This study evaluated the effect of pearl millet subjected to different processing on the intestinal health of rats. The animals were fed a standard iron-free diet (28 days) (hemoglobin depletion: 8.65 + 1.40 g/dL of hemoglobin). Subsequently, they were divided into four groups for hemoglobin repletion (21 days): standard diet + ferrous sulfate (SD + FS); standard diet + non-germinated open-pan cooked millet flour (SD + NGOPCMF); standard diet + germinated open-pan cooked millet flour (SD + GOPCMF); and standard diet + extrusion-cooked millet flour (SD + ECMF). Hemoglobin level did not differ among groups. The SD + NGOPCMF, SD + GOPCMF and SD + ECMF groups demonstrated a higher Chao index in the microbiome and a higher number and area of goblet cells and longitudinal muscle layer width. The SD + NGOPCMF and SD + GOPCMF groups demonstrated increased cecum weight, crypt depth, crypt thickness, and circular muscle layer width; lower fecal pH; and a higher relative abundance of Bacteroidota, while the SD + FS group showed the highest abundance of Actinobacteriota. The SD + GOPCMF group stood out for showing the lowest fecal pH, better α-diversity (Chao and Shannon index), and the highest width of the longitudinal muscle layer. In conclusion, pearl millet subjected to different processing, mainly germination, has the potential to improve the composition of the intestinal microbiota and the intestinal morphology in rats induced to iron deficiency. Background: Citrinin (CIT) is a mycotoxin produced by various fungi contaminating stored cereals and fruits. While biomonitoring and food occurrence data indicate widespread exposure, its public health risks remain unclear due to the lack of human toxicokinetic (TK) data. Methods: A UHPLC-MS/MS method was validated for CIT quantification in capillary blood (VAMS Mitra® tips), feces, and urine obtaining LLOQs ≤ 0.05 ng/mL. A human TK study was conducted following a single oral bolus of 200 ng/kg bw CIT. Individual capillary blood (VAMS Mitra® tips), feces, and urine samples were collected for 48 h after exposure. Samples were analyzed to determine CIT’s TK profile. Results: TK modeling was performed using a multi-compartmental structure with a hierarchical Bayesian population approach, allowing robust parameter estimation despite the lack of standards for CIT metabolites. Conclusions: The derived TK parameters align with preliminary human data and significantly advance CIT exposure assessment via biomonitoring. A human inter-individual toxicokinetic variability (HKAF) of 1.92 was calculated based on the derived AUC, indicating that EFSA’s current default uncertainty factor for TK variability is adequately protective for at least 95% of the population. Metakaolin-based geopolymers with different molar ratios of Si/Al were synthesized and utilized as an efficient adsorbent for the removal of methylene blue (MB) as a model cationic dye from aqueous solution. Various analytical techniques were employed to characterize the synthesized geopolymers. The influence of the main operation conditions on the adsorption kinetics of MB onto the geopolymer was examined under various operating conditions. Results showed a significant maximum MB adsorption capacity at the temperature of 30 °C for all four types of geopolymers studied (designated as A, B, C, and D) up to 35.3, 23.6, 25.5, and 19.0 mg g−1, respectively. The corresponding order of Si/Al ratio was A < C < B < D. Adsorption kinetics was so fast and reached equilibrium in 10 min, and the experimental results were described using the adsorption dynamic intraparticle model (ADIM). The equilibrium data for MB removal was in agreement with the Langmuir isotherm. Full article

Automated waste classification is an essential step toward efficient recycling and waste management. Traditional deep learning models, such as convolutional neural networks, rely on extensive labeled datasets to achieve high accuracy. However, the annotation process is labor-intensive and time-consuming, limiting the scalability of these approaches in real-world applications. Zero-shot learning is a machine learning paradigm that enables a model to recognize and classify objects it has never seen during training by leveraging semantic relationships and external knowledge sources. In this study, we investigate the potential of zero-shot learning for waste classification using two vision-language models: OWL-ViT and OpenCLIP. These models can classify waste without direct exposure to labeled examples by leveraging textual prompts. We apply this approach to the TrashNet dataset, which consists of images of municipal solid waste organized into six distinct categories: cardboard, glass, metal, paper, plastic, and trash. Our experimental results yield an average classification accuracy of 76.30% with Open Clip