Branched-Chain Amino Acids Combined with Exercise Improves Physical Function and Quality of Life in Older Adults: Results from a Pilot Randomized Controlled Trial

This pilot, randomized, double-blind, placebo-controlled trial investigated the effects of branched-chain amino acids (BCAAs)—provided in a 2:1:1 ratio of leucine:isoleucine:valine—combined with exercise on fatigue, physical performance, and quality of life in older adults. Twenty participants (63% female; BMI: 35 ± 2 kg/m2; age: 70.5 ± 1.2 years) were randomized to 8 weeks of either exercise + BCAAs (100 mg/kg body weight/d) or exercise + placebo. The program included moderate aerobic and resistance training three times weekly. Physical function was assessed using handgrip strength, chair stands, gait speed, VO2 max, and a 400 m walk. Psychological health was evaluated using the CES-D, Fatigue Assessment Scale (FAS), Insomnia Severity Index (ISI), and global pain, fatigue, and quality of life using a visual analog scale (VAS). Significant group x time interactions were found for handgrip strength (p = 0.03), chair stands (p < 0.01), and 400 m walk time (p < 0.01). Compared to exercise + placebo, exercise + BCAAs showed greater improvements in strength, mobility, and endurance, along with reductions in fatigue (−45% vs. +92%) and depressive symptoms (−29% vs. +5%). Time effects were also observed for ISI (−30%), FAS (−21%), and VAS quality of life (16%) following exercise + BCAA supplementation. These preliminary results suggest that BCAAs combined with exercise may be an effective way to improve physical performance and reduce fatigue and depressive symptoms in older adults. Full article

The heart is innervated by the autonomic nervous system (ANS), which plays a role in regulating the heart rate. Cross-country mountain biking (MTBXC) is a sport with high physiological demands, where the autonomic nervous system plays a significant role. The main objective of this study was to analyze the asymmetry of heart rate in Colombian National Team mountain bikers, sub-23 category, during a 20 min cardiovascular autonomic profile test. Method: The cardiovascular autonomic profile was measured through heart rate variability during a 20 min test, divided into eight phases (supine, controlled ventilation at 10 cycles/min, controlled ventilation at 12 cycles/min, postural change, orthostasis, Ruffier test, 1 min recovery, and final recovery) in a group of n = 10 MTB cyclists from the National Sub-23 Team, including 5 males and 5 females. Results: The results for the male athletes were as follows: age: 19 ± 1 years; VO2max: 67.5 mL/kg/min; max power: 355 W; HRmax: 204 bpm. The results for the female athletes were as follows: age: 19 ± 1 years; VOmax: 58.5 mL/kg/min; max power: 265 W; HRmax: 194 bpm. Both genders showed the expected autonomic behavior in each phase. Asymmetrical propagation of heart rate was observed, with a greater deceleration pattern after postural changes and effort and a symmetrical acceleration pattern in these two phases. Discussion: Athletes exhibit increased vagal response compared to non-athletes. Mountain bikers show rapid heart rate reduction after exertion. Conclusion: This study demonstrates how mountain bikers exhibit increased heart rate deceleration following sympathetic stimuli. Full article

