Dear Colleagues,
Cirrhosis is the final pathway of chronic liver disease and a major cause of death, hospitalization, and healthcare expenditure worldwide. While compensated cirrhosis may remain stable for years, acute decompensation and further decompensation define clinically meaningful transitions associated with infections, renal dysfunction, portal hypertension-related events, acute-on-chronic liver failure, and increased mortality. At the same time, the evolving concept of recompensation highlights that advanced chronic liver disease is dynamic and may improve after removal or control of the underlying cause.
This Special Issue aims to present and disseminate recent advances in the pathophysiology, diagnosis, risk stratification, and management of cirrhosis, with particular emphasis on decompensated cirrhosis and its complications. We welcome original studies and reviews addressing kidney injury in cirrhosis, including acute kidney injury, acute kidney disease, chronic kidney disease, and hepatorenal syndrome, as well as bacterial infections, multidrug-resistant organisms, spontaneous bacterial peritonitis, sepsis, and infection-driven organ failure. Contributions focusing on further decompensation, recompensation, biomarkers, prognostic models, therapeutic strategies, transplant referral, and clinically relevant outcomes are particularly encouraged. By integrating mechanistic, epidemiological, and clinical research, this issue seeks to support more precise management and better patient-centered outcomes.
Topics of interest for publication include, but are not limited to, the following:
- Natural history, epidemiology, and prognostic assessment of cirrhosis;
- Acute decompensation, further decompensation, and acute-on-chronic liver failure;
- Recompensation of cirrhosis: definitions, predictors, and clinical impact;
- Acute kidney injury, acute kidney disease, chronic kidney disease, and hepatorenal syndrome in cirrhosis;
- Bacterial infections, spontaneous bacterial peritonitis, sepsis, and multidrug-resistant organisms in cirrhosis;
- Biomarkers and risk stratification tools for decompensated cirrhosis;
- Novel therapeutic strategies, management algorithms, and transplant referral in advanced chronic liver disease.
