Background, Clinical Significance: Anterior cruciate ligament (ACL) injuries are common, however the occurrence of concurrent gastrocnemius muscle tears is exceptionally rare. Given this, the diagnosis and management of this pattern of injury is poorly characterised, with lack of current clinically relevant classification systems and evidence-based guidelines to guide treatment. Early recognition is essential. with advanced imaging critical to guiding the diagnosis and management of patients with this pattern of injury. Case presentation: A 39 year old man presented with acute right knee swelling, pain and difficulty weightbearing following a sports-related fall. Clinical examination was suspicious for an ACL injury. Magnetic Resonance Imaging (MRI) of the knee demonstrated the disrupted and displaced ACL fibres, with extensive peri-cruciate oedema around the expected position of the ACL. It was associated with partial avulsion of the medial gastrocnemius origin and incomplete avulsion of the lateral gastrocnemius origin. The patient was referred for an urgent orthopaedics review and is currently on trial of conservative management. Conclusions: In this case report and review of the literature, we evaluate the current understanding of the complexities of combined musculoskeletal injuries and limitations of existing classifications in providing accurate diagnosis and management strategies. Given the rarity of this presentation, the case underscores the lack of evidence-based recommendations for early management, particularly in young, active individuals who are at risk of significant long-term functional impact. Participation in running events has expanded worldwide, consolidating itself as a form of active leisure and a driver of social and tourism engagement. Although runners’ motivations have been extensively studied, perceived inclusivity, understood as motivation derived from the event’s promotion of equitable participation across gender, age and functional ability, has rarely been examined as a distinct motivational dimension within structural models. This study analyses the motivational structure of participants in the Elche Half Marathon (Spain) and assesses the incremental contribution of inclusivity to traditional motivational frameworks. Based on a sample of 1053 valid responses, a two-stage psychometric and segmentation approach was applied. Exploratory and confirmatory factor analyses (EFA and CFA) were conducted to compare a four-factor model (sport-related hedonism, competition, socialization and digital socialization) with an extended five-factor model incorporating inclusivity. Subsequently, cluster analyses were performed using factor scores derived from each model. The results show that the inclusion of inclusivity improves model fit and increases explained variance, while also generating a more differentiated segmentation structure. The extended model revealed six motivational profiles, some of which displayed continuity with the classical solution, while others were reconfigured when inclusivity was introduced. Overall, the findings indicate that inclusivity functions as a complementary and context-dependent motivational dimension that refines the understanding of participation heterogeneity in running events. Rather than replacing traditional motives, inclusivity contributes incremental explanatory value and enhances the identification of motivational profiles, offering relevant insights for the design and management of mass-participation sporting events. Compression garments are widely employed in medical and sports contexts for their ability to promote venous return, manage oedema, support musculoskeletal function, and enhance athletic recovery. Advances in textile-based compression systems have been driven by innovations in fibres, yarn structures, fabric structure engineering, and design methods. This review critically examines the current literature on compression garments, highlighting the influence of raw materials and yarn architectures on performance, durability, and wearer comfort. Attention is given specially to fabric structures and manufacturing methods, where the evolution from traditional cut-and-sew methods to advanced seamless, flatbed, and circular knitting technologies is highlighted, along with their impact on pressure distribution and overall garment efficacy. The integration of 3D body scanning, finite element analysis, and predictive modelling, which enables more personalised and precise garment design, is also speculated upon. Moreover, the review highlights testing and evaluation methodologies, spanning both in vivo and in vitro based assessments, pressure sensor studies for real-time monitoring, and theoretical models mostly based on Laplace’s law. This literature survey provides a foundation for future innovations aimed at optimising compression garment design for both therapeutic and athletic use. Full article

