Diabetic kidney disease (DKD) is the leading cause of end-stage kidney disease, accounting for approximately 45% of all cases, and represents a major risk factor for myocardial infarction and stroke. Although intensive treatment with insulin, renin–angiotensin system (RAS) blockers, sodium–glucose cotransporter 2 inhibitors (SGLT2i) and other agents can slow DKD progression, it rarely halts or reverses the disease. Thus, novel therapeutic strategies are urgently required. A thorough understanding of the underlying pathogenic mechanisms is essential for developing such interventions.
Oxidative stress, triggered by elevated glucose (hyperglycemia) and/or free fatty acids (FFAs) (hyperlipidemia), plays a central role in the pathogenesis of both microvascular and macrovascular diabetic complications. Oxidative stress is defined as tissue injury resulting from excessive production of reactive oxygen species (ROS) that overwhelms intrinsic antioxidant defense systems. Enhanced ROS generation has long been implicated in renal cellular processes such as mitogenesis, hypertrophy, senescence, fibrosis and apoptosis, though the precise mechanisms remain incompletely characterized.
In recent years, multiple oxidative-stress-related molecules have been identified, and their roles in mitochondrial dysfunction, cellular senescence, fibrosis and injury have been increasingly documented. These discoveries provide valuable insights into potential antioxidant therapeutic strategies. Nevertheless, further studies are needed to elucidate their molecular mechanisms in both in vitro and in vivo models before translation into clinical application.
