Cover Story: Myocardial infarction is the principal cause of death and disability in Europe. In recent decades, primary percutaneous intervention (PCI) has improved outcomes of ST-segment-elevation myocardial infarction (STEMI) patients. Notably, mortality in recent years has been unchanged due to the suboptimal myocardial reperfusion that many patients experience. This is related to longer ischemic times and some pathophysiological events, such as microvascular obstruction, distal embolization, and reperfusion lesion. All these phenomena lead to a greater infarct size and, subsequently, to the development of heart failure. Recently, several innovative treatments have been developed to overcome this issue. In this article, we review new interventional therapies beyond stenting that aim to improve results in STEMI patients. View this paper