Objectives/Background: In this study, we aimed to examine the associations of participation in leisure dance activity with perceived health status, happiness level, and perceptions of leisure, and the relevance of demographic characteristics to these associations. Moreover, we sought to explore ways of revitalizing leisure dance activity. Methods: We used data from the 2022 Korea National Leisure Activity Survey organized by the Korean Ministry of Culture, Sports, and Tourism. The 2022 Korea National Leisure Activity Survey was conducted from September to November 2022. The survey was conducted among 10,046 Koreans aged ≥15 who lived in 17 cities and provinces across the country. The participants were informed about the survey schedule in advance, and interviews were conducted at the participants’ homes using tablet PCs. Random telephone verification was performed on the recovered questionnaires to ensure the accuracy of responses. Questionnaires that passed the first verification were subjected to secondary verification by a computerized program, and questionnaires that did not pass the verification were supplemented and re-examined. The collected data were entered electronically through an encoding process, and only the data that passed the final test were compiled in the multi-stage verification process. The data were analyzed using frequency analysis, chi-squared tests, and multivariate logistic regression analysis. Results: Of the 1004 participants, 655 (6.5%) participated in leisure dance activity. Women, individuals in their twenties and thirties, college graduates or those with lower-level educational qualifications, and unmarried individuals were more likely to participate in leisure dance activities. In addition, leisure dance activities were found to be likely to increase happiness levels and leisure life satisfaction, and leisure expenses were likely to be low. Conclusions: Schools should provide high-quality dance education and enhance the professionalism of physical education teachers in the management of dance classes. This is because dance-related experiences in childhood can increase the likelihood of engaging in dancing in adulthood. Efforts are also needed to increase men’s awareness of and participation in leisure dance activities and to lower barriers to entry. The convergence of dance, games, and technology can make this possible. Full article

This study proposes an integrated agility assessment system that combines Millimeter-Wave (MMW) radar, Ultra-Wideband (UWB) ranging, and Mixed Reality (MR) technologies to quantitatively evaluate athlete performance with high accuracy. The system utilizes the fine motion-tracking capability of MMW radar and the immersive real-time visualization provided by MR to ensure reliable operation under low-light conditions and multi-object occlusion, thereby enabling precise measurement of mobility, reaction time, and movement distance. To address the challenge of player identification during doubles testing, a one-to-one UWB configuration was adopted, in which each base station was paired with a wearable tag to distinguish individual athletes. UWB identification was not required during single-player tests. The experimental protocol included three specialized agility assessments—Table Tennis Agility Test I (TTAT I), Table Tennis Doubles Agility Test II (TTAT II), and the Agility T-Test (ATT)—conducted with more than 80 table tennis players of different technical levels (80% male and 20% female). Each athlete completed two sets of two trials to ensure measurement consistency and data stability. Experimental results demonstrated that the proposed system effectively captured displacement trajectories, movement speed, and reaction time. The MMW radar achieved an average measurement error of less than 10%, and the overall classification model attained an accuracy of 91%, confirming the reliability and robustness of the integrated sensing pipeline. Beyond local storage and MR-based live visualization, the system also supports cloud-based data uploading for graphical analysis and enables MR content to be mirrored on connected computer displays. This feature allows coaches to monitor performance in real time and provide immediate feedback. By integrating the environmental adaptability of MMW radar, the real-time visualization capability of MR, UWB-assisted athlete identification, and cloud-based data management, the proposed system demonstrates strong potential for professional sports training, technical diagnostics, and tactical optimization. It delivers timely and accurate performance metrics and contributes to the advancement of data-driven sports science applications. Full article

