Background: Local anesthesia is essential for most dental procedures, but its parenteral administration is often painful. Topical anesthetics are commonly used to minimize local anesthesia pain; however, commercial formulations fail to fully prevent the discomfort of local anesthetic injection. Methods: We developed and characterized a novel lidocaine and epinephrine co-loaded liquid crystalline precursor system (LCPS) for topical anesthesia. The formulation was structurally characterized using polarized light microscopy (PLM) and small-angle X-ray scattering (SAXS). Rheological behavior was assessed through continuous and oscillatory rheological analyses. Texture profile analysis, in vitro mucoadhesive force evaluation, in vitro drug release and permeation studies, and an in vivo toxicity assay using the chicken chorioallantoic membrane (CAM) model were also conducted. Results: PLM and SAXS confirmed the transition of the LCPS from a microemulsion to a lamellar liquid crystalline structure upon contact with artificial saliva. This transition enhanced formulation consistency by over 100 times and tripled mucoadhesion strength. The LCPS also provided controlled drug release, reducing permeation flow by 93% compared to the commercial formulation. Importantly, the CAM assay indicated that the LCPS exhibited similar toxicity to the commercial product. Conclusions: The developed LCPS demonstrated promising physicochemical and biological properties for topical anesthesia, including enhanced mucoadhesion, controlled drug delivery, and acceptable biocompatibility. These findings support its potential for in vivo application and future clinical use to reduce pain during dental anesthesia procedures. Full article

This study aimed to evaluate the cytotoxicity of two resin materials, Tera Harz TC-85 DAC and Clear-A, along with the effects of two different post-printing protocols applied to Clear-A. Samples were produced using the Ackuretta Sol printer. The following three groups were formed based on the resins used and the post-curing methods applied: Group 1: Tera Harz TC-85 DAC resin + Tera Harz Cure; Group 2: Clear-A resin + Curie machine; and Group 3: Clear-A resin + Tera Harz Cure. All samples were sterilized in 70% ethanol for 5 min, rinsed with sterile deionized water, and incubated in Dulbecco’s Modified Eagle Medium at 37 °C for 72 h. Cytotoxicity assessment was performed by the XTT and RTCA methods using the human gingival fibroblast cell line. According to the XTT assay, undiluted resin extracts exhibited approximately 75–80% cell viability at 24 h, while further dilutions resulted in a viability exceeding 90%. No significant differences in viability were observed among the groups at any dilution at 48 and 72 h. The xCELLigence RTCA results aligned with the XTT findings, showing a transient decrease in cell viability within the first 24 h, followed by continued cell growth. This study demonstrated that extracts from all tested 3D-printed resins exhibited biocompatibility with human gingival fibroblasts. These findings support their potential for further applications in the dental and biomedical fields. Full article

Child and adolescent health plays a critical role in shaping future public health and intergenerational outcomes. In China, rising rates of myopia, obesity, mental health issues, and other common conditions highlight the need for continuous monitoring. Since 2016, the Chinese National Surveillance on Students’ Common Diseases and Risk Factors (CNSSCDRF) has provided comprehensive, nationwide data on student health. By 2022, the system had expanded to nearly all counties, tracking key indicators such as vision problems, overweight/obesity, dental caries, and health-risk behaviors across multiple administrative levels. This review outlines the surveillance methodology, including sampling, data collection, and analysis. Findings have directly informed school health policies and technical guidelines, supporting national goals such as those in the Healthy China 2030 Plan. Full article

Background: The digital transformation in dentistry is increasingly reshaping treatment procedures, offering new opportunities and advancements. While digitalization promises enhanced efficiency and quality of care through the standardization, acceleration, and simplification of workflows, it also introduces challenges related to mental health. Studies investigating digitization-associated stressors and resources, as well as health- and work-related outcomes, in the dental sector are still rare. In the context of ongoing digitalization, further studies are needed to examine the need for and the current status of the implementation of measures preventing techno-stress and stress-related outcomes. This study explores the use of digital tools in dental practices and their relationship to the techno-stress among German dentists. It identifies key stressors and resources associated with digital technologies, aiming to inform preventive measures, as well as training and support strategies to mitigate digital stress. Methods: A qualitative study was employed, involving ten problem-centered, guideline-based expert interviews with German dentists. The interviews were analyzed using MAXQDA software, following the focused interview analysis framework by Kuckartz and Rädiker. Coding and thematic analysis adhered to the Consolidated Criteria for Reporting Qualitative Research (COREQ) checklist and qualitative research quality criteria by Mayring. Results: This study identified a dual impact of digitalization in dentistry. On the one hand, digital tools significantly enhance workflow efficiency, diagnostic accuracy, and patient outcomes. On the other hand, they pose challenges like technostress, high financial costs, and the need for continuous learning. Findings reveal that the perceived usefulness of digital technologies is closely linked to the level of techno-stress experienced, while the amount, intuitiveness, and ease of use significantly influence stress levels. Conclusions: Digital transformation offers substantial benefits for dental practices but requires a balanced approach to implementation. Participants highlighted the need for proactive measures, such as targeted training, technical support, and stress-reducing interventions to reduce techno-stress levels. The digital transformation must be supported by coordinated efforts across academia, industry, and policy to strengthen digital competencies—creating a healthier, more resilient digital work environment. Future research should focus on the causal relationship between techno-stress and adverse long-term consequences, such as burnout or mental disorders, among dentists. Full article

