With the continuous increase in the proportion of distributed energy output in the distribution network and the limited equipment on the management side of the active distribution network, it is very important to give full play to the regulating role of the dispatchable potential of large-scale electric vehicles for the economic operation of the distribution network. To deal with this issue, this paper proposes an optimal dispatching model of the distribution network considering the combination of the dispatchable potential of electric vehicle clusters and demand response. Firstly, the active distribution network dispatching model with the demand response is introduced, and the equipment involved in the active distribution network dispatching is modeled. Secondly, the bidirectional long short-term memory network algorithm is used to process the historical data of electric vehicles to reduce the uncertainty of the model. Then, the shared energy-storage characteristics based on the dispatchable potential of electric vehicle clusters are fully explored and the effect of peak shaving and valley filling after the demand response is fully explored. This approach significantly reduces the network loss and operating cost of the active distribution network. Finally, the modified IEEE-33 bus test system is utilized for test analysis in the case analysis, and the test results show that the established active distribution network model can reduce the early construction cost of the system’s energy-storage equipment, improve the energy-utilization efficiency, and realize the economic operation of the active distribution network. The switching characteristics of GaN HEMT devices exhibit a very complex dynamic nonlinear behavior and multi-physics coupling characteristics, and traditional switching current models based on physical mechanisms have significant limitations. This article adopts a hybrid architecture of convolutional neural network and long short-term memory network (CNN-LSTM). In the 1D-CNN layer, the one-dimensional convolutional neural network can automatically learn and extract local transient features of time series data by sliding convolution operations on time series data through its convolution kernel, making these local transient features present a specific form in the local time window. In the double-layer LSTM layer, the neural network model captures the transient characteristics of switch current through the gating mechanism and state transfer. The hybrid architecture of the constructed model has significant advantages in accuracy, with metrics such as root mean square error (RMSE) and mean absolute error (MAE) significantly reduced, compared to traditional switch current models, solving the problem of insufficient accuracy in traditional models. The neural network model has good fitting performance at both room and high temperatures, with an average coefficient close to 1. The new neural network hybrid architecture has short running time and low computational resource consumption, meeting the needs of practical applications. Full article

Introduction: Vitamin D deficiency, a reversible cause of osteoporosis, is increasingly prevalent, showing varying degrees of severity that are notably pronounced among the growing population of multimorbid elderly patients. Given that the aging pancreas undergoes senescent processes leading to impaired function—which negatively impacts enteral vitamin D absorption and, consequently, elderly bone metabolism—a specific diagnostic and treatment approach is crucial. Our study aimed to determine the prevalence of vitamin D deficiency and exocrine pancreatic insufficiency (EPI) in orthogeriatric patients. We also evaluated differences in vitamin D deficiency severity between patients with normal and impaired pancreatic function. Furthermore, a short-term monitoring of vitamin D level increases after 12 days of substitution therapy in both groups aimed to inform osteoanabolic therapy for specific high-fracture-risk patients, assessing the influence of pancreatic function on substitution efficacy. Methods: We conducted a retrospective, monocentric cohort study, evaluating data from all patients hospitalized with manifest osteoporosis in an orthogeriatric department during a six-month spring/summer period. Demographic data, relevant comorbidities, the type of fracture, the amount of faecal elastase 1 (CALEX® Cap Bühlmann), and the serum levels of 25-hydroxyvitamin D (25(OH)D) were assessed. Results: We found a high prevalence (70.6%) of vitamin D deficiency (25(OH)D < 30 µg/L) among all orthogeriatric patients. Of these, 16% met the criteria for mild to severe EPI. The group with normal exocrine pancreatic function showed a higher average vitamin D value, and their increase in vitamin D levels following short-term substitution was up to 100% greater compared to the group with impaired pancreatic function. Notably, 69% of women and 20% of men met the therapeutic threshold for specific osteoanabolic osteoporosis therapy, even without a T-score. Conclusions: Our findings reveal a very high prevalence of vitamin D deficiency and a high prevalence of EPI in orthogeriatric patients. Those with impaired exocrine pancreatic function exhibit lower baseline vitamin D levels and a diminished capacity for vitamin D absorption during short-term monitoring. These results have significant clinical implications for osteoporotic therapy, given that a substantial proportion of patients, particularly women, meet the criteria for specific osteoanabolic treatment. This article presents a comprehensive review of methods used for forecasting electricity consumption. The studies analyzed by the authors encompass both classical statistical models and modern approaches based on artificial intelligence, including machine-learning and deep-learning techniques. Electricity load forecasting is categorized into four time horizons: very short term, short term, medium term, and long term. The authors conducted a comparative analysis of various models, such as autoregressive models, neural networks, fuzzy logic systems, hybrid models, and evolutionary algorithms. Particular attention was paid to the effectiveness of these methods in the context of variable input data, such as weather conditions, seasonal fluctuations, and changes in energy consumption patterns. The article emphasizes the growing importance of accurate forecasts in the context of the energy transition, integration of renewable energy sources, and the management of the evolving electricity system, shaped by decentralization, renewable integration, and data-intensive forecasting demands. In conclusion, the authors highlight the lack of a universal forecasting approach and the need for further research on hybrid models that combine interpretability with high predictive accuracy. This review can serve as a valuable resource for decision-makers, grid operators, and researchers involved in energy system planning. Full article

