Dear Colleagues,

Sleep breathing disorders in critical care represent a key factor that impacts mechanically ventilated patients (invasive or noninvasive ventilation). Although there is a growing interest in early diagnosis and appropriate treatment to improve outcomes in mechanically ventilated critical care patients, there are important and complex conditions in the critical care environment that limit an appropriate approach. In the last year, an important amount of research has confirmed these conditions, and in parallel, new tools to resolve this problem have been offered.

In this Special Issue, we analyze the pathophysiology and clinical determinants of sleep breathing disorders in more common conditions that impact short- and long-term outcomes, such as weaning mechanical ventilation, prevention and treatment of estuation failure, and the spectrum chronic and acute respiratory diseases (e.g., acute respiratory distress syndrome, severe pneumonia, and cardiac pulmonary edema). Additionally, we describe barriers and the best appropriate approach for early diagnosis in high-risk populations (e.g., those with obstructive sleep apnea and obesity hypoventilation). Clinical conditions in mechanically ventilated critical ill patients (from nasal high flow, non-invasive and invasive, and tracheostomized) and sleep breathing disorders are presented, and the best diagnosis and appropriate prevention measurement are described. The high prevalence and cost associated with failure weaning from mechanical ventilation in SBD justify the proposal of this Special Issue, entitled “Sleep Breathing Disorders in Critical Care Medicine”.
