenging during seasonally exacerbated periods. Systemic treatments such as oral corticosteroids or cyclosporine may be alternative options. Recently, dupilumab’s efficacy in refractory vernal keratoconjunctivitis treatment has been documented. Here, we report a case of refractory vernal keratoconjunctivitis coexisting with atopic dermatitis that rapidly improved after upadacitinib administration. An 18-year-old Japanese woman presented with atopic dermatitis, vernal keratoconjunctivitis, and hay fever. In winter, the patient experienced widespread erythema and escalated itching, leading to significant discomfort and insomnia. Owing to the difficulty in maintaining her current regimen, upadacitinib (15 mg), a Janus kinase inhibitor was initiated. After upadacitinib administration, the treatment-resistant vernal keratoconjunctivitis and erythema improved. Upadacitinib is beneficial in severe cases of atopic dermatitis. Consequently, in our case, upadacitinib may offer therapeutic benefits for refractory vernal conjunctivitis by improving the T-helper 1/2 type immune response, autoimmunity, and oxidative stress. To our knowledge, this is the first report suggesting the potential utility of upadacitinib in managing severe vernal conjunctivitis. Full article

(1) Background. Coeliac disease (CD) often co-occurs with autoimmune conditions or genetic syndromes, but there are few studies on the co-existence of CD and immunoglobulin E (IgE)-mediated allergies. The purpose of this study was to assess sensitization to food and aero-allergens in pediatric patients with CD. (2) Methods. A multiplex ALEX®2 test was used to determine specific IgEs (sIgEs). (3) Results. The study included 108 children newly diagnosed with CD. Allergen extract- and/or allergen molecule-sIgEs were detected in 49.1% of children. Most children (41.5%) were sensitized to both inhalant and food allergens. The three most common aero-allergens (timothy pollen, ryegrass, silver birch) were molecules Phl p 1, Lol p 1, and Bet v 1. The most common food allergens (hazelnut, apple, and peanut) were Cor a 1, Mal d 1, and Ara h 8 molecules of the PR-10 subfamily. Patients were not sensitized to cereal allergens containing gluten. Spearman’s rank correlation analysis of sensitized patients showed a significant positive relationship (r = 0.31) between the patients’ age and the occurrence of positive sIgEs (≥0.3 kUA/L) for inhalant allergen molecules (p = 0.045). In sensitized patients, mainly symptoms of inhalant allergy were observed, such as hay fever, conjunctivitis, and bronchial asthma. (4) Conclusions. The current study indicates the co-occurrence of IgE sensitization to food and inhalant allergens in children with CD. The study highlights the need to take a closer look at the diagnosis of IgE-mediated allergy in patients with CD, which may help in their care and lead to a better understanding of the relationship between CD and IgE-mediated allergy. Mometasone furoate is a synthetic corticosteroid used in the treatment of skin inflammatory conditions, hay fever and asthma. The industrial manufacturing routes to mometasone furoate are generally accompanied by the formation of numerous process impurities that need to be detected and quantified, as requested by regulatory authorities. The ready availability of such impurities in the required quantity and purity is therefore essential for toxicological studies, analytical method development and process validation. Herein, we report the multi-gram scale preparation of 21′-chloro-(16′α-methyl-3′,11′,20′-trioxo-pregna-1′,4′-dien-17′-yl)-furan-2-carboxylate (mometasone furoate EP impurity C), one of the known impurities of mometasone furoate. This study also includes the systematic investigation of the final acylation step, as well as the characterization of the difuroate enol ether intermediate and its conversion to the target impurity C. Corals are facing a range of threats, including rises in sea surface temperature and ocean acidification. Some now argue that keeping corals ex situ (in aquaria), may be not only important but necessary to prevent local extinction, for example in the Florida Reef Tract. Such collections or are already becoming common place, especially in the Caribbean, and may act as an ark, preserving and growing rare or endangered species in years to come. However, corals housed in aquaria face their own unique set of threats. For example, hobbyists (who have housed corals for decades) have noticed seasonal mortality is commonplace, incidentally following months of peak pollen production. So, could corals suffer from hay fever? If so, what does the future hold? In short, the answer to the first question is simple, and it is no, corals cannot suffer from hay fever, primarily because corals lack an adaptive immune system, which is necessary for the diagnosis of such an allergy. However, the threat from pollen could still be real. In this review, we explore how such seasonal mortality could play out. We explore increases in reactive oxygen species, the role of additional nutrients and how the microbiome of the pollen may introduce disease or cause dysbiosis in the holobiont. Full article