Background: Patient care following botulinum toxin injections has long been guided by anecdotal instructions, often based on theoretical considerations. This study evaluates the necessity of extended post-treatment instructions by analyzing outcomes and satisfaction in patients who followed only a 10 min precaution protocol. Materials and Methods: A retrospective, multicentric study was conducted across six Italian centers, analyzing 5014 patients treated with botulinum toxin for upper facial wrinkles between 2015 and 2020. Outcomes included adverse effects—particularly upper eyelid ptosis—and patient satisfaction. Follow-up was performed at two weeks. Results: No cases of upper eyelid ptosis were observed. Among 4000 patients who attended follow-up, adverse effects occurred in only 5.99%, notably lower than rates reported in the literature. Of the 2010 patients who completed the satisfaction questionnaire, 90% reported being very satisfied. These findings support the safety of limiting post-treatment instructions to 10 min. Conclusions: Our findings indicate that omitting extended post-injection instructions does not negatively impact patient satisfaction or complication rates. Given the toxin’s rapid internalization and localized effect, extended behavioral restrictions may be redundant. However, the absence of a control group and lack of statistical analyses limit the strength of these conclusions. In addition, this is a short-term study. Future prospective, randomized trials are needed to develop evidence-based post-care protocols to optimize esthetic outcomes, patient safety, and long-term efficacy. Full article

Osteoporosis is a chronic disease associated with significant morbidity and mortality and requires long-term therapy. Efficacious and well-tolerated treatments are available, but their effect is either short-lived or lost following their discontinuation. The exception is bisphosphonates that reduce bone resorption and turnover, can be administered in regimens ranging from once-daily to once-yearly, and have been shown in randomized clinical trials to reduce the incidence of all osteoporotic fractures, but their effect persists following their discontinuation. This is due to their property of being taken-up selectively by the skeleton and being slowly released following treatment arrest. This property allows the discontinuation of bisphosphonate treatment for different periods of time, the so-called drug holiday, which reduces the risk of rare adverse events while maintaining the effect; an action particularly important for patients at very high risk of fractures for whom sequential therapy with different agents is currently advised. Thus, bisphosphonates, apart from being the treatment of choice for certain groups of patients, are also indispensable for the consolidation and maintenance of the gains of all other treatments, providing, in addition, the opportunity of temporary treatment arrest. Most patients with postmenopausal osteoporosis will, therefore, receive bisphosphonate at some stage during therapy of their disease, regardless of their initial fracture risk. Full article