ViT-L/14-336 model, demonstrating the feasibility of zero-shot learning for waste classification while highlighting challenges in prompt sensitivity and class imbalance. Despite lower accuracy than CNN- and ViT-based classification models, zero-shot learning offers scalability and adaptability by enabling the classification of novel waste categories without retraining. This study underscores the potential of zero-shot learning in automated recycling systems, paving the way for more efficient, scalable, and annotation-free waste classification methodologies. Background and Objectives: While α1-blockers like silodosin are the mainstay for treating lower urinary tract symptoms (LUTS) due to benign prostatic hyperplasia (BPH), combination therapy with phytotherapeutics may provide enhanced symptom control. Xipag® is a novel formulation containing Graminex® G96 (pollen extract) and Teupol® 25P (teupolioside), offering anti-inflammatory and antiandrogenic effects. This study aimed to evaluate the efficacy of Xipag® versus hexanic extract of Serenoa repens (HESr), both in combination with silodosin, in patients with LUTS/BPH. Materials and Methods: We conducted a single-center, prospective, observational, comparative study involving male patients with moderate-to-severe LUTSs undergoing treatment with silodosin. Patients were allocated to receive either Xipag® or HESr in addition to silodosin, with follow-up every 3 months for 12 months. Primary outcomes included changes in symptom scores such as IPSS, QoL, and functional improvements such as peak urinary flow rate (Qmax). Multivariable regression analyses were used to assess predictors of the response. Results: Patients receiving Xipag® showed significantly greater improvements in Qmax at all follow-up points (p < 0.05), with earlier and more sustained benefits compared to the HESr group. QoL index scores and PSA levels were also significantly better in the Xipag® group starting from month six onward. IPSS scores improved in both groups but were significantly lower in the Xipag® group only at 12 months (p = 0.04). No differences in erectile function (IIEF-5) or adverse events were observed. Conclusions: Xipag® in combination with silodosin provides superior improvement in urinary flow, symptom-related QoL, and PSA reduction compared to HESr plus silodosin, with a favorable safety profile. These findings support the use of multi-target nutraceuticals like Xipag® as a valuable adjunct in the management of LUTS/BPH. Larger randomized trials are warranted to confirm these results and explore underlying mechanisms. Full article

Asymptomatic bacteriuria (ASB) is a very frequent condition in kidney transplant recipients (KTRs). Guidelines advise against screening and treatment of ASB beyond the first month after renal transplantation. Here, we report the case of a 40-year-old female KTR with untreated ASB complicated with allograft pyelonephritis with urosepsis and acute kidney injury. The reported case highlights that ASB remains a grey area in the management of KTRs (after the first month), and there is a need for new ad hoc studies to identify which patients should be screened and eventually treated. Until new findings are available, it is suggested not to treat KTRs with ASB; however, if ASB is detected, stricter monitoring and non-antibiotic prophylaxis are necessary to favor prevention or prompt therapy of severe urinary tract infections. Background: Fermented foods rich in bioactive compounds have been proposed as potential strategy to combat non-communicable diseases. Among them is kombucha, a beverage fermented from sugared Camellia sinensis tea by a symbiotic culture of bacteria and yeasts (SCOBY). Recently, there has been an increased focus on assessing the actual effect of this beverage on human health. In this manner, this systematic review aimed to gather clinical evidence on the impact of kombucha consumption on human health. Methods: The databases Cochrane CENTRAL, MEDLINE/PubMed, and Embase® were searched, and the risk of bias tool used was the Critical Appraisal Tools outlined in the Joanna Briggs Institute. This review followed the PRISMA guidelines and was registered on PROSPERO (CRD42024599464). Results: Eight clinical trials were included (two pre- and post-interventions and six randomized controlled trials) with durations ranging from 10 days to 10 weeks. Two studies reported beneficial effects of kombucha on gastrointestinal symptoms, such as reduced intensity of constipation-related complaints. Two trials observed changes in gut microbiota composition, including increased abundance in Bacteroidota, Akkermansiaceae, Saccharomyces, and Weizmannia coagulans, alongside reductions in Ruminococcus, Dorea, and Rhodotorula. Moreover, five clinical trials evaluated glucose metabolism, evidencing