Bridging Molecular Endotypes and Clinical Phenotypes in Sleep Apnea-From Mechanistic Insights to Precision Therapeutics

Dear Colleagues,
Obstructive sleep apnea (OSA) is a biologically heterogeneous disorder arising from complex interactions among anatomical deformity, neuromuscular control, sleep-state dependency, and metabolic regulation. This Special Issue, “Mechanisms and Treatments of Sleep Apnea: From Phenotype, Preventive Medicine to Public Health,” emphasizes phenotype-driven mechanisms to advance molecular and translational understanding of OSA. We highlight six representative phenotypes—REM-predominant OSA, positional OSA, comorbid insomnia and sleep apnea (COMISA), excessive daytime sleepiness, obesity-related OSA, and craniofacial restriction involving retrognathia—each associated with distinct pathophysiological and metabolic signatures. In addition to targeted therapies, this Special Issue explores preventive perspectives, including evidence that sleep surgery may ameliorate insulin resistance, as reflected by improvements in HOMA-IR. At the population level, lifestyle-based interventions such as home gardening are discussed for their potential molecular and cardiometabolic benefits. Together, these contributions integrate mechanistic insights with phenotype-specific interventions across individual and population scales.