The integration of shared autonomous vehicles into historic urban areas presents both opportunities and challenges. In heritage-rich environments like very old Asian (such as Suzhou old town, which serves as a use case example) or European (especially Mediterranean coastal cities) areas—characterized by narrow alleys, dense development, and sensitive cultural landscapes—shared autonomous vehicle adoption raises critical spatial and social questions. This study employs a qualitative, user-centered approach based on the ripple model to examine residents’ perceptions across four dimensions: residential patterns, parking land use, regional accessibility, and street-level infrastructure. Semi-structured interviews with 27 participants reveal five key findings: (1) public trust depends on transparent decision-making and safety guarantees; (2) shared autonomous vehicles may reshape generational residential clustering; (3) the short-term parking demand remains stable, but the long-term reuse of space is feasible; (4) shared autonomous vehicles could enhance accessibility in historic cores; (5) transport systems may evolve toward intelligent, human-centered designs. Based on these insights, the study proposes three strategies: (1) transparent risk assessment using explainable artificial intelligence and digital twins; (2) polycentric development to diversify land use; (3) hierarchical street retrofitting to balance mobility and preservation. While this study is limited by its qualitative scope and absence of simulation, it offers a framework for culturally sensitive, small-scale interventions supporting sustainable mobility transitions in historic urban contexts. Background/Objectives: Dyspnea is one of the main causes of visits to the Emergency Department (ED) and hospitalization, with its differential diagnosis representing a challenge for the clinician. Lung ultrasound (LUS) is a widely used tool in ED. The objective of this study was to evaluate the impact of LUS, performed by a non-expert operator, in determining diagnosis and prognosis of patients with dyspnea. Methods: A total of 60 patients presenting with dyspnea at the ED were prospectively enrolled and underwent LUS examination by a medical student, after brief training, within 3 h of triage. LUS findings were classified into four patterns: N.1, absence of notable ultrasound findings, attributable to COPD/ASMA exacerbation; N.2, bilateral interstitial syndrome, suggestive of acute heart failure; N.3, subpleural changes/parenchymal consolidations, suggestive of pneumoniae; and N.4, isolate polygonal triangular consolidation, attributable to infarction in the context of pulmonary thromboembolism. Results: The diagnostic hypothesis formulated after LUS was compared with the final diagnosis after further investigations in the ED, showing agreement in 90% of cases. The mean LUS score value was higher in patterns N.2 (18.4 ± 8.5) and N.3 (17 ± 6.6), compared to patterns N.1 and N.4 (9.8± 6.7 and 11.5 ± 2.1). Given the high prevalence of pattern N.2, the diagnostic accuracy of LUS in this context was further evaluated, showing a sensitivity of 82% and specificity of 100%. In terms of the prognostic value of LUS, hospitalized patients had a higher LUS score compared to those discharged (17.3 ± 8.1 vs. 8.5 ± 6.8, p value 0.004). A similar trend was obtained in the subgroup of patients requiring non-invasive ventilation (NIV), who present a higher LUS score (21.1 ± 6.6 vs. 13.1 ± 8.1, p value 0.002). When considering a combined outcome (death and NIV), patients with worse outcomes more often had a LUS score > 15 (p value < 0.001). Conclusions: In conclusion, this study confirms that LUS is a very useful tool in the ED, assisting the clinical evaluation for diagnosis, treatment decision, and determination of the appropriate care setting for patients with acute dyspnea. Its short learning curve allows even non-expert staff to use it effectively. Full article

InSAR Detection of Slow Ground Deformation: Taking Advantage of Sentinel-1 Time Series Length in Reducing Error Sources