Background/Objectives: Cardiorespiratory fitness (CRF) is of great interest in children and adolescents. Due to the limited availability of cardiopulmonary exercise testing, simple and reliable alternatives are needed. A stair climbing test (SCT) for the assessment of CRF developed at the Department of Pediatric Cardiology of the LMU University Hospital in Munich showed a strong correlation with VO2max. The aim of this study is to prove its feasibility in a non-clinical setting and to analyse its results in a larger study population. Methods: During the “Hand aufs Herz” study, a comprehensive cardiovascular examination was carried out on 922 pupils and siblings (13.2 ± 7.8 years) at a high school in Bavaria. The SCT was performed to evaluate CRF: participants had to run up and down a total of four floors (14.8 m) as quickly as possible without skipping steps or holding on to the banister. Absolute time has been normalized over the standard height of 12 m to allow comparisons with different settings. An SCT Index was calculated to adjust results to the different weights of participants and the exact height of the staircase. Results: The SCT proved to be easily feasible and safe in non-clinical contexts. Out of 922 participants, 13 (1.4%) were not able to perform the test, and 3 (0.3%) had to interrupt it following fatigue or stumbling. A total of 827 participants aged from 9 to 17 years (13.1 ± 2.1 years, 45.8% girls) had a mean absolute SCT time of 53.4 ± 6.2 s and 43.3 ± 5.1 s when normalized over 12 m. Conclusions: The SCT represents a simple, cost- and time-saving test that allows a rapid and solid assessment of cardiorespiratory fitness in children and adolescents. We could demonstrate that it is safe and feasible in non-clinical contexts. Its short duration and universal applicability are valuable advantages that could facilitate the establishment of a repetitive cardiovascular screening in the pediatric population, particularly in outpatient departments or settings with low-resource systems. Background: Soccer is a physically demanding sport characterized by frequent high-intensity efforts, which are particularly relevant in women’s competitions. Improving high-speed running and aerobic capacity has been linked to better on-field performance. Strength training has shown promise in enhancing these physical attributes, but its application in young female soccer players remains underexplored. This study aimed to investigate the effects of a 10-week in-season strength training program on physical performance and match running demands in young female soccer players. Methods: Thirty-two U18 Danish female professional soccer players from two comparable teams voluntarily participated in the study. Teams were allocated to either an experimental group, performing twice-weekly strength training (EG, n = 16) or a control group (CG, n = 16). Vertical jump performance and Yo-Yo IR2 performance as an estimation for maximal oxygen uptake (VO2max) were assessed both pre and post intervention. Additionally, players’ match external demands (i.e., total distance, distance covered at speeds above 23 km·h−1, and maximum velocity achieved) were monitored using Global Positioning System devices during four matches before and after the intervention. Results: Significant within-group differences were observed across all variables for the EG (p = 0.001; ES = 1.08 to 1.45, large), without differences in the CG (p > 0.01). Between-group analysis indicated significant differences favoring the EG in all variables (F = 27.40 to 47.17; p = 0.001). Conclusions: The application of a 10-week strength training program led to improvements in physical and match running performance among young female soccer players, underscoring the importance of incorporating strength training programs into female soccer periodization to enhance performance. The aim of this systematic review was to examine the effects of high-intensity interval training (HIIT) on physical performance and body composition in female basketball players. The review followed PRISMA guidelines, and the protocol was registered in the PROSPERO database (registration number: CRD420251006285). A comprehensive search was conducted across PubMed, Scopus, Web of Science, and Google Scholar. Nine studies that met the inclusion criteria were analyzed, with intervention durations ranging from 4 to 12 weeks. Despite differences in protocols, a majority of studies reported improvements in VO2max (6/9), explosive strength (7/9), agility (5/6), and speed (5/6) and reductions in body mass and fat percentage (3/3). These findings highlight HIIT as an effective method for enhancing both aerobic and anaerobic capacities, as well as optimizing body composition. Despite variations in study protocols, HIIT consistently offers improvements in performance, irrespective of training level. The results underscore the importance of HIIT in preparing athletes, not only during the preseason but also throughout the competition period. Coaches should consider integrating HIIT into training programs, adjusting intensity and volume based on the season to optimize performance and prevent overtraining. Full article

Background: Premature ventricular contractions (PVCs) are common arrhythmias associated with symptoms such as fatigue and, in severe cases, PVC-induced cardiomyopathy. Catheter ablation (CA) is a primary treatment for symptomatic PVCs, particularly when pharmacological therapies fail or are undesired. While improvements in: quality-of-life following ablation are documented, its impact on functional capacity remains underexplored. Objectives: This study evaluated the impact of CA on functional capacity and cardiac stress markers in patients with symptomatic PVCs using cardiopulmonary exercise testing (CPET) and NT-proBNP levels. Methods: A total of 30 patients underwent successful PVC ablation and completed baseline and follow-up CPET evaluations under the Bruce protocol. PVC burden, left ventricular ejection fraction (LVEF), NT-proBNP levels, and CPET parameters, including VO2 max, METS, ventilatory efficiency, and anaerobic threshold (AT), were analyzed pre- and post-ablation. Results: PVC burden significantly decreased post-ablation (23,509.3 ± 10,700.47 to 1759 ± 1659.15, p < 0.001). CPET revealed improved functional capacity, with VO2 max increasing from 24.97 ± 4.16 mL/kg/min to 26.02 ± 4.34 mL/kg/min (p = 0.0096) and METS from 7.16 ± 1.17 to 7.48 ± 1.24 (p = 0.0103). NT-proBNP significantly decreased (240.93 ± 156.54 pg/mL to 138.47 ± 152.91 pg/mL, p = 0.0065). LVEF and ventilatory efficiency metrics (VE/VO2 and VE/VCO2) remained stable. Conclusions: Catheter ablation improves functional capacity, reduces cardiac stress, and minimizes medication dependency in patients with symptomatic PVCs. These findings support the utility of ablation in enhancing aerobic capacity and overall exercise performance. Full article