The Impact of Preventive Protocols on Oral Health Outcomes in Cancer Patients Undergoing Chemotherapy or Radiotherapy: A Systematic Review and Meta-Analysis

Background/Objectives: Cancer patients undergoing chemotherapy (CT) or radiotherapy (RT) are at increased risk of oral complications. Preventive dental care has been proposed to mitigate these risks, yet its effectiveness is not sufficiently evaluated. This systematic review and meta-analysis aimed to assess the impact of preventive oral health interventions on key clinical outcomes in oncology patients. Methods: A systematic search of MEDLINE, Embase, and CENTRAL databases was conducted (March 2025), adhering to PRISMA guidelines with a PROSPERO-registered protocol (CRD 420251006799). Eligible studies included randomized trials, cohort studies, and pre–post intervention studies evaluating preventive dental care in patients receiving CT or RT. The outcomes included gingival index (GI), dental caries (DMFT), plaque levels, and periodontal health. Meta-analyses were performed on GI and DMFT outcomes using random-effects models. Results: Eleven studies were included in the qualitative synthesis and four in the meta-analyses. Preventive interventions, such as fluoride applications, oral hygiene education, and regular professional cleanings, were associated with stabilization or improvement of gingival health. The pooled estimate for GI showed no significant deterioration over time (MD = −0.05, 95% CI: −0.34 to 0.24, p = 0.72). For DMFT, a slight but significant increase was observed (MD = 1.07, 95% CI: 0.08 to 2.05, p = 0.03), suggesting a continued risk of caries despite intervention. Conclusions: Preventive dental care interventions appear to support the maintenance of gingival health in cancer patients undergoing CT or RT. However, despite these interventions, a slight increase in dental caries was still observed, indicating that preventive strategies may not fully eliminate the risk of caries. These findings highlight the importance of sustained and individualized oral health programs as part of comprehensive oncology care. Future studies using standardized protocols and longer follow-up periods are needed to better evaluate their long-term effectiveness across diverse cancer populations. Full article

