Early Positive Fluid Balance Associates with Increased Mortality in Neurological Critically Ill Patients: A 10-Year Cohort Study

Background: Fluid management is a critical aspect of care for neurocritically ill patients, yet the optimal approach remains unclear. The relationship between fluid balance and clinical outcomes in these patients requires further investigation, particularly regarding the timing and volume of fluid administration. Methods: This retrospective observational study analyzed 2186 adult patients admitted to the neurosurgical intensive care unit (ICU) from January 2013 to December 2022. We employed a generalized additive model (GAM) with cubic spline smoothing to examine non-linear relationships between fluid balance and mortality. The maximally selected rank statistics method was used to determine the optimal cutoff value for fluid balance. Associations between fluid balance patterns and 28-day mortality were analyzed using a multivariable logistic regression model. Results: Initial analysis identified fluid balance on day 1 as the most significant predictor of mortality; patients with positive fluid balance showed a higher 28-day mortality. Non-survivors showed significantly higher fluid input throughout the 7-day observation period, particularly during the first 24 h (4444 mL vs. 3978 mL, p = 0.007). Multivariable analysis confirmed that fluid balance on day 1 remained independently associated with 28-day mortality after adjusting for confounders (adjusted odd ratio 1.705, 95% confidence interval: 1.001–2.905, p = 0.049). Additionally, the relationship between fluid input day 1 and mortality demonstrated a progressively increasing probability of 28-day mortality with higher fluid volumes. Early fluid balance, particularly during the first 24 h of ICU admission, shows a significant association with mortality in neurocritically ill patients. Conclusions: These findings emphasize the crucial importance of careful fluid management in the early phase of neurocritical care and suggest that implementation of strict fluid monitoring protocols, especially during the initial period of care, may improve patient outcomes. Although the strawberry tree (A. unedo L.) has been long considered a neglected species of the Mediterranean maquis, the valorization of its fruit production may enhance its socioeconomic value, especially in rural areas. In this study, strawberry trees from different Sicilian sites were investigated in terms of macronutrients, fatty acid (FA) composition, tocopherols, total phenols, carotenoids, and minerals. Sicilian berries were a good source of carbohydrates (mainly fructose, glucose and sucrose) and dietary fiber. They were low in fat; however, the FA composition revealed the abundance of unsaturated FAs over saturated FAs and an advantageous n-6/n-3 ratio. Additionally, Sicilian berries showed an inversed linoleic/α-linolenic acid ratio with respect to berries from other Mediterranean regions, that had previously investigated in literature. This evidence suggests that this ratio may have a chemotaxonomic relevance. Considering antioxidants, the fruits had levels of tocopherols, particularly α-tocopherol, total phenols and carotenoids similar to those of certain commercial fruits. Precious amounts of minerals, such as Ca, K, Zn and Fe were also determined. Interestingly, berries harvested near a Sicilian volcanic area had higher levels of minerals, as well as tocopherols, phenols and carotenoids, than fruits from other Sicilian sites, thereby advancing the hypothesis that fruits from volcanic areas may have a superior nutritional value. Overall, data from this study elaborated by a proper statistical analysis revealed that the geographical origin was a relevant variable to consider in the reliable study of this fruit species. High-Performance Pd-Pt/α-MnO2 Catalysts for the Oxidation of Toluene

Herein, α-MnO2-supported Pt-Pd bimetal (xPd-yPt/α-MnO2; x and y are the weight loadings (wt%) of Pd and Pt, respectively; x = 0, 0.23, 0.47, 0.93, and 0.92 wt%; and y = 0.91, 0.21, 0.46, 0.89, and 0 wt%) catalysts were prepared using the polyvinyl alcohol-protected NaBH4 reduction method. The physicochemical properties of the catalysts were determined by means of various techniques and their catalytic activities for toluene oxidation were evaluated. It was found that among the xPd-yPt/α-MnO2 samples, 0.93Pd-0.89Pt/α-MnO2 showed the best catalytic performance, with the toluene oxidation rate at 156 °C (rcat) and space velocity = 60,000 mL/(g h) being 6.34 × 10−4 mol/(g s), much higher than that of 0.91Pt/α-MnO2 (1.31 × 10−4 mol/(g s)) and that of 0.92Pd/α-MnO2 (6.13 × 10−5 mol/(g s)) at the same temperature. The supported Pd-Pt bimetallic catalysts possessed higher Mn3+/Mn4+ and Oads/Olatt molar ratios, which favored the enhancement in catalytic activity of the supported Pd-Pt bimetallic catalysts. Furthermore, the 0.47Pd-0.46Pt/α-MnO2 sample showed better resistance to sulfur dioxide poisoning. The partial deactivation of 0.47Pd-0.46Pt/α-MnO2 was attributed to the formation of sulfate species on the sample surface, which covered the active site of the sample, thus decreasing its toluene oxidation activity. In addition, the in situ DRIFTS results demonstrated that benzaldehyde and benzoate were the intermediate products of toluene oxidation. Full article