Background/Objectives: Although there is an increasing interest among athletes in adopting plant-based diets, there is insufficient research available to determine how a vegan diet affects soccer performance. Methods: This interventional pilot study examined the effect of an 8-week vegan diet (VEG, n = 10) on nutritional status and athletic performance in semi-professional soccer players compared to controls (CON, n = 8). The study employed a controlled, non-randomized, longitudinal pilot study design during the season to compare the two groups. Results: Both groups displayed overall differences in nutrient intake, including insufficient energy and carbohydrates (t2: 46.2 [40.3–52.2] En% (VEG) vs. 37.6 [34.1–41.1] En% (CON); p = 0.036, Cohen’s d = 1.321). Notably, biochemical parameters 25(OH)D and ferritin levels fell within the normal ranges for both groups. The VEG group exhibited favorable changes in total and LDL cholesterol levels. Both groups had increased performances on the treadmill over the entire course of the study (VEG: +0.87 km/h (6.6%); CON: +0.96 km/h (7%); p > 0.05). The initial relative VO2max at t0 was comparable between the groups. Primarily due to the significant weight loss in the VEG group (−1.94 kg, p = 0.007) rather than a change in absolute VO2max values, we found an increased relative VO2max in the VEG group, which was significantly different from that of the CON group (57.0 [53.7–60.3] mL/kg/min (VEG) vs. 51.6 [48.1–55.0] mL/kg/min (CON); p = 0.041, Cohen’s d = 1.675). Conclusions: These findings suggest that a short-term vegan diet does not adversely affect training-induced performance improvements and may be suitable for semi-professional soccer players. The purpose of this non-randomized study was to determine the effect of strength training and aerobic interval training on the anaerobic and aerobic power and endurance of young men (assessed by determination of the second ventilatory threshold (VT2)) in non-trained men. Participants (n = 45) were recruited into three groups of 15 each. The first group performed strength training (ST), the second performed aerobic interval training (AIT), and the third group was the control group (CON). In each group, somatic measurements and tests of aerobic (graded test with VT2 determination) and anaerobic capacity (Wingate test) were performed twice (before and after the exercise intervention in the training groups). In the graded test, the level of maximal load (Pmax), maximal oxygen uptake (VO2max) and intensity and oxygen uptake at VT2 were determined. In the Wingate test, peak power (PP) and mean power (MP) were determined. The exercise intervention in the ST and AIT groups lasted 6 weeks, with three workouts per week. Training in the ST and AIT groups resulted in significant increase in absolute Pmax (p < 0.001, ES = 0.52 and p < 0.05, ES = 0.36), VO2max (p < 0.001, ES = 0.50 and p = 0.02, ES = 0.55) in the participants. Only AIT was significantly effective in improving oxygen uptake at VT2 (p < 0.04, ES = 0.64), and ST in improving PP. Strength training can be an effective training method in training aerobic and anaerobic capacity (significantly increases Pmax, VO2max, and PP), while it does not significantly affect work intensity at VT2. Our results suggest that, particularly in anaerobic–aerobic sports, strength training may be a training method that can simultaneously improve both anaerobic power and maximal oxygen uptake. It can also complement endurance training. Full article