Flourishing in children is an indicator of positive development in the areas of emotional, social, and cognitive development. Using a recent dataset from the US National Survey of Children’s Health, this study investigates the association between access to a quality healthcare system and flourishing indicators in school-aged children with autism. The outcome variable describes the proportion of children aged 6–17 with autism meeting the flourishing criteria, which include measures related to learning, resilience, and self-regulation. The main independent variable includes six core indicators for school-aged children and adolescents, which assess whether the family feels like a partner in their child’s care, the child has a medical home, receives regular medical and dental preventive care, has adequate insurance, has no unmet needs or barriers to accessing services, and has prepared for transition to adult healthcare. Multivariable logistic regression models were used to examine the relationships between various independent variables and the outcome of interest. The results show that children with autism who receive comprehensive and coordinated care are more likely to flourish compared to those without such care for five of these six indicators, while controlling for sex, race, parental education, household income, self-reported autism severity, general health status, and the number of adverse childhood experiences. Children with autism are more likely to flourish when their families and healthcare providers work together effectively. Addressing gaps in the quality care system is essential for developing holistic approaches that empower children with autism to thrive and reach their full potential. Supportive health policies and effective coordination between families and healthcare providers are crucial for fostering the flourishing of children with autism, ensuring comprehensive, individualized, and continuous care. Background/Objectives: Composite dental restorations are continuously exposed to dietary substances, which may compromise their structural integrity. This study aimed to assess the chemical and mechanical effects of coffee, red wine, and Coca-Cola on two widely used commercial resin composites, Herculite Ultra XRV and Omnichroma. Methods: Forty disk-shaped specimens (20 per material) were immersed for 10 days in the selected beverages. Changes in chemical composition were analyzed using Fourier Transform Infrared (FTIR) and Raman spectroscopy, while Vickers microhardness testing evaluated surface hardness. Results: FTIR and Raman analyses revealed that coffee and red wine caused the most substantial chemical degradation, particularly in carbonyl (C=O), aromatic (C=C), and siloxane (Si–O–Si) groups. Herculite XRV demonstrated higher chemical stability, while Omnichroma showed more pronounced molecular degradation. In contrast, microhardness testing indicated that Omnichroma maintained better surface hardness compared to Herculite XRV after exposure. Across all solutions, Coca-Cola induced the least effect. Conclusions: The tested beverages significantly affected both the chemical and mechanical properties of the resin composites. Omnichroma exhibited superior mechanical durability, while Herculite XRV showed greater resistance to chemical degradation. These results highlight the importance of material composition in restorative dentistry and support the development and selection of composites with improved resistance to acidic and staining agents to ensure long-term clinical performance. (1) Background: Advancements in dental imaging technologies have significantly transformed diagnostic and surgical practices. The integration of cone beam computed tomography (CBCT), artificial intelligence (AI), augmented reality (AR), and virtual reality (VR) is enhancing clinical precision, streamlining workflows, and redefining dental education. This review examines the evolution, applications, and future potential of these technologies in modern dental practice. (2) Methods: A narrative literature review was conducted, synthesizing findings from recent studies on digital radiography, CBCT, AI-assisted diagnostics, 3D imaging, and involving simulation tools (AR/VR). Peer-reviewed journal articles, systematic reviews, and clinical studies were analyzed to explore their impact on diagnosis, treatment planning, surgical execution, and training. (3) Results: Digital and 3D imaging modalities have improved diagnostic accuracy and reduced radiation exposure. AI applications enhance image interpretation, automate clinical tasks, and support treatment simulations. AR and VR technologies provide involved, competency-based surgical training and real-time intraoperative guidance. Integrating 3D printing and portable imaging expands accessibility and personalization in care delivery. Reliability and Validity of the Modified Dental Anxiety Scale Among Children Aged 9 to 12 Years

Objectives: Our aim was to study whether the Modified Dental Anxiety Scale (MDAS) is reliable and valid for use in children aged 9 to 12 years. Methods: The study population was a convenient sample of Finnish comprehensive school pupils in the third and sixth grades (N = 57 and N = 69, respectively). Dental fear and anxiety (DFA) were measured with the Finnish validated adult version of MDAS, the modified Child Fear Survey Schedule—Dental Subscale (CFSS-DS-M), and a single question. Reliability was evaluated using Cronbach’s alpha. The criterion validity of MDAS was assessed using Spearman rank correlation coefficients against CFSS-DS-M and the single question. Construct validity was assessed by examining the ability of MDAS to find differences according to gender and treatment procedures using the chi-square test for categorized and the Mann–Whitney and Jonckheere–Terpstra tests for continuous variables. Results: The Cronbach alphas were 0.841, 0.708, and 0.778 for MDAS total, anticipatory, and treatment-related DFA, respectively. Correlations between MDAS and CFSS-DS-M total and subscale scores were moderate to strong (ρ = 0.559–0.794), supporting criterion validity. Girls in third grade had lower mean MDAS anticipatory DFA (3.4, SD = 1.44) than boys (4.5, SD = 2.21, p = 0.051). In sixth graders, girls had higher mean MDAS treatment-related DFA (8.4, SD = 3.17) than boys (6.9, SD = 2.61, p = 0.067). Children reporting orthodontic treatment had lower anticipatory DFA (mean = 3.4, SD = 2.13) than children not reporting (mean = 4.0, SD = 1.83; p = 0.009), supporting construct validity. Conclusions: The Finnish version of the MDAS showed good reliability, good criterion validity, and acceptable construct validity, supporting its use in children aged 9–12 years. Full article

Background/Objectives: The integration of digital technologies into dental education is becoming increasingly important, particularly in prosthodontics, where digital design tools offer enhanced precision and efficiency. This study aimed to evaluate second-year dental students’ perceptions regarding conventional versus digital design in prosthodontics, assessing their theoretical knowledge, practical skills, and attitudes toward these approaches. Methods: A total of 141 dental students enrolled in Faculty of Dentistry from “Carol Davila” University of Medicine and Pharmacy, Bucharest, Romania, participated in an online survey consisting of 19 questions evaluating their understanding, preferences, attitudes, and expectations regarding digital and conventional prosthodontic workflows. Additionally, students’ practical exam grades and task completion times for both conventional and digital design methods were evaluated. Results: Participating students reported sufficient understanding of theoretical concepts in both conventional (92.9%) and digital design (91.5%). A significant proportion (78.7%) felt confident in their practical skills for conventional design, while 78% expressed the same for digital design. Statistically significant correlations indicated that students who believed digital design could replace conventional methods associated digital design with greater accuracy (p = 0.020), predictability (p = 0.048), and sustainability (p = 0.032). Students who believed they had acquired enough skills in digital design responded more frequently that the time allocation for digital design by the university was sufficient (p < 0.001). Moreover, students scored significantly higher in digital design practical exams compared to conventional design (p < 0.001). Task completion times were also shorter for digital workflows, further supporting their efficiency. Conclusions: The findings suggest that students are highly receptive to digital technologies in prosthodontics, favoring digital workflows over conventional techniques. These results highlight the need for continued integration of digital tools into dental curricula to enhance students’ competency and prepare them for modern clinical practice. Full article

