Dear Colleagues,
In recent years, the management of hepatocellular carcinoma (HCC) has undergone a profound transformation thanks to the introduction of immunotherapy and combined treatments involving locoregional approaches. This Special Issue aims to explore the most innovative and personalized strategies in the treatment of HCC and liver transplantation, with particular focus on patients in BCLC-B and BCLC-C stages. For BCLC-B patients, the therapeutic goal is shifting from simple disease control to downstaging toward resection or liver transplantation through the use of TACE or TARE alone or combined with immunotherapy. In BCLC-C patients, different clinical scenarios emerge: in cases with extrahepatic spread, the goal remains prolonging survival, while in those with vascular invasion, surgical downstaging may be possible in selected cases. Special attention is also given to the evolution of transplant criteria beyond the Milan criteria and to assessing the risk of post-transplant recurrence. Additional topics include clinical and biological selection of candidates, treatment response as a prognostic marker, and future perspectives in precision medicine, including genomic profiling and the use of biomarkers. This Special Issue aspires to provide an updated and multidisciplinary overview of HCC in the era of personalized therapy.