inconsistent results, and other studies identified improvements in salivary microbiota composition and serum metabolomic profile. Conclusion: These findings suggest that kombucha consumption may provide health benefits, particularly in alleviating gastrointestinal symptoms, and demonstrates a modest capacity for modulating gut and salivary microbiota, as well as metabolomic profiles. Although the results are promising, the heterogeneity of the studies and the limited number of available clinical trials highlight the need for further robust research to confirm these effects. (1) Background: postoperative surgical-site infections are a significant complication in small-animal surgical procedures, with detected rates ranging from 0.8% to 18.1%, depending on the type of surgery. The sterilization process of surgical instruments is a crucial factor in infection control, but the shelf life of surgical instruments and the best packaging method are not precisely defined due to the multiple variables that influence them. This study aimed to assess the shelf life of surgical instruments stored under controlled environmental conditions in a veterinary hospital and compare two packaging methods: using a self-sealing single pouch versus a self-sealing double pouch. (2) Methods: a sample of 400 non-sterile screws was divided into three groups: Group 1 (175 screws in single pouches), Group 2 (175 screws in double pouches), and Group 3 (50 non-sterile screws as a control group to verify the microbial culture efficiency). Microbiological tests were conducted at 1, 7, 15, 30, 60, 182, and 390 days post-sterilization. (3) Results: no bacterial growth was detected on screws packaged in single and double pouches up to 182 days. However, after 390 days, bacterial growth was observed in one screw packaged in a single pouch. In Group 3, only two screws turned out to be bacteriologically positive. (4) Conclusions: no statistical difference was found between the two groups; however, the detection of a single positive screw in the single-pouch group raised a potential clinical consideration, suggesting the need for further studies based on events and time. Metabolic reprogramming and epigenetic alterations are fundamental hallmarks of cancer cells, contributing to adaptation, progression, and resistance. In melanoma, high metabolic-epigenetic plasticity enables the rapid modulation of cell states in response to environmental and therapeutic pressures. Recent studies have highlighted a bidirectional crosstalk between cellular metabolism and epigenetic regulation. Epigenetic modifications influence the transcriptional control of metabolic genes, thereby shaping metabolic phenotypes. Conversely, specific metabolites are essential cofactors or substrates for epigenetic enzymes, directly modulating the epigenome. Understanding the intricate mechanisms of this interaction offers opportunities for the development of innovative tumor management that combines epigenetic, metabolic, and therapy interventions. In this review, we summarize the latest evidence on the role of the metabolism–epigenetics axis in melanoma and discuss its potential clinical implications, aiming to provide a comprehensive overview of metabolic/epigenetic interconnections. Cardiac tumors represent rare neoplasms, but they include a very wide range of neoplasia—first primary benign and malignant cardiac tumors, then cardiac metastases, with these latter ones being far more common in adulthood. These diagnoses may be challenging because of frequently non-specific signs and symptoms; for example, their clinical management may be difficult because of the site and because of possible hemodynamic or arrhythmogenic consequences, independent from their biology. Cardiac tumors may be asymptomatic and incidentally diagnosed, or they may cause heart failure, life-threatening arrhythmias, or even sudden cardiac death. Although they may still represent a post-mortem finding, the evolution and the larger use of cardiac imaging tools, initially echocardiography, has progressively and significantly increased their in vivo detection. Magnetic resonance imaging and computed tomography may give crucial information as to the composition and localization of cardiac masses, useful for investigating them and for planning surgery. Histology is mandatory for the definite and differential diagnosis of the cardiac masses, for assessing predictive factors in malignancies, and for then establishing the appropriate management of patients. Modern techniques applied to histology, including immunohistochemistry and molecular biology, may be required to characterize cardiac tumors, to properly classify them and to assess predictive and/or prognostic markers. Surgical procedures, including minimally invasive surgery, have also dramatically evolved in the last decades, allowing adequate treatment in most cardiac tumors. Finally, biopsy may be useful in selected cases, particularly when radical surgery is not feasible, and histological diagnosis is fundamental for other possible therapeutic approaches. The scope of this review covers advancements in the imaging diagnosis, histology, and treatment of primary and secondary cardiac tumors. Full article