Background: It has been recognized from the early days of allergology that aerobiological investigations have an important role to play in the development of respiratory allergic diseases. An increasing number of allergic complaints occurred among the atopic population during the blooming season of Kans grass, Saccharum spontaneum (SS), an obnoxious weed growing in and around suburban West Bengal. The present study aimed to identify SS pollen as a potential aero allergen through aerobiological, clinical, statistical, and biochemical analyses. Methods: An aerobiological survey was conducted for 2 years followed by a clinical diagnosis of 134 local atopic patients suffering from a respiratory allergy by a standard questionnaire survey and the skin prick test (SPT) using SS antigens. The antigenic protein profile was analyzed by SDS-PAGE and the allergizing potential of this pollen was investigated by an in vitro enzyme-linked immunosorbent assay to recognize the presence of the sero-reactive proteins which were the suspected cause of the respiratory allergy. A Box-plot and regression analysis were performed to establish the significance of clinical data. Results: SS pollen was found to evoke about 70.14% sensitivity among the atopic population causing early spring hay fever, allergic rhinitis, and seasonal allergic conjunctivitis. A regression analysis for the pollen antigen for estimating the total IgE value of a patient’s sera from their specific IgE value was a novel approach by our study. The antigenic extract of pollen resolved into more than 15 distinct protein bands ranging from 14.4 to 116 kDa, some of which were found to be glycosylated. The results showed that SS pollen has a significant presence in the atmosphere, which may trigger an allergic response in immunocompromised patients. Conclusions: This is, to our knowledge, the first attempt to identify allergens from Kans pollen causing seasonal pollinosis among the Indian atopic population using an immuno-clinical approach. Brain tumor-initiating cells (BTICs) and tumor cell plasticity promote glioblastoma (GBM) progression. Here, we demonstrate that clemastine, an over-the-counter drug for treating hay fever and allergy symptoms, effectively attenuated the stemness and suppressed the propagation of primary BTIC cultures bearing PDGFRA amplification. These effects on BTICs were accompanied by altered gene expression profiling indicative of their more differentiated states, resonating with the activity of clemastine in promoting the differentiation of normal oligodendrocyte progenitor cells (OPCs) into mature oligodendrocytes. Functional assays for pharmacological targets of clemastine revealed that the Emopamil Binding Protein (EBP), an enzyme in the cholesterol biosynthesis pathway, is essential for BTIC propagation and a target that mediates the suppressive effects of clemastine. Finally, we showed that a neural stem cell-derived mouse glioma model displaying predominantly proneural features was similarly susceptible to clemastine treatment. Collectively, these results identify pathways essential for maintaining the stemness and progenitor features of GBMs, uncover BTIC dependency on EBP, and suggest that non-oncology, low-toxicity drugs with OPC differentiation-promoting activity can be repurposed to target GBM stemness and aid in their treatment. NK-4 plays a key role in the treatment of various diseases, such as in hay fever to expect anti-allergic effects, in bacterial infections and gum abscesses to expect anti-inflammatory effects, in scratches, cuts, and mouth sores from bites inside the mouth for enhanced wound healing, in herpes simplex virus (HSV)-1 infections for antiviral effects, and in peripheral nerve disease that causes tingling pain and numbness in hands and feet, while NK-4 is used also to expect antioxidative and neuroprotective effects. We review all therapeutic directions for the cyanine dye NK-4, as well as the pharmacological mechanism of NK-4 in animal models of related diseases. Currently, NK-4, which is sold as an over-the-counter drug in drugstores, is approved for treating allergic diseases, loss of appetite, sleepiness, anemia, peripheral neuropathy, acute suppurative diseases, wounds, heat injuries, frostbite, and tinea pedis in Japan. The therapeutic effects of NK-4’s antioxidative and neuroprotective properties in animal models are now under development, and we hope to apply these pharmacological effects of NK-4 to the treatment of more diseases. All experimental data suggest that different kinds of utility of NK-4 in the treatment of diseases can be developed based on the various pharmacological properties of NK-4. It is expected that NK-4 could be developed in more therapeutic strategies to treat many types of diseases, such as neurodegenerative and retinal degenerative diseases. Full article

Avoidance Measures for Patients with Allergic Rhinitis: A Scoping Review

Environmental allergen control is recommended as an essential part of allergic rhinitis (AR) management guidelines. In this scoping review, our objective is to identify measures of allergen avoidance and to evaluate their effectiveness in the management of AR. We conducted systematic searches for randomized controlled trials and observational studies in PubMed, the Cochrane Central Register of Controlled Trials, and the Web of Science databases. We included all types of control measures based on allergen eviction or reduction in exposure. Overall, 18 studies satisfied our criteria and were thus included for further analysis. The majority of the studies (15 out of 18) reported decreases in overall AR symptom scores, improvements in quality of life, or reductions in medication usage. However, due to the low number of participants and the limitations in study designs, it is not possible to make a definitive recommendation on the use of these interventions in the management of AR. A multifaceted approach, including treatment, prevention, and eradication of allergens from the environment, may be necessary to effectively reduce symptoms. Full article
