Dear Colleagues,
Sjogren’s disease (SjD) is a complex, multisystem, but often under-recognized autoimmune disease. SjD is primarily characterized by the lymphocytic infiltration of target tissues. The disease can present with diverse symptoms and signs and require a variety of clinical specialists, ranging from ocular and oral clinicians to those in neurology, pulmonology, rheumatology, and primary care.
Key to diagnosing SjD is being aware of the diverse spectrum of possible disease manifestations that can occur over a long time period, considering SJD in differential diagnosis, and connecting the dots to create a hypothesis. This hypothesis can be tested with current serology, imaging, and tissue pathology studies to diagnose SjD. In caring for patients diagnosed with SjD, it is important to include ocular and oral experts in order to prevent vision and dental damage.
The pathogenesis of SjD can involve innate and adaptive immune dysregulation, which can guide treatment strategies. Several clinical practice guidelines are available for managing SjD. However, there is not yet a regulatory-approved medication for SjD. This may soon change, as two out of several late-stage clinical development programs have successfully achieved their primary clinical outcome measures.