Background/Objectives: Women’s wrestling is one of the fastest growing sports within the United States at the high school and collegiate level. Weight-class sports, such as wrestling, present unique challenges for female athletes, particularly in managing acute weight fluctuations associated with the menstrual cycle. However, less is known regarding how sex-specific physiology may influence weight management strategies among female wrestlers. The purpose of this review was to highlight current weight loss trends among high school and collegiate female wrestlers and summarize the physiological mechanisms driving potential menstrual-related fluid retention and subsequent changes in body mass or composition. Methods: A literature review was conducted to include studies evaluating weight loss trends in female wrestlers competing in the United States. Additional literature focused on fluid, body mass, and body composition changes throughout the menstrual cycle was also included. Results: Recent evidence has provided observational data that can be used to develop descriptive summaries of current body composition profiles and weight loss trends among female wrestlers. These data can help to quantify the typical magnitude of weight loss changes observed in female wrestlers competing at the high school and collegiate level within the United States. Conclusions: These findings help provide insight into the magnitude of weight loss wrestlers undergo in an effort to compete in their minimal weight class, thus informing practitioners of potential health risks and helping guide optimal weight management efforts. This study presents a comprehensive approach to the integration of trend practices in waste management processes within sports and recreation facilities, focusing on four countries: Turkey, Lithuania, Morocco, and Sri Lanka. The aim of the study is to identify the social, technological, managerial, and behavioral factors shaping waste management practices and trends in these areas and to provide a comparative cross-country analysis. The research was conducted using a qualitative research design. Data were collected from a total of 96 experts across the four countries through a semi-structured interview form. The data obtained were analyzed using thematic analysis, and findings were classified under the themes of infrastructure, policy, technology, governance, and awareness. Based on the analysis, the findings reveal that Lithuania stands out with its strong digital infrastructure grounded in the EU legal framework; Turkey, despite its high potential, experiences a behavior–intention gap; Morocco exhibits institutional resistance and regime lock-in; while Sri Lanka demonstrates a community-based yet institutionally weak structure. Overall, the research highlights that sustainable waste management depends not only on technical infrastructure but also on the holistic interaction of socio-technical factors such as cultural norms, institutional coordination, and behavioral alignment. The gut microbiota represents a complex microbial ecosystem with the potential to influence athletic performance, energy metabolism, inflammatory responses, and recovery capacity in athletes. However, the specific relationship between microbiota and performance in combat sport athletes remains poorly characterized. This scoping review systematically maps current evidence on gut microbiota-combat sports performance relationships, identifies microbial response patterns to training and competition, evaluates nutritional prebiotic and probiotic interventions, and highlights methodological gaps to guide future research. Following the PRISMA-ScR framework, 8 studies were identified, which included 247 elite and high-level athletes, comprising 169 males and 78 females, with sample sizes ranging from 12 to 53 across wrestling, mixed martial arts, martial arts, judo, and taekwondo. Associations were observed between gut microbiota characteristics and training intensity, competition level, weight management, and pre-competition psychological states. Limited taxonomic consistency was observed across studies, with most bacterial genera appearing in a single investigation, precluding the identification of robust sport-specific microbial signatures. Preliminary trials demonstrated improvements in gastrointestinal symptoms, aerobic performance, and psychological fatigue with prebiotic and probiotic interventions. However, small sample sizes and methodological heterogeneity across studies limit generalizability and preclude definitive conclusions regarding the role of gut microbiome in combat sports performance. Obstructive sleep apnea (OSA) is an underdiagnosed condition in athletes, strongly influenced by anthropometric factors such as body mass index (BMI) and neck circumference, which often reflect long-term sport-specific training and nutritional strategies. This review examines the impact of OSA on athletes’ recovery, injury risk, and performance, with emphasis on BMI and neck circumference as key risk markers. A comprehensive search was conducted in PubMed, Web of Science, EBSCO, and Google Scholar for studies published between 2015 and 2025. Eleven studies met the inclusion criteria. The findings indicate a substantial prevalence of OSA in athletes, particularly those involved in collision sports such as rugby and American football, where higher BMI and increased neck circumference were consistently associated with increased risk of OSA. OSA was associated with reduced sleep quality, hormonal disruption, excessive daytime sleepiness, and slower reaction times. These factors may collectively impair recovery, increase injury susceptibility, and negatively affect aerobic capacity, cognitive function, and sport-specific performance. The results highlight the need for