Section Information
Chronic diseases are a leading cause of death worldwide. The Chronic Care section focuses on the long-term management of persistent health conditions. It encompasses comprehensive care planning and continuity of care, aiming to improve patient outcomes, well-being, and quality of life, as well as prevent complications and multimorbidity. This includes symptom monitoring, medication management, lifestyle interventions, and patient-centered approaches. Advanced studies in these areas, including multidisciplinary collaborations for supporting individuals with chronic illnesses, are welcome.