This study aimed to develop a functional powder using whey and milk matrices, leveraging the protective capacity of chia–alginate hydrogels and the advantages of electrohydrodynamic spraying (EHDA), a non-thermal technique suitable for encapsulating probiotic cells under stress conditions commonly encountered in food processing. A hydrogel matrix composed of chia seed mucilage and sodium alginate was used to form a biopolymeric network that protected probiotic cells during processing. The encapsulation efficiency reached 99.0 ± 0.01%, and bacterial viability remained above 9.9 log10 CFU/mL after lyophilization, demonstrating the excellent protective capacity of the hydrogel matrix. Microstructural analysis using confocal laser scanning microscopy (CLSM) revealed well-retained cell morphology and homogeneous distribution within the hydrogel matrix while, in contrast, scanning electron microscopy (SEM) showed spherical, porous microcapsules with distinct surface characteristics influenced by the encapsulation method. Encapsulates were incorporated into beverages flavored with red fruits and pear and subsequently freeze-dried. The resulting powders were analyzed for moisture, protein, lipids, carbohydrates, fiber, and color determinations. The results were statistically analyzed using ANOVA and response surface methodology, highlighting the impact of ingredient ratios on nutritional composition. Raman spectroscopy identified molecular features associated with casein, lactose, pectins, anthocyanins, and other functional compounds, confirming the contribution of both matrix and encapsulants maintaining the structural characteristics of the product. The presence of antioxidant bands supported the functional potential of the powder formulations. Chia–alginate hydrogels effectively encapsulated L. reuteri, maintaining cell viability and enabling their incorporation into freeze-dried beverage powders. This approach offers a promising strategy for the development of next-generation functional food gels with enhanced probiotic stability, nutritional properties, and potential application in health-promoting dairy systems. We consider the problem of privately updating a message out of K messages from N replicated and non-colluding databases where a user has an outdated version of the message W^θ of length L bits that differ from the current version Wθ in at most f bits. The user also has a cache containing coded combinations of the K messages (with a pre-specified structure), which are unknown to the N databases (unknown prefetching). The cache Z contains linear combinations from all K messages in the databases with r=lL being the caching ratio. The user needs to retrieve Wθ correctly using a private information retrieval (PIR) scheme without leaking information about the message index θ to any individual database. Our objective is to jointly design the prefetching (i.e., the structure of said linear combinations) and the PIR strategies to achieve the least download cost. We propose a novel achievable scheme based on syndrome decoding where the cached linear combinations in Z are designed to be bits pertaining to the syndrome of Wθ according to a specific linear block code. We derive a general lower bound on the optimal download cost for 0r1, in addition to achievable upper bounds. The upper and lower bounds match for the cases when r is exceptionally low or high, or when K=3 messages for arbitrary r. Such bounds are derived by developing novel cache-aided arbitrary message length PIR schemes. Our results show a significant reduction in the download cost if f<L2 when compared with downloading Wθ directly using typical cached-aided PIR approaches. Full article

In compressed sensing, it is believed that the L0 norm minimization is the best way to enforce a sparse solution. However, the L0 norm is difficult to implement in a gradient-based iterative image reconstruction algorithm. The total variation (TV) norm minimization is considered a proper substitute for the L0 norm minimization. This paper points out that the TV norm is not powerful enough to enforce a piecewise-constant image. This paper uses the limited-angle tomography to illustrate the possibility of using the L0 norm to encourage a piecewise-constant image. However, one of the drawbacks of the L0 norm is that its derivative is zero almost everywhere, making a gradient-based algorithm useless. Our novel idea is to replace the zero value of the L0 norm derivative with a zero-mean random variable. Computer simulations show that the proposed L0 norm minimization outperforms the TV minimization. The novelty of this paper is the introduction of some randomness in the gradient of the objective function when the gradient is zero. The quantitative evaluations indicate the improvements of the proposed method in terms of the structural similarity (SSIM) and the peak signal-to-noise ratio (PSNR). Full article