Using interferometric synthetic aperture radar (InSAR) to observe slow ground deformation can be challenging due to many sources of error, with tropospheric phase delay and unwrapping errors being the most significant. While analytical methods, weather models, and data exist to mitigate tropospheric error, most of these techniques are unsuitable for all InSAR applications (e.g., complex tropospheric mixing in the tropics) or are deficient in spatial or temporal resolution. Likewise, there are methods for removing the unwrapping error, but they cannot resolve the true phase when there is a high prevalence (>40%) of unwrapping error in a set of interferograms. Applying tropospheric delay removal techniques is unnecessary for C-band Sentinel-1 InSAR time series studies, and the effect of unwrapping error can be minimized if the full dataset is utilized. We demonstrate that using interferograms with long temporal baselines (800 days to 1600 days) but very short perpendicular baselines (<5 m) (LTSPB) can lower the velocity detection threshold to 2 mm y−1 to 3 mm y−1 for long-term coherent permanent scatterers. The LTSPB interferograms can measure slow deformation rates because the expected differential phases are larger than those of small baselines and potentially exceed the typical noise amplitude while also reducing the sensitivity of the time series estimation to the noise sources. The method takes advantage of the Sentinel-1 mission length (2016 to present), which, for most regions, can yield up to 300 interferograms that meet the LTSPB baseline criteria. We demonstrate that low velocity detection can be achieved by comparing the expected LTSPB differential phase measurements to synthetic tests and tropospheric delay from the Global Navigation Satellite System. We then characterize the slow (~3 mm/y) ground deformation of the Socorro Magma Body, New Mexico, and the Tampa Bay Area using LTSPB InSAR analysis. The method we describe has implications for simplifying the InSAR time series processing chain and enhancing the velocity detection threshold. Full article

Providing protected access to many everyday-used resources is becoming increasingly necessary. Research on applying eye movement for this purpose has been conducted for many years. However, due to technological advancements and the lack of stable solutions, subsequent explorations remain valid. The presented work is one of such studies. Two methods of biometric identification based on eye movements that utilize neural networks have been developed. In the first case, a feature vector was constructed from a 100-element time series depicting eye movement dynamics, which included velocity, acceleration, jerk, their point-to-point percentage changes, and frequency-domain representations. The same eye movement dynamic features were used in the second method, but this time, statistical values were calculated based on the previously defined time series. Long Short-Term Memory (LSTM) and dense networks were used in the user identification task in the first and second approaches, respectively. In the exploration, the publicly available GazeBase dataset was used, from which data collected for the ‘jumping point’ stimulus were chosen. The obtained results are very promising, with an accuracy of 96% for the LSTM model and the time series feature vector set and 76% for the second method. They were achieved over a three-year time span of eye movement recordings; however, different time periods were investigated, as well as various numbers of stimulus positions. Full article

Midterm Outcomes of Medial Patellofemoral Ligament Reconstruction in Adolescent Athletes: Comparison Between Acute and Recurrent Patella Dislocation