This study aimed to describe the training leading to the Zwift Academy (ZA) Finals of a world-class road cyclist who earned a professional contract after winning the contest. Four years of daily power meter data were analyzed (male, 25 years old, 68 kg, VO2max: 85 mL·min−1·kg−1, and 20-min power: 6.37 W·kg−1), focusing on load, volume, intensity, and strategies. Early training alternated between long, moderate-intensity sessions and shorter high-intensity sessions, with easy days in between. Gradually, the structure was progressively modified by increasing the duration of moderate-intensity (MIT) and high-intensity (HIT) and, subsequently, moving them to “high-volume days”, creating a sort of “all-in days” with low-intensity (LIT), MIT, and HIT. Moderate use of indoor training and a few double low-volume, low-intensity sessions were noted. These data provide a deep view of a 4-year preparation period of ZA, providing suggestions for talent identification and training, thereby highlighting the importance of gradual progression in MIT and HIT. Background: The perceived importance of physical practice and its contribution to students’ academic success have evolved considerably throughout the history of the modern educational system. Aim: The purpose of this study was to understand the relationship between physical fitness (measured as VO2max) and cognitive abilities (attention and concentration) and academic performance in different subjects: sciences, letters, language, arts, and physical education. Method: Fifty Spanish male students who participated in extracurricular sports activities (mean age (SD): 11.59 ± 1.30; range: 9–15 years) were included in the analysis. The 6 min walk test was used to assess physical fitness (6MWT), while for selective attention and concentration, the students completed the D2 test, which is usually considered to analyse the visual ability to select the most relevant stimulus of an exercise and ignore precisely the most irrelevant stimuli. Results: Correlation the individual contribution analyses revealed no significant associations between VO2max and academic performance in sciences (r = 0.04, p = 0.77), humanities (r = 0.00, p = 0.98), language (r = 0.03, p = 0.83), or arts (r = 0.04, p = 0.76). Similarly, no relationship was found between VO2max and overall academic performance (r = 0.10, p = 0.46), or cognitive abilities. However, a small positive correlation was observed between VO2max and physical education scores. Conclusions: Physical fitness showed no significant association with cognitive abilities or academic performance in most subjects, although a small positive correlation with physical education scores was observed. These findings emphasise the importance of promoting physical activity for its health and physical benefits. However, future research should explore broader cognitive outcomes and include more diverse and representative samples. Full article

Introduction: Home-based cardiac telerehabilitation (HBCTR) is a multidisciplinary intervention aimed at optimizing functional, psychological, and social recovery in patients undergoing percutaneous coronary intervention (PCI). This rehabilitation model serves as an effective alternative to traditional center-based rehabilitation, providing a cost-effective and clinically advantageous approach. Methods: Following PRISMA guidelines, we conducted a systematic literature search across multiple databases (PubMed, CINAHL, Cochrane, Scopus, Web of Science). We included randomized controlled trials (RCTs), cohort, and observational studies assessing telerehabilitation in post-PCI patients. Primary outcomes focused on health-related quality of life (HRQoL) and adherence, while secondary outcomes included functional capacity (6 min walk test, VO2max), cardiovascular risk factor control, and psychological well-being. Risk of bias was assessed using the Cochrane RoB 2.0 and ROBINS-I tools. Results: A total of 3575 articles were identified after removing duplicates, of which 877 were selected based on title and abstract, and 17 met the inclusion criteria, with strong RCT representation ensuring robust evidence synthesis. HBCTR was associated with significant improvements in exercise capacity, with increases in VO2max ranging from +1.6 to +3.5 mL/kg/min and in 6 min walk distance from +34.7 to +116.6 m. HRQoL scores improved significantly, with physical and mental component scores increasing by +6.75 to +14.18 and +4.27 to +11.39 points, respectively. Adherence to telerehabilitation programs was consistently high, often exceeding 80%, and some studies reported reductions in hospital readmissions of up to 40%. Wearable devices and smartphone applications facilitated self-monitoring, enhancing adherence and reducing readmissions. Several studies also highlighted improvements in anxiety and depression scores ranging from 10% to 35%. Conclusions: HBCTR is a promising strategy for rehabilitation and quality-of-life improvement after PCI. It offers a patient-centered solution that leverages technology to enhance long-term outcomes. By integrating structured telerehabilitation programs, healthcare systems can expand accessibility, promote adherence, and improve equity in cardiovascular care. Full article

Breathing reserve (BR) is the remaining proportion of achievable minute ventilation that remains unutilized at total exhaustion during exercise. Previous studies have found a smaller BR in endurance-trained athletes compared to untrained controls. However, most of these studies have examined men. Given that women have a greater ventilatory limitation than stature-matched men, the present cross-sectional study aimed to investigate how this sex difference influences BR and lung function tests in endurance-trained females compared to matched, untrained females. To obtain further insight, we also aimed to investigate whether VO2max serves as a predictor of BR. We examined 15 female elite runners and 15 healthy, matched female controls aged 24–33 years with regard to pulmonary function, MVV, VEmax, BR, and VO2max. The elite runner group had a median BR of 5%, while that of the controls was 21%, representing a significant difference. Lung function tests showed no differences between the two groups. A moderate association was found between VO2max and BR. The female elite runners demonstrated a lower BR than the group of matched, untrained controls, which was lower than that found for male elite athletes in previous studies. This may indicate a greater ventilatory demand in female relative to male endurance athletes. Full article