Introduction: Hypoglossal nerve stimulation (HNS) “Inspire© therapy” has garnered popularity among obstructive sleep apnea (OSA) patients seeking an alternative to continuous positive airway pressure (CPAP) therapy. The growth in HNS has been particularly high in older adults living with OSA. Consistent and proper use of HNS in the geriatric population faces unique age-associated barriers: a high rate of multiple chronic conditions (MCC) and polypharmacy (being on five or more drugs). Early recognition and patient-centered management of these barriers will allow older patients to obtain maximum benefits from HNS. HNS has distinct advantages in the geriatric population because it overcomes many concerns related to CPAP therapy adherence, such as mechanical limitations due to manual dexterity, maxillofacial anatomy, dental issues such as usage of dentures, allergy/otolaryngology-related disorders, and pre-existing post-traumatic stress disorder-related claustrophobia. This paper describes how we worked with older patients with OSA and their care partners to overcome these barriers so patients can continue to derive cardiovascular, neurologic, and quality of life benefits resulting from optimal OSA management. These benefits are especially important in the older population because of higher rates of comorbidities (dementia, coronary artery disease, and atrial fibrillation) exacerbated by sub-optimally treated OSA. In this article, we describe our clinical experience with elderly patients on Inspire© therapy, with a focus on the everyday difficulties faced by these patients and the measures implemented to address and mitigate these barriers. Methods: A retrospective chart review was conducted to identify patients aged 65 and above who underwent hypoglossal nerve stimulator insertion. Experiences of older patients during and after the insertion procedure were documented and compared to a younger population of patients on HNS therapy. We specifically collected information on difficulties encountered during activation or follow-up visits and compared them between the different age groups. Using this information, we identified areas to improve treatment adherence from the patients’ perspectives. Results: We identified 43 geriatric (65 to 86 years old) patients who received the Inspire implant at a tertiary academic medical center and compared them to a younger population of 23 patients. Most common challenges noted—with a potential to impact adherence—included orofacial and lingual neuropraxia (ischemic or demyelination-induced neuropathy) at activation, cognitive dysfunction (memory problems), preexisting anxiety, and insomnia. Other difficulties that are less commonly reported but equally important to consistent and proper use of HNS included headaches, concerns of device malfunction, change in comfort levels after cardiac procedures, and general intolerance of the device. The older patient population had a statistically significant higher incidence of cognitive difficulties (30.2% vs. 4.4%) and a smaller social support system (62.8% vs. 91.3%) affecting device usage compared to the younger population. There were no statistically significant differences in the rates of other more commonly reported adverse effects such as headaches, dry mouth, and anxiety between the two age groups. Conclusion: Despite several challenges faced by geriatric patients, Inspire© hypoglossal nerve stimulation remains a viable, alternative treatment option for OSA with improved tolerance and adherence compared to CPAP. After identifying less commonly reported barriers such as cognitive decline, sensory deficits, and decreased social support systems, minor adjustments and appropriate education on use allows older patients to correctly use and benefit from Inspire© device therapy, with subsequent improvement in sleep and overall quality of life. Background/Objectives: Digital transformation is reshaping dentistry by improving clinical efficiency, diagnostic accuracy, and patient care. However, the adoption of digital technologies in dental clinics varies widely, influenced by multiple factors, including technology readiness. This study aimed to assess the relationship between technology readiness and digital technology adoption among German dentists, focusing on the impact of clinic characteristics and professional development. Methods: A cross-sectional survey was conducted among 200 licensed German dentists. Technology readiness was measured using the validated Technology Readiness Index (TRI 2.0), encompassing four dimensions: optimism, innovativeness, discomfort, and insecurity. Data on the current use of digital technologies were collected, including digital radiography, CAD/CAM systems, AI-supported tools, and patient management solutions. Statistical analyses included correlation and quartile-based comparisons to identify patterns and significant associations. Results: Clinics with higher TRI scores demonstrated significantly greater adoption of digital technologies. Larger clinics (MVZs) showed higher levels of digital integration compared to solo practices. Younger dentists and those engaged in continuous professional development exhibited higher technology readiness and usage of advanced digital tools. No significant gender-based differences were identified in technology readiness or digital adoption. While basic technologies like digital radiography and CAD/CAM systems were widely used, AI-based diagnostics and 3D printing remained underutilized. Key barriers included financial constraints and limited training opportunities. Conclusions: Technology readiness plays a critical role in shaping digital adoption in dental clinics. The findings highlight the need for targeted support, especially for smaller clinics, through professional training and investment in digital infrastructure. This study contributes to a better understanding of digital transformation in dentistry and supports strategies aligned with global health goals to improve access to digital care. Full article