Background/Objectives: Patellar instability in adolescents is a significant cause of short- and long-term morbidity and disability. Traditionally, patients with first-time patellar dislocation are managed nonoperatively, although most studies are not focusing on the adolescent athletic population. The primary objective of the current study was to compare patient-reported outcomes and complications in adolescent athletes who underwent surgery either after the first patellar dislocation or after the recurrence of the dislocation with a minimum postoperative follow-up of 48 months (48–75 months). Methods: A total of 39 adolescent athletes who underwent medial patellofemoral ligament (MPFL) reconstruction (Group A, after the first dislocation, and Group B, recurrent patella dislocation) were included in this study. In all the patients, the same MPFL reconstruction technique was applied using a semitendinosus autograft. The graft was fixed on the patella using a transverse tunnel and adjustable loop button fixation and, in the femur, using a tunnel and absorbable screw fixation. The tunnel was drilled obliquely to prevent penetration of the distal femoral physis. The preoperative and postoperative clinical and functional evaluations of the patients were conducted via the visual analog scale (VAS), the Lysholm Knee Scoring System, the Kujala Anterior Knee Pain Scale, and the Pediatric International Knee Documentation Committee (Pedi-IKDC), and the return to sports score was assessed via the Tegner Activity Scale (TAS). Results: At the latest follow-up, both groups demonstrated significant improvement in the Lysholm scores, with Group A achieving a mean of 92.57 ± 6.21 and Group B achieving a mean of 90.53 ± 8.21 (p = 0.062). Postoperatively, Group A achieved a mean Kujala score of 94.21 ± 9.23, whereas Group B reached 92.76 ± 12.39, with no statistically significant difference (p = 0.08). The Pedi-IKDC score improved postoperatively in both groups. In Group A, it increased from 67.98 ± 12.29 to 93.65 ± 4.1, and in Group B, from 56.21 ± 13.6 to 91.67 ± 6.21 (p = 0.067). The preoperative visual analog scale (VAS) score for pain was significantly lower in Group A (3.1 ± 1.13) than in Group B (4.2 ± 3.01, p < 0.01). At the latest follow-up, the VAS scores improved in both groups, with Group A reporting a mean score of 0.47 ± 1.01 and Group B 0.97 ± 1.32 (p = 0.083). The Tegner activity scores were similar between the groups preoperatively, with Group A at 7.72 ± 1.65 and Group B at 7.45 ± 2.09 (p = 0.076). Postoperatively, Group A had a mean score of 7.28 ± 2.15, whereas Group B had a mean score of 6.79 ± 3.70 (p = 0.065). The mean time to return to sports was significantly shorter in Group A (5.1 ± 1.3 months) than in Group B (7.6 ± 2.1 months) (p < 0.01). Overall, 84.61% of the patients returned to their previous activity level. Specifically, 95.2% (20/21) of patients in Group A achieved this outcome, whereas 72.22% (13/18) achieved it in Group B. Patient satisfaction was generally high, with 76% (16/21) of patients in Group A reporting being satisfied or very satisfied, compared with 77% (14/18) in Group B. Conclusions: MPFL reconstruction is a safe and effective procedure for both acute and recurrent patellar dislocation in adolescent athletes. While patients who underwent acute reconstruction returned to sport more quickly and showed higher absolute postoperative scores, the greatest overall improvement from preoperative to final follow-up was observed in those treated for recurrent instability. Both surgical approaches demonstrated high satisfaction rates and minimal complications, supporting MPFL reconstruction as a reliable option in both acute and recurrent cases. Full article

Background/Objectives: The main objective of the present study is to assess the short-term effects of Forest Bathing (FB) conducted in a Mediterranean forest on individuals with fibromyalgia (FM) and/or chronic fatigue syndrome/myalgia encephalomyelitis (CFS/ME) on perceived pain, fatigue, state anxiety, positive and negative affect, mood states, and state mindfulness. Methods: A total of 44 participants with FM and/or CSF/ME agreed to participate in this study. The FB session consisted of a 3 km silent walk, lasting three hours and guided by a specialized psychologist and a mountain guide to guarantee the safety of the activity. Paired-sample t-tests were used to analyze the pre–post changes in perceived pain, fatigue, state anxiety, positive and negative affect, mood states, and mindfulness. Results: All reported variables but self-reported pain showed statistically significant pre–post variations after the FB session. Particularly, large-to-very-large improvements in positive and negative affect, state anxiety, tension, depression, anger, and vigor were found. Small-to-moderate effect sizes for fatigue, friendliness, and state mindfulness were also reported. Conclusions: This study provides preliminary evidence of the short-term benefits of FB in individuals with FM and/or CFS/ME, especially on state anxiety and negative affect. Full article