Dye residues from the textile industry constitute a critical wastewater problem. This study aimed to evaluate the removal capacity of methylene blue (MB) in aqueous media, using an adsorbent formulated from activated and sonicated nanoclay (NC) and microatomized Nostoc sphaericum (ANS). NC was obtained by acid treatment, followed by activation with 1 M NaCl and sonication, while ANS was obtained by microatomization in an aqueous medium. NC/ANS was mixed in a 4:1 weight ratio. The NC/ANS synergistic adsorbent was characterized by the point of zero charge (PZC), zeta potential (ζ), particle size, FTIR spectroscopy, and scanning electron microscopy (SEM). NC/ANS exhibited good colloidal stability, as determined by pHPZC, particle size in the nanometer range, and heterogeneous morphology with functional groups (hydroxyl, carboxyl, and amide), removing between 72.59 and 97.98% from an initial concentration of 10 ppm of MB, for doses of 20 to 30 mg/L of NC/ANS and pH of 5 to 8. Optimal adsorption conditions are achieved at pH 6.8 and 32.9 mg/L of adsorbent NC/ANS. It was observed that the pseudo-first-order (PFO) and pseudo-second-order (PSO) kinetic models best described the adsorption kinetics, indicating a predominance of the physisorption process, with adsorption capacity around 20 mg/g. Isotherm models and thermodynamic parameters of adsorption, ΔS, ΔH, and ΔG, revealed that the adsorption process is spontaneous, favorable, thermodynamically stable, and occurs at the monolayer level, with a regeneration capacity of 90.35 to 37.54% at the fifth cycle. The application of physical activation methods, such as sonication of the clay and microatomization of the algae, allows proposing a novel and alternative synergistic material from organic and inorganic sources that is environmentally friendly and promotes sustainability, with a high capacity to remove cationic dyes in wastewater. Compared with vanadium extraction by sodium roasting followed by water leaching, the calcification roasting–sulfuric acid leaching method is considered a promising approach for the comprehensive utilization of vanadium titanomagnetite, as it avoids the introduction of alkali metals. However, during vanadium extraction by sulfuric acid heap leaching, the diffusion of leaching reagents and leaching products was hindered by the deposition of leaching solid products. To address this issue, this study systematically investigated the leaching kinetics and the mechanisms underlying the deposition of leaching solid products. The results indicated that vanadium leaching was governed by a combination of liquid film diffusion and internal diffusion through solid-phase products during days 0–2, and by internal diffusion alone from day 2 to day 9. The primary solid products formed during leaching were calcium sulfate and silica gel. Calcium sulfate precipitated and grew within the pore via two-dimensional nucleation, while silicates formed silica gel through dehydration. By optimizing the sulfuric acid leaching conditions—specifically, maintaining an H+ concentration of 2 mol/L, a leaching temperature of 40 °C, and a liquid-to-solid ratio of 5:1—the formation of calcium sulfate and silica gel was effectively suppressed. Under these conditions, the vanadium leaching efficiency reached 75.82%. Full article

This paper addresses the critical challenge of selecting suitable pile foundations in port engineering by systematically investigating the axial bearing behavior of large-diameter steel pipe piles and prestressed high-strength concrete (PHC) piles. The study integrates both numerical simulations and field tests within the context of the Yancheng Dafeng Port Security Facilities Project. A self-balanced static load numerical model for PHC piles was developed using Plaxis 3D, enabling the simulation of load-displacement responses, axial force transfer, and side resistance distribution. The accuracy of the model was verified through a comparison with field static load test data. With the verified model parameters, the internal force distribution of steel pipe piles was analysed by modifying material properties and adjusting boundary conditions. A comparative analysis of the two pile types was conducted under identical working conditions. The results reveal that the ultimate bearing capacities of the 1# steel pipe pile and the 2# PHC pile are 6734 kN and 6788 kN, respectively. Despite the PHC pile having a 20% larger diameter, its ultimate bearing capacity is comparable to that of the steel pipe pile, suggesting a more efficient utilisation of material strength in the latter. Further numerical simulations indicate that, under the same working conditions, the ultimate bearing capacity of the steel pipe pile exceeds that of the PHC pile by 18.43%. Additionally, the axial force distribution along the steel pipe pile shaft is more uniform, and side resistance is mobilised more effectively. The reduction in side resistance caused by construction disturbances, combined with the slenderness ratio (L/D = 41.7) of the PHC pile, results in 33.87% of the pile’s total bearing capacity being attributed to tip resistance. The findings of this study provide crucial insights into the selection of optimal pile types for terminal foundations, considering factors such as bearing capacity, environmental conditions, and economic viability. Treatment-resistant depression (TRD) is associated with immune dysregulation. Ketamine, a rapid-acting antidepressant, may exert effects via immunomodulation. The aim was to examine ketamine’s impact on immune markers in TRD, including T-cell subsets, cytokines, and in vitro T-cell responses. Eighteen TRD inpatients received 0.5 mg/kg iv ketamine. Blood was sampled at baseline, 4 h, and 24 h to analyze T-cell phenotypes (CD28, CD69, CD25, CD95, HLA-DR) and serum cytokines (IL-6, IL-8, IL-10, TNF-α, IL-1β, IL-12p70). In vitro, PBMCs from TRD patients and controls were exposed to low (185 ng/mL) and high (300 ng/mL) ketamine doses. Ketamine induced a transient increase in total T cells and CD4+CD25+ and CD4+CD28+ subsets at 4 h, followed by a reduction in CD4+ and an increase in CD8+ T cells at 24 h, decreasing the CD4+/CD8+ ratio. Activation markers (CD4+CD69+, CD4+HLA-DR+, CD8+CD25+, CD8+HLA-DR+) declined at 24 h. Serum IL-10 increased, IL-6 decreased, and IL-8 levels—initially elevated—showed a sustained reduction. In vitro, high-dose ketamine enhanced the proliferation of TRD CD4+ T cells and dose-dependent IL-8 and IL-6 secretion from activated cells. Ketamine induces rapid, transient immune changes in TRD, including reduced T-cell activation and cytokine modulation. A sustained IL-8 decrease suggests anti-inflammatory effects and potential as a treatment-response biomarker. Full article