Background/Objectives: Lately, there has been a greater focus on the function of the dento-mandibular apparatus, specifically on the location of the jaw and occlusion. Given the new potential insights, the current study aimed to comprehensively analyze the published literature on the use of occlusal splints and their effects on exercise performance. Methods: A search was conducted on PubMed, Scopus, and Web of Science for papers published between 2014 and 2024. Starting from the 128 identified records, 28 were finally included for review. Results: The extensive literature review revealed significant diversity in the experimental conditions, suggesting that the occlusal splints may enhance exercise performance and support dental health. Conclusions: The present study highlights the growing interest in occlusal splints research and its impact on sport and exercise science. Mouthguards or occlusal splints should continue to be worn in sports with a considerable risk of orofacial injury. Regardless of how they affect performance, mouthguards or occlusal splints are crucial for athletes in many sports to prevent oral and dental injuries. Full article

Background: Autogenous bone grafting has long been the standard for augmenting bone prior to implant placement in atrophic ridges. However, innovative techniques are continually sought to enhance outcomes. This study introduces the honeycomb technique for horizontal bone augmentation in edentulous posterior mandibular ridges, presenting the methodology and long-term follow-up results of this novel approach. Methods: This study includes healthy patients with moderate to severe horizontal atrophy in posterior mandibular regions who underwent bone augmentation using the honeycomb technique and were followed up for a period of 10 to 14 years. The patients had orthoradiographs immediately post-surgery and underwent regular clinical and radiographic evaluations. Computed tomography at four months assessed the bone gain, followed by reentry for implant insertion and evaluation of the bone volume and quality. Fixed prosthesis-supported dental implants were placed four months post-insertion. The survival and success of the dental implants were evaluated based on the acceptable clinical and radiographic criteria. Results: A cohort of 23 patients (17 women, 6 men, mean age 47 years) underwent bone augmentation at 39 sites, with follow-up ranging from 10 to 14 years. The procedure demonstrated a 95–100% success rate with minimal morbidity and horizontal bone gain averaging 3–8 mm. Partial graft exposure occurred in two cases but was successfully managed without compromising augmentation. A total of 103 implants were placed in the augmented sites in 37 sites. The long-term survival of the dental implants was confirmed based on clinical and radiographic evaluation, with minimal marginal bone loss observed during the extended follow-up period. Conclusions: The honeycomb technique proves effective in horizontal bone augmentation of atrophic ridges in posterior mandibular defects. The satisfactory long-term outcomes validate its potential as a valuable addition to bone augmentation strategies preceding implant placement. Full article

Background: Dental implantology has undergone significant advancements with the integration of digital workflows, transforming the processes of planning, designing, surgical delivery, and prosthetic rehabilitation. Among these innovations, intraoral optical scanning (IOS) has emerged as a preferred method over traditional analogue impressions. This preference is due to its cost-effectiveness, efficiency, and streamlined patient-friendly use while producing clinically acceptable results in terms of trueness and precision, particularly for short-span implant prostheses. Methods: However, the clinical utility of intraoral scanning is significantly affected by the lack of reference points and difficulties in moisture and bleeding control at the time of immediate implant placement surgery in the fully edentulous arch. Current evidence supports the general consensus that the traditional analog impression technique still provides superior trueness and precision compared to IOS, specifically in full-arch implant cases. Results: The continuous quest for precision in dental implantology has led to the introduction of photogrammetry, which is now considered the most accurate technique for the digital scanning of dental implants. Photogrammetry has demonstrated superior results compared to those obtained using the analog technique. Conclusions: The aim of this case report is to provide an overview of analog techniques, digital intraoral optical scanning, and photogrammetry, setting the stage for the introduction of a novel technique involving a dedicated optical scan-transfer device (IPD®) that can be scanned with ease using IOS, either intra- or extra-orally, due to its unique design features and digital properties.