Background: Cold dissection (CD) during nerve-sparing robot-assisted radical prostatectomy (nsRARP) in patients with prostate cancer has been proposed to improve functional outcomes by reducing the thermal damage to neurovascular bundles (NVBs). This study compares the impact of CD versus thermal dissection (TD) on postoperative early continence and erectile function recovery. Methods: A prospective comparative analysis was conducted on patients undergoing nsRARP, comparing CD and TD techniques. Continence was assessed at 15, 30, and 90 days, while erectile function was evaluated at 30, 90, and 180 days using IIEF-5 scores. Logistic and linear regression analyses were performed to identify predictors of functional recovery. Results: CD significantly improved early continence at 15 and 30 days (p < 0.05), although the difference diminished by 90 days. Erectile function recovery was consistently higher in the CD group at all time points (p < 0.01). Age negatively impacted continence and erectile function recovery, while preoperative IIEF-5 scores were strong predictors of erectile function. Prostate volume, BMI, and metabolic syndrome showed no significant influence on outcomes. Conclusions: CD enhances early continence and erectile function recovery following nsRARP by preserving NVBs and minimizing thermal damage. This technique offers a valuable approach to improving patient functional outcomes. Further studies are required to validate its long-term benefits. Full article

Surgical Antibiotic Prophylaxis in Small Animal Surgery: A Retrospective Outcome-Based Study from the Veterinary Teaching Hospital of Naples

Hospital-acquired infections in operating rooms are contributing factors to the increasing presence of pathogens, with surgical site infections being the most frequent among them. Surgical antimicrobial prophylaxis is administered to minimize the risk of developing surgical site infection. Surgeons are responsible for weighing the benefit of surgical site infection-risk reduction for the individual patient afforded by prophylactic antimicrobial administration against the broader societal risk from such use. The aim of this study was to analyze the data on surgical antimicrobial prophylaxis related to patient status, surgery duration, wound classification and the occurrence of surgical site infection in two operating rooms of the Veterinary Teaching Hospital of Naples from January 2023 to January 2024. Two hundred seventy-seven surgical procedures met the inclusion criteria. For soft tissue clean-contaminated procedures and clean orthopedic surgeries not involving implants, no statistically significant difference was observed between the various antibiotic protocols used and surgical site infection; this finding supports the possibility of avoiding antibiotic use for these procedures. In contrast to previous reports, this study identified a statistically significant association between ASA status and surgical site infection; no correlation was found between surgical duration and the occurrence of infection. Further research on antibiotic use and risk factors associated with surgical site infection will be crucial to strengthen clinical guidelines. Full article

Proline-rich antimicrobial peptides (PrAMPs) primarily exert their antimicrobial effects intracellularly, inhibiting protein synthesis. B7-005, a synthetic 16-amino acid PrAMP, has a broader antimicrobial spectrum compared to native counterparts, despite shorter PrAMPs typically exhibiting reduced activity. This study aimed to enhance B7-005’s potency by extending it with 6 or 11 amino acids derived from the C-terminal sequences of cetacean Tur1A and Lip1 PrAMPs, as well as bovine Bac7(1-35). Six chimeric derivatives were evaluated for antimicrobial and bactericidal potency, cytotoxicity, bacterial membrane permeabilization, and in vitro inhibition of protein synthesis. Extending B7-005 with sequences from other PrAMPs increased its activity against most ESKAPE+E pathogens, reducing minimum inhibitory concentration (MIC) values by 2- to 8-fold, with notable differences among bacterial species, without increasing cytotoxicity toward the A549 cell line. All chimeras retained the ability to inhibit protein synthesis in Escherichia coli and to modestly perturb the E. coli membranes like B7-005. These novel chimeric PrAMPs, particularly the 22-mer derivatives, hold promise for developing new antimicrobial agents. The study also highlights variability in bacterial responses to PrAMPs and underscores how minor sequence differences can significantly impact efficacy against specific microorganisms. PrAMPs thus represent a valuable scaffold to rationally design derivatives targeting high-priority pathogens. Full article