routine sleep screening in athletes, especially those with elevated BMI or larger neck circumference. Early detection and management of OSA may improve recovery and performance. Further longitudinal studies are needed to evaluate the long-term effects of OSA treatment in athletic populations. Rapid weight loss (RWL) is a common strategy among combat sports athletes aiming for a competitive advantage. However, it imposes significant immunological stress that compromises both innate and adaptive immune defenses. This systematic review synthesizes current experimental and mechanistic evidence on the effects of RWL in combat sports, focusing on cellular immunity, neuroendocrine regulation, and inflammatory pathways. Acute RWL activates the hypothalamic–pituitary–adrenal axis, elevating plasma cortisol and suppressing lymphocyte proliferation, T-cell function, and natural killer cell cytotoxicity. Although neutrophil counts increase, their phagocytic and oxidative burst capacities decline, reflecting impaired host defense. Monocyte and macrophage systems shift toward proinflammatory phenotypes, while mucosal immunity is weakened by reductions in secretory immunoglobulin A, leading to increased upper respiratory tract infection risk. The magnitude and speed of weight loss are critical determinants of immune dysfunction, with reductions exceeding 5% of body mass producing particularly severe consequences. Evidence-based intervention strategies—including gradual weight management, nutritional optimization, and biomarker monitoring—are essential to mitigate immunosuppression and safeguard athlete health. This review highlights key gaps in combat sports-specific protocols and proposes integrated approaches to preserve immune competence and optimize performance. Full article

Road running tourism events continue to grow worldwide and are increasingly leveraged by destinations seeking diversification and seasonality reduction. This study examines the motivational structure of participants in the 2025 Half Marathon Magaluf (Mallorca, Spain)—a mature Mediterranean resort undergoing tourism repositioning—and analyses how motivation-based segments relate to socio-demographic, sporting and tourism behaviours. Data were collected through a self-administered online survey (N = 306). An Exploratory Factor Analysis (EFA), followed by a Confirmatory Factor Analysis (CFA), validated a five-factor motivational structure: sport-related hedonism, socialisation, personal challenge, inclusiveness and digital interaction. A k-means cluster analysis identified five distinct segments—Digital Enthusiasts, Inclusive Enjoyers, Socializers, Hedonic Achievers and Inclusivists—each exhibiting differentiated patterns in Experience-Use History (EUH), origin, gender, and running-club membership. Notably, Socializers recorded the longest stays, Inclusive Enjoyers were overrepresented among first-time visitors, and Digital Enthusiasts and Hedonic Achievers included a higher share of international runners. These findings expand traditional motivational models by incorporating inclusiveness and digital interaction as emerging drivers and offer actionable recommendations for event organisers and destination managers seeking to enhance overnight stays and support destination repositioning strategies. With the growing popularity of equestrian sports, the incidence of athletic injuries in horses has also risen. Among these injuries, proximal sesamoid bone fracture (PSBF) and flexor tendinitis are particularly common in the forelimbs of sport horses and represent major causes of musculoskeletal impairment. A 5-year-old horse presented with obvious symptoms such as swelling at the left fetlock joint and metacarpal region after exercise. Through lameness assessment, diagnostic imaging, and hematological testing, the horse was diagnosed with PSBF complicated by flexor tendinitis. The affected horse was treated with non-steroidal anti-inflammatory drugs combined with low-intensity pulsed ultrasound (LIPUS) therapy. After treatment, local microcirculation at the fracture and flexor tendon sites was improved, tissue healing was accelerated, and clinical indicators were stabilized. This case report demonstrates the potential of LIPUS-assisted therapy in promoting the recovery of horses with PSBF and concurrent flexor tendinitis, providing a valuable clinical reference for the management of complex musculoskeletal injuries in veterinary practice. Non-Operative vs. Operative Treatment of Pediatric Proximal Humerus Fractures: Surgery Offers No Clinical or Economic Benefit, a Retrospective Study of 152 Children

