Dear Colleagues,
Neurological complications remain a major cause of perioperative morbidity and long-term disability in patients undergoing surgery and critical care. Perioperative brain injury, delirium, stroke, hypoxic–ischemic injury, and secondary neurological insults in the intensive care unit are associated with prolonged hospitalization, impaired quality of life, and increased healthcare utilization. The perioperative and critical care periods represent crucial windows during which early identification of neurological vulnerability and timely, targeted interventions may prevent or attenuate brain injury. Some additional focus on the rehabilitation of post-ICU patients and the prevention of critical illness neuropathies would further strengthen the application.
This Special Issue focuses on clinically relevant and innovative strategies to improve neurological outcomes across the perioperative and critical care continuum. Particular emphasis will be placed on novel approaches to cerebral monitoring and neuroprotection, including advanced multimodal neuromonitoring, individualized hemodynamic, ventilatory, and oxygenation targets, and data-driven clinical decision support. Contributions addressing precision sedation, perioperative delirium prevention, integration of artificial intelligence, and real-time physiological signal analysis to detect and limit secondary brain injury are especially encouraged. Translational studies, emerging biomarkers, and innovative therapeutic strategies with direct applicability at the bedside are strongly welcome. By promoting multidisciplinary collaboration between anesthesiology, neurocritical care, and intensive care medicine, this Special Issue aims to advance evidence-based, patient-centered approaches and improve both short- and long-term neurological outcomes.