The effect of ultrasonic frequencies of 40 kHz/300 W (U-40) and 120 kHz/300 W (U-120) on the aqueous extraction of bioactive compounds from dried whole-fruit powders (DPs) of red-peel/white-flesh (WFP) and red-peel/red-flesh (RFP) pitayas was investigated, and shaking at 120 rpm (S-120) was used for a comparison. The effects of temperature and the solid-to-liquid ratio on the extraction efficiencies of the total phenolic content (TPC) and ferric-reducing antioxidant power (FRAP) of WFP and RFP were evaluated. The impact of extraction time on the aqueous extraction of specific compounds, namely, chlorogenic acid (CGA) and quercetin, from WFP and RFP was assessed with extraction modes of U-40, U-120, and S-120. At 40 °C and a 1/20 (g DP/mL) solid-to-liquid ratio, the use of U-40 achieved higher TPC and FRAP values at 15 min than U-120 and S-120 for WFP. The use of U-40 and U-120 extracted higher amounts of free CGA and free quercetin from WFP and RFP at 15 and 60 min than S-120 but showed different extraction efficiencies for free CGA and free quercetin. This study demonstrates that different ultrasonic frequencies can be applied in the green extraction of target bioactive compounds for use in nutraceutical foods. Full article

Background/Objectives: While ART effectively suppresses HIV viremia, many PLWH exhibit persistent immune dysfunction. This study aimed to assess immune recovery and immune exhaustion (PD-1/PD-L1 expression) in newly diagnosed versus long-term ART-treated individuals. Methods: We analyzed 79 PLWH: 52 newly diagnosed individuals (12-month follow-up) and 27 long-term-treated patients (Ukrainian refugees). Flow cytometry was used to evaluate CD4+ and CD8+ counts, the CD4+/CD8+ ratio, and PD-1/PD-L1 expression on CD3+, CD4+, and CD19+ lymphocytes. ART regimen and HIV subtype were included as covariates in linear regression models. Results: At 12 months, CD4+ counts were similar between groups (median 596.5 vs. 621 cells/μL, p = 0.22), but newly diagnosed patients had higher CD8+ counts (872 vs. 620 cells/μL, p = 0.028) and a lower CD4+/CD8+ ratio (0.57 vs. 1.05, p = 0.0027). Immune exhaustion markers were significantly elevated in newly diagnosed individuals: CD4+ PD-1+ T cells (24.4% vs. 3.85%, p = 0.0002) and CD3+ PD-1+ T cells (27.3% vs. 12.35%, p < 0.0001). Linear regression confirmed group membership independently predicted higher CD3+ (β = +21.92, p < 0.001), CD4+ (β = +28.87, p < 0.0001), and CD19+ (β = +8.73, p = 0.002) percentages. Lipid parameters and SCORE2 did not differ significantly. Conclusions: Despite virologic suppression and CD4+ recovery, immune exhaustion markers remain elevated in newly diagnosed PLWH, suggesting incomplete immune normalization. Traditional parameters (CD4+ count and CD4+/CD8+ ratio) may not fully capture immune status, warranting broader immunologic profiling in HIV care. Full article

