Innovate to Prevent: Pathophysiology, Personalized Care and Future of Preeclampsia Management

Preeclampsia remains one of the leading causes of maternal and perinatal morbidity and mortality worldwide. Once defined primarily by clinical symptoms, it is now recognized as a complex, multisystem disorder rooted in abnormal placentation, endothelial dysfunction, immune maladaptation, and angiogenic imbalance. Despite advances in antenatal surveillance and timely delivery strategies, definitive treatment still relies on the delivery of the placenta.

In recent decades, the integration of biomarker research, Doppler studies, and risk-based aspirin prophylaxis has shifted the paradigm from reactive management toward early prediction and prevention. Nevertheless, implementation gaps, inequities in access to screening, and the heterogeneous phenotypes of the disease continue to pose significant challenges for clinicians and researchers.

We are pleased to invite you to contribute to this Special Issue, which aims to advance innovation-driven strategies for the prevention and personalized management of preeclampsia. Our goal is to bridge mechanistic science with clinical translation, with a focus on early risk stratification, individualized preventive interventions, and health system implementation strategies. This Issue will explore the continuum from molecular pathophysiology to real-world clinical application, with particular attention to improving outcomes across diverse populations and resource settings.

We welcome contributions that provide emerging insights into the following:

• Molecular and immunologic mechanisms of abnormal placentation.
• Angiogenic biomarkers and multi-marker predictive models.
• First-trimester screening algorithms integrating maternal, biophysical, and biochemical factors.
• Advanced imaging modalities and innovations in uterine artery Doppler assessment.
• Artificial intelligence and machine learning-based prediction tools.
• Pharmacologic prevention strategies beyond aspirin
• Long-term cardiovascular implications for mothers and offspring
• Implementation science addressing barriers to screening and prevention

For this Special Issue, we invite high-quality submissions including:

• Original research (basic, translational, and clinical studies).
• Prospective cohort studies and validation of predictive models.
• Randomized controlled trials and implementation research.
• Systematic reviews and meta-analyses.
• Narrative reviews on emerging concepts.
• Health policy and global health perspectives.

This Special Issue aims to foster interdisciplinary collaboration and accelerate the translation of innovation into clinical practice—moving from prediction to prevention, and from uniform protocols to precision maternal care.
