Dear Colleagues,
Rheumatic and musculoskeletal diseases are complex, multisystem disorders characterized by interactions among chronic inflammation, immune dysregulation, metabolic alterations, vascular dysfunction, body composition changes, and psychosocial factors. Although these conditions primarily affect the joints, muscles, bones, and connective tissues, their clinical consequences frequently extend to the cardiovascular, renal, hematological, neurological, endocrine, and microvascular systems. Chronic kidney disease (CKD) is an increasingly recognized comorbidity in rheumatic conditions, driven by shared inflammatory and metabolic pathways, nephrotoxic treatments, and disease-specific renal involvement. Furthermore, sex-based differences profoundly influence disease susceptibility, clinical presentation, metabolic profiles, treatment responses, and long-term outcomes across the spectrum of rheumatic diseases. Identifying reliable metabolic and systemic biomarkers that account for both renal involvement and sex-specific variation may therefore improve understanding of disease mechanisms, risk stratification, treatment monitoring, and individualized patient management.
This Special Issue aims to bring together clinical, translational, and interdisciplinary studies investigating metabolic biomarkers, multisystem alterations, chronic kidney disease, and sex differences in inflammatory and non-inflammatory rheumatic and musculoskeletal conditions. Particular attention will be given to the relationships between systemic inflammation, immunometabolism, vascular and microvascular dysfunction, renal metabolic complications, hematological indices, bone and muscle metabolism, sarcopenia, body composition, oxidative stress, neuroimmune mechanisms, and psychosocial manifestations, with an emphasis on how these parameters differ between sexes and across the CKD spectrum.
We welcome original research articles, reviews, systematic reviews, meta-analyses, and brief reports examining metabolic and clinical biomarkers in conditions such as rheumatoid arthritis, psoriatic arthritis, spondyloarthritis, Behçet’s disease, fibromyalgia, osteoporosis, sarcopenia, and related disorders. Studies evaluating renal function and CKD prevalence in rheumatic populations; sex-based differences in disease presentation and biomarker profiles; vascular risk; retinal microcirculation; hematological parameters; body composition; disease activity; treatment-related metabolic responses; and interactions among the immune, musculoskeletal, cardiovascular, renal, and nervous systems are particularly encouraged. By integrating metabolic, renal, and sex-specific perspectives, this Special Issue seeks to support the development of more comprehensive and personalized approaches to the assessment and management of rheumatic and musculoskeletal diseases.

Manuscripts can be submitted until the deadline. All submissions that pass pre-check are peer-reviewed. Accepted papers will be published continuously in the journal (as soon as accepted) and will be listed together on the special issue website. Research articles, review articles as well as short communications are invited. For planned papers, a title and short abstract (about 250 words) can be sent to the Editorial Office for assessment.
Submitted manuscripts should not have been published previously, nor be under consideration for publication elsewhere (except conference proceedings papers). All manuscripts are thoroughly refereed through a single-anonymized peer-review process.