Dengue, Zika, yellow fever, chikungunya, and Mayaro arboviruses represent an increasing threat to public health because of the serious infections they cause annually in many countries. Serological diagnosis of these viruses is challenging, making the development of new diagnostic strategies imperative. In this study, we investigated the effectiveness of gold nanorods (GNRs) functionalized with specific anti-dengue and anti-orthoflavivirus antibodies in detecting viral particles. GNRs were created with a length-to-width ratio of up to 5.5, a size of 71.4 ± 6.5 nm, and a light absorption peak at 927 nm, and they were treated with 4 mM polyethyleneimine. These GNRs were attached to a small amount of monoclonal antibodies that target flaviviruses, and the viral particles were detected by measuring the localized surface plasmon resonance using an UV-Vis/NIR spectrometer. The tests found Orthoflavivirus dengue and Orthoflavivirus zikaense in diluted human serum and ground-up mosquitoes, with the lowest detectable amount being 100 PFU/mL. The GNRs described in this study can be used to enhance flavivirus diagnostic tests or to develop new, faster, and more accurate diagnostic techniques. Additionally, the functionalized GNRs presented here are promising for supporting virological surveillance studies in mosquitoes. Our findings highlight a fast and highly sensitive method for detecting Orthoflavivirus in both human and mosquito samples, with a detection limit as low as 100 PFU/mL. Full article

Cyanobacteria are a prolific source of bioactive metabolites with expanding applications in sustainable agriculture and biotechnology. This work explores, for the first time in thermotolerant Colombian isolates, the impact of light spectrum, photoperiod, and irradiance on the co-production of exopolysaccharides (EPS) and indole-3-acetic acid (IAA). Six strains from hot-spring environments were screened under varying blue:red (B:R) LED ratios and full-spectrum illumination. Hapalosiphon sp. UFPS_002 outperformed all others, reaching ~290 mg L−1 EPS and 28 µg mL−1 IAA in the initial screen. Response-surface methodology was then used to optimize light intensity and photoperiod. EPS peaked at 281.4 mg L−1 under a B:R ratio of 1:5 LED, 85 µmol m−2 s−1, and a 14.5 h light cycle, whereas IAA was maximized at 34.4 µg mL−1 under cool-white LEDs at a similar irradiance. The quadratic models exhibited excellent predictive power (R2 > 0.98) and a non-significant lack of fit, confirming the light regime as the dominant driver of metabolite yield. These results demonstrate that precise photonic tuning can selectively steer carbon flux toward either EPS or IAA, providing an energy-efficient strategy to upscale thermotolerant cyanobacteria for climate-resilient biofertilizers, bioplastics precursors, and other high-value bioproducts. Background/Objectives: Platelets play a role in bone metabolism and fracture healing. This study aimed to investigate the association between platelet indices and the derived systemic immune inflammation index (SII) with fracture risk in postmenopausal women. Methods: Platelet count, mean platelet volume, platelet distribution width (PDW), platelet crit, percentage of large platelets (P-LCR), platelet–lymphocyte ratio, and the SII, calculated as (NxP)/L, where N, P, and L represented neutrophils, platelets and lymphocytes counts, respectively, were evaluated. Bone mineral density (BMD) was measured using dual-energy X-ray absorptiometry. Results: A total of 124 women (mean age 68.4 ± 9 years) were stratified into two groups based on the median platelet count; the “lower platelet count group” (n = 58) had a count of 200,000 (174,000 to 226,000), while the “higher platelet count group” (n = 66) had a count of 281,500 (256,500 to 308,500). The higher platelet count group showed a higher hip fracture risk (7.4 vs. 4.5%, p = 0.08) and lower lumbar spine BMD (0.773 vs. 0.83 gr/cm2, p = 0.03). By dividing the participants into two groups with higher SSI (950,848.6 ± 746,097.99) (n = 61) and lower SII (355,751.2 ± 88,662.6) (n = 63), the group with the higher SII showed the higher hip fracture risk (7.4 vs. 3.6%, p = 0.01). Univariate regression analysis revealed correlations between chronological age and PDW (r = 0.188, p = 0.047), and P-LCR (r = 0.208, p = 0.03), as well as associations between vitamin D status and P-LCR (r = −0.301, p = 0.034), and between SII and hip fracture risk (r = 0.12, p = 0.007). Conclusions: Platelet count and SII were associated with fracture risk in postmenopausal women undergoing osteoporosis assessment. Given their reproducibility and cost-effectiveness, these markers warrant further investigation in future prospective studies focused on bone fragility. Full article