Background: Pediatric proximal humerus fractures (PHFs) typically heal well due to their strong remodeling potential, supporting non-operative management even in displaced injuries. However, surgery for Neer–Horowitz grade III–IV fractures has become more frequent despite limited evidence of superior outcomes. Methods: A retrospective analysis of 152 children (<14 years) treated for isolated PHFs at a tertiary pediatric orthopedic center (2004–2023) was performed. Clinical records and telephone follow-up provided demographic data, fracture classification, management, complications, and functional outcomes (QuickDASH, Tegner, return to sport). A direct cost analysis compared conservative and surgical pathways. Results: Of 152 patients, 133 were treated non-operatively and 19 surgically. Conservative management achieved excellent results across all fracture types: nearly all patients reported normal QuickDASH scores and full shoulder function. Among Neer III–IV fractures (n = 37), functional outcomes, activity levels, and sport resumption were similar between treatment strategies. Minor transient issues (pin migration, temporary stiffness, delayed return to sport) occurred only after surgery. No meaningful complications were observed in the conservative cohort. Mean costs differed substantially: €1452.09 for non-operative management versus €7832.12 for surgical treatment. Conclusions: Long-term outcomes of pediatric PHFs were uniformly excellent, regardless of fracture severity or treatment modality. Surgery did not improve recovery, function, or return to sport and was associated with higher costs and minor postoperative issues. Conservative management should remain the standard of care for nearly all pediatric PHFs, with surgery reserved for exceptional circumstances such as open fractures, neurovascular compromise, or failed closed reduction. Precision medicine, which relies on genomic, multi-omic, phenotypic, and environmental data, has the potential to transform healthcare from population-focused heuristics to individualized prevention, diagnosis, and treatment. Moreover, recent advances in sequencing, molecular profiles, wearable sensors, and machine learning have created opportunities for rapid translational innovation: rapid genomic diagnosis in neonatal and paediatric rare diseases, targeted oncology, pharmacogenomic-based prescribing strategies, and individual sport performance. Nevertheless, the vast majority of innovations remain in centers of specialism or pilot programs, rather than routinely or equitably integrated into clinical or athletic practice. This narrative review synthesizes translational evidence across the life course—in pregnancy, paediatrics, adult medicine, geriatrics, and sportomics—to find reproducible clinical and performance examples which enable precision-based alternative approaches to management, outcome, or preparation; and to reshape those examples into pragmatic, scalable priorities which minimize inequity, and maximize benefit. We undertook a structured narrative synthesis of peer-reviewed literature, trials, clinician translation programs, implementation studies, and sportomics reports, prioritizing examples that demonstrate utility, reproducibility, and impact. Important findings suggest that multi-omics and rapid sequencing improve diagnostic yield and time to diagnosis. Molecular profiling and circulating tumor DNA help realize adaptive treatment selection. Integrated genomics, metabolomics, wearable physiology, and AI analytics facilitate individualized training, injury-risk stratification, and recovery optimization. But systematic value is limited by insufficient representative validation, dataset bias, poor interoperability, regulatory uncertainty, workforce preparedness, and inequities of access. Converting a promise into population- and performance-level value requires coordinated action across four fronts: representative validation; interoperable, privacy-preserving infrastructures; clinician- and coach-centered implementation; and templates for scalable, cost-sensitive deployment. Urbanicity is a recognized determinant of mental health, yet conventional measures such as population density or the rural–urban divide often fail to capture the complex realities of informal settlements in low- and middle-income countries. This paper conceptualizes neighborhood effects through the lived experiences of young women in Kampala, Uganda, drawing on participatory research from the NIH-funded TOPOWA study. Using community mapping and Photovoice, participants identified neighborhood features that shape wellbeing, including sanitation facilities, drainage systems, alcohol outlets, health centers, schools, boda boda stages (motorcycle taxis), lodges, religious institutions, water sources, markets, and recreational spaces. These methods revealed both stressors—poor waste management, flooding, violence, gendered harassment, crime, and alcohol-related harms—and protective resources, including education, places of worship, health centers, social networks, identity, and sports activities. We argue that urbanicity in slum contexts should be understood as a multidimensional construct encompassing deprivation, fragmentation, exclusion, and resilience. This reconceptualization advances conceptual clarity, strengthens the validity of mental health research in low-resource settings, and informs interventions that simultaneously address structural risks and promote community assets. The case of Kampala demonstrates how participatory evidence can reshape the understanding of neighborhood effects with implications, for global mental health research and practice. Full article