Background: Primary focal hyperhidrosis, characterized by excessive sweating primarily affecting the hands and axillae, can significantly impact quality of life. Bilateral thoracic sympathectomy is the gold standard treatment, providing permanent resolution of palmar hyperhidrosis. Most studies evaluating outcomes and patient satisfaction after sympathectomy focus on short- to medium-term follow-up, typically up to 5 years. This study aimed to assess anxiety, satisfaction, and sweat redistribution 20 years after bilateral endoscopic thoracic sympathectomy. Methods: Between January 2002 and December 2003, 106 patients with primary hyperhidrosis underwent bilateral endoscopic thoracic sympathectomy targeting ganglia T2–T3 at our center. The patients were contacted via telephone in 2023 and asked to complete the same survey they had filled out preoperatively and 12 months postoperatively. The survey evaluated sweat redistribution, anxiety, hyperhidrosis-related symptoms, and satisfaction. Anxiety, satisfaction, and perceived sweating were rated using a 5-point visual analog scale (VAS) ranging from 0 (unsatisfied/no symptoms) to 4 (very satisfied/severe symptoms). Results: Of the 106 eligible patients, 24 (22.6%) completed the survey. Most reported persistent anhidrosis (palmar or palmar–axillary) 20 years post-surgery. The survey results remained consistent with those at the 1-year follow-up. Significant sweat redistribution to the abdomen and back was observed. Patient satisfaction remained high, with no significant differences between the 1-year and 20-year assessments. Anxiety significantly decreased compared to preoperative levels (p < 0.001). Conclusions: Bilateral endoscopic thoracic sympathectomy is an effective long-term treatment for reducing excessive sweating. Patient satisfaction remains high over time, despite the persistence of compensatory sweating. Full article

Long-Term Durability of CFRP Strips Used in Infrastructure Rehabilitation

Prefabricated unidirectional carbon fiber reinforced polymer (CFRP) composite strips are extensively used as a means of infrastructure rehabilitation through adhesive bonding to the external surface of structural concrete elements. Most data to date are from laboratory tests ranging from a few months to 1–2 years providing an insufficient dataset for prediction of long-term durability. This investigation focuses on the assessment of the response of three different prefabricated CFRP systems exposed to water, seawater, and alkaline solutions for 5 years of immersion in deionized water conducted at three temperatures of 23, 37.8 and 60 °C, all well below the glass transition temperature levels. Overall response is characterized through tensile and short beam shear (SBS) testing at periodic intervals. It is noted that while the three systems are similar, with the dominant mechanisms of deterioration being related to matrix plasticization followed by fiber–matrix debonding with levels of matrix and interface deterioration being accelerated at elevated temperatures, their baseline characteristics and distributions are different emphasizing the need for greater standardization. While tensile modulus does not degrade appreciably over the 5-year period of exposure with final levels of deterioration being between 7.3 and 11.9%, both tensile strength and SBS strength degrade substantially with increasing levels based on temperature and time of immersion. Levels of tensile strength retention can be as low as 61.8–66.6% when immersed in deionized water at 60 °C, those for SBS strength can be 38.4–48.7% at the same immersion condition for the three FRP systems. Differences due to solution type are wider in the short-term and start approaching asymptotic levels within FRP systems at longer periods of exposure. The very high levels of deterioration in SBS strength indicate the breakdown of the materials at the fiber–matrix bond and interfacial levels. It is shown that the level of deterioration exceeds that presumed through design thresholds set by specific codes/standards and that new safety factors are warranted in addition to expanding the set of characteristics studied to include SBS or similar interface-level tests. Alkali solutions are also shown to have the highest deteriorative effects with deionized water having the least. Simple equations are developed to enable extrapolation of test data to predict long term durability and to develop design thresholds based on expectations of service life with an environmental factor of between 0.56 and 0.69 for a 50-year expected service life. With the availability of more ground-based remote-sensing meteorological equipment at Hong Kong International Airport, many more interesting features of terrain-disrupted airflow have been observed, such as the applications of short-range Doppler LIDAR. This paper documents a number of new features observed at the airport area, such as the hydraulic jump-like feature, vortex, and extensive mountain wake/reverse flow. The technical feasibility of using a numerical resolution weather prediction model to simulate such features is also explored. It is found that the presently available input data and numerical model may not be able to capture the fine features of the atmospheric boundary layer, and thus they are not very successful in reproducing many small-scale terrain-disrupted airflow features downstream of an isolated hill. On the other hand, more larger-scale terrain-disrupted flow features may be better captured, but there are still limitations with the available turbulence parameterization schemes. This paper aims at documenting the newly observed flow features at the Hong Kong International Airport, enhancing the understanding of low-level windshear, and evaluating the outputs of numerical resolution simulations for reproducing such observed features and its technical feasibility on short-term forecasting.