Background: Considering that the prevalence of obesity has risen rapidly in recent decades, the aim of this study was to investigate the effects of a carbohydrate mouth rinse (CMR) on the outcomes of aerobic training among adult men with obesity, focusing particularly on the effects of repeated use on body composition and exercise performance. Methods: The intervention targeted 20 men with obesity in their 20s and 30s randomly assigned to either a CMR group (n = 10) or a placebo mouth rinse (PMR) group (n = 10). Both groups completed treadmill-based aerobic training three times per week for eight weeks. Prior to each session, participants used a mouth rinse at 60, 40, and 20 s before the start of each exercise, holding either a 6% maltodextrin solution (CMR) or purified water (PMR) in their mouths for 5 to 10 s before expectorating. Pre- and post-intervention assessments included body composition (body weight and body fat percentage), resting metabolic rate (RMR), maximal oxygen uptake (VO2max), and exercise performance (rate of perceived exertion [RPE], exercise distance, speed, and time). Data were analyzed using 2 × 2 repeated measures analysis of variance. Results: Following the intervention, the CMR group showed significantly greater improvements than the PMR group did in body fat percentage, RMR, VO2max, exercise distance, speed, and time (p < 0.01). However, the interaction effect for RPE was not statistically significant between the groups (p = 0.175). Overall, the repeated use of the CMR during aerobic training contributed to enhanced exercise performance and favorable physiological changes without additional caloric intake. Conclusions: A CMR may be a practical and non-caloric ergogenic aid to support exercise performance and metabolic function in individuals with obesity. Its repeated use during aerobic training appears to be effective and safe, especially when fasting while exercising, when improving endurance without compromising fat loss is essential. Full article

This study investigated the effects of high-intensity interval training (HIIT) in elite youth soccer players using a novel multidomain correlational mapping approach. A four-week HIIT intervention was applied in a randomized controlled design, with physiological, cognitive, and neuromuscular data collected through laboratory, field-based, and biochemical tests. Metrics such as VO2max, BDNF levels, lactate dynamics, and cognitive load were analyzed across time points and groups. HIIT elicited statistically significant improvements in aerobic capacity, buffering efficiency, and perceptual-cognitive function, with a notable emergence of cross-domain associations. Unlike the control group, HIIT participants showed strengthened correlations between metabolic, cognitive, and neuromuscular indices, such as lactate slope with exertion perception and BDNF response with cardiac recovery. Hierarchical clustering further revealed tightly integrated multidomain clusters in the HIIT group, absent in the controls, suggesting a reorganization of physiological networks. These findings support the concept that HIIT not only improves discrete capacities but fosters systemic adaptation through enhanced inter-domain coordination. These results align with emerging frameworks in network physiology and highlight the potential for using correlation structures as biomarkers of holistic training adaptation. This multidimensional perspective offers new insights into how targeted training reshapes performance-related systems and may inform individualized athletic programming. Full article

Background: Cardiovascular fitness has been proposed as a key factor influencing executive functioning and academic performance during childhood and adolescence. However, the extent and consistency of this relationship remain unclear across diverse populations and educational contexts. This systematic review aimed to evaluate whether cardiovascular fitness, particularly measured through VO2max, is consistently associated with improvements in executive function and academic performance among students from early childhood to adolescence. Methods: A systematic search of PubMed, Web of Science, SPORTDiscus, and ProQuest Central was conducted up to 15 November 2022. Studies were included if they examined correlations between VO2max and cognitive or academic outcomes in students from preschool to high school. Methodological quality was assessed using the MINORS checklist. Results: Out of 271 identified studies, 12 met all inclusion criteria. Evidence suggests that higher VO2max is generally associated with improved executive function domains such as attention, working memory, and inhibitory control, as well as academic performance indicators including mathematics and reading scores. Neurophysiological studies also indicate links between cardiovascular fitness and brain structure/function. However, the strength and specificity of these associations vary across studies due to methodological differences, limited sample diversity, and inconsistent control for confounders. Conclusions: Cardiovascular fitness appears to have a positive, albeit complex, relationship with cognitive function and academic performance in youth. Future research should adopt longitudinal and experimental designs to clarify causal pathways and consider moderating factors such as sex, age, and psychosocial variables. Full article