Background and Objectives: Mitral valve transcatheter edge-to-edge repair (TEER) is a widely adopted therapeutic approach for managing significant mitral regurgitation (MR) in high-risk surgical candidates. While procedural safety and efficacy have been demonstrated, the impact of institutional expertise on outcomes remains unclear. We aimed at evaluating whether the institutional monthly volume of TEER influences short- and long-term clinical results. Materials and Methods: This analysis from the multicenter, prospective GIOTTO trial study evaluated the impact of institutional monthly volume on outcomes of TEER to remedy significant mitral regurgitation. Centers were stratified into tertiles based on monthly volumes (≤2.0 cases/month, 2.1–3.5 cases/month, >3.5 cases/month), and key clinical, echocardiographic, and procedural outcomes were analyzed. Statistical analysis was based on standard bivariate tests as well as unadjusted and multivariable adjusted Cox models. Results: A total of 2213 patients were included, stratified into tertiles based on institutional procedural volume: 645 (29.1%) patients in the first tertile, 947 (42.8%) patients in the second tertile, and 621 (28.1%) patients in the third tertile. Several baseline differences were found, with some features disfavoring less busy centers (e.g., functional class and surgical risk, both p < 0.05), and others suggesting a worse risk profile in those treated in busier institutions (e.g., frailty and history of prior mitral valve intervention, both p < 0.05). Procedural success rates were higher in busier centers (p < 0.001), and hospital stay was also shorter there (p < 0.001). Long-term follow-up (median 14 months) suggested worse outcomes in patients treated in less busy centers at unadjusted analysis (e.g., p = 0.018 for death, p = 0.015 for cardiac death, p = 0.014 for death or hospitalization for heart failure, p < 0.001 for cardiac death or hospitalization for heart failure), even if these associations proved no longer significant after multivariable adjustment, except for cardiac death or hospitalization for heart failure, which appeared significantly less common in the busiest centers (p < 0.05). Similar trends were observed when focusing on tertiles of overall center volume and when comparing for each center the first 50 cases with the following ones. Conclusions: High institutional monthly volume of TEER mitral valve repair appears to correlate with an improved procedural success rate and shorter hospitalizations. Similarly favorable results were found for long-term rates of cardiac death or hospitalization for heart failure. These findings inform on the importance of operator experience and center expertise in achieving state-of-the-art results with TEER, while confirming the usefulness of the proctoring approach when naïve centers begin a TEER program. Post-Insemination Infusion of Wharton’s Jelly Mesenchymal Stromal/Stem Cells-Derived Conditioned Medium: A Novel Approach for Improving Pregnancy Outcomes in Problem Mares

Treating endometritis is one of the major challenges faced by veterinarians. The effect of post-insemination intrauterine infusion Wharton’s jelly mesenchymal stromal/stem cell-derived conditioned medium (WJ-MSC-CM) on the uterine response and pregnancy outcomes in problem mares was evaluated in this study. The cycles of 12 problem mares were treated seven/eight hours after insemination with intrauterine infusion of 20 mL of WJ-MSC-CM (TRT) or non-conditioned Ringer’s lactate solution (CTR). Before (PRE) and 12 h after (POST) treatment, an ultrasonographic examination and a low-volume flush (LVF) for efflux clarity evaluation and bacteriological and cytological analysis were made. Pregnancy diagnosis was performed at 14 days and confirmed at 60 days after ovulation. A decrease (p < 0.05) in polymorphonuclear neutrophils infiltration and a trend toward significance in LVF efflux clarity (p < 0.1) were observed in TRT cycles. The TRT (50%) had a higher pregnancy rate (p < 0.05) than CTR group (0%) at first cycle. Only a trend toward significance was observed comparing pregnancy frequencies between the two groups (CTR 12.5% vs. TRT 54.5%), the pregnancy rate of TRT cycles was comparable to that of normal mares (p > 0.05) and higher (p < 0.05) than that reported for problem mares in the literature. Post-insemination uterine infusion of WJ-MSC-CM in mares restores pregnancy rates to normal levels, by its anti-inflammatory effect. Full article
