Background/Objectives: Adherence to the EAT-Lancet Planetary Health Diet (PHD) may promote human health and environmental sustainability, yet evidence regarding adherence and nutritional adequacy in Australia is limited. Globally, no research to date has used the recently updated 2025 PHD guidelines. We benchmarked the compatibility of Australian adults’ dietary patterns with the 2025 PHD and examined its associations with nutritional adequacy and sociodemographic factors. Methods: This was a cross-sectional analysis of dietary data from 5655 adults who participated in the National Nutrition and Physical Activity Survey. Usual intakes were estimated from two 24 h recalls using the Multiple Source Method. PHD adherence was measured using the Healthy Reference Diet Score (0–130 points). Nutrient adequacy was assessed using the full probability method for iron and the Australian/New Zealand Estimated Average Requirement Cut-Point Method for all other nutrients. Survey-weighted regression models examined associations with nutritional adequacy and sociodemographic factors. Results: The mean PHD adherence score was 50 (SE 0.3) points. Higher adherence was associated with lower odds of inadequate intakes of several micronutrients, but with higher odds of inadequacy for vitamin B12 (OR: 1.24; 95% CI: 1.06, 1.45) and calcium (OR: 1.09; 95% CI: 1.01, 1.17). PHD adherence was higher among females, older adults, those with higher educational attainment, those born in countries where English is not the main language, two-person households and non-smokers; adherence was non-linearly associated with alcohol and was lower among those with a Body Mass Index ≥ 30 kg/m2. Conclusions: PHD adherence in Australia was low. Higher adherence was associated with improved adequacy for several micronutrients. Trade-offs for vitamin B12 and calcium warrant consideration. Equity-conscious strategies will be needed to support the adoption of nutritionally adequate, environmentally sustainable diets. Full article

Background: Sarcopenic obesity (SO) and diabetic sarcopenia (DS) represent overlapping metabolic–musculoskeletal disorders characterized by the coexistence of excessive adiposity, insulin resistance, and progressive muscle wasting. The Planetary Health Diet (PHD), proposed by the EAT–Lancet Commission, emphasizes plant-forward, nutrient-dense, and environmentally sustainable food patterns that may concurrently address metabolic and muscle health. This review aimed to systematically evaluate dietary and bioactive nutritional interventions aligned with the PHD and their effects on muscle mass, strength, metabolism, and underlying mechanisms in SO and DS. Methods: Following PRISMA guidelines, studies published between 2015 and 2025 were identified across PubMed, Scopus, and Google Scholar. Eligible studies included dietary, nutritional, or supplement-based interventions reporting muscle-related outcomes in obesity- or diabetes-associated conditions. Results: Ninety-one eligible studies were categorized into plant-derived, animal/marine-based, microorganism/fermented, synthetic/pharmaceutical, and environmental interventions. Across diverse models, bioactive compounds such as D-pinitol, umbelliferone, resveratrol, GABA, ginseng, whey peptides, probiotics, and omega-3 fatty acids consistently improved muscle mass, strength, and mitochondrial function via AMPK–SIRT1–PGC-1α and Akt–mTOR signaling. These mechanisms promoted mitochondrial biogenesis, suppressed proteolysis (MuRF1, Atrogin-1), and enhanced insulin sensitivity, antioxidant capacity, and gut–muscle communication. Conclusions: PHD-aligned foods combining plant proteins, polyphenols, and fermented products strengthen nutrient sensing, mitochondrial efficiency, and cellular resilience, representing a sustainable nutritional framework for preventing and managing SO and DS. Taking into account both the health and environmental aspects of food, the EAT-Lancet Commission proposed a healthy reference diet (Planetary Health Diet—PHD). The aim of this narrative review is to summarize the results obtained in epidemiological studies on the association between the PHD and risk factors, non-communicable diseases, and cause-specific mortality. The literature search was conducted in February 2025 and was based on the PubMed electronic database. The results of this review are divided into four parts, which include the results of cohort studies, cross-sectional studies, case–control studies, and meta-analyses. This review, showing what types of studies have been conducted so far, allows for a summary of the current knowledge of the relationship between the PHD and risk factors, non-communicable diseases, and cause-specific mortality. Cohort studies provided most of the results, which confirmed that higher adherence to the PHD has a beneficial effect on human health, especially taking into account the lower risk of diabetes, cardiovascular disease (CVD) and CVD mortality, cancer and cancer mortality, as well as all-cause mortality. However, it is concluded that the association between the PHD and stroke, different types of stroke, hypertension, dyslipidemia, and some specific types of cancer must be confirmed. Specifically, randomized controlled trials should be conducted, as, to our knowledge, there is a lack of these types of studies to date. Such studies should be conducted in different regions using the Planetary Health Diet adapted to the local, cultural, geographical, and demographical aspects of a particular region. Full article

Background: The escalating prevalence of food allergies, alongside the global call for environmentally sustainable dietary transitions, has drawn attention to plant-based dietary models—particularly those inspired by the EAT-Lancet Commission. These frameworks not only reduce reliance on animal-sourced foods, benefiting planetary health, but may also play a role in modulating immune tolerance and allergic responses. Methods: This scoping review followed PRISMA guidelines and included 53 peer-reviewed studies published between 2000 and 2024, retrieved from PubMed, Scopus, and Google Scholar. Eligible articles were classified into two thematic domains: prevention of food allergy onset (n = 31) and modulation of allergic symptoms in sensitized individuals (n = 22). Included studies comprised randomized controlled trials (n = 6), observational studies (n = 17), systematic reviews and meta-analyses (n = 11), and narrative/scoping reviews (n = 19). Results: Sustainable plant-based diets were consistently associated with a lower incidence of allergic sensitization and reduced symptom severity. These effects were partly due to the exclusion of common allergens (e.g., dairy, eggs, and shellfish) but more importantly due to immunomodulatory mechanisms. Fermentable fibers can enhance short-chain fatty acid (SCFA)-producing bacteria (e.g., Faecalibacterium prausnitzii), elevating butyrate and acetate levels, which interact with G-protein-coupled receptors 43 and 109A (GPR43 and GPR109A) to induce regulatory T cells (Tregs) and reinforce epithelial integrity via tight junction proteins such as occludin and claudin-1. Polyphenols (e.g., quercetin and luteolin) can inhibit Th2-driven inflammation by stabilizing mast cells and downregulating IL-4 and IL-1. Conclusions: Following sustainable dietary guidelines such as those proposed by the EAT-Lancet Commission may confer dual benefits: promoting environmental health and reducing the burden of allergic diseases. By emphasizing plant-based patterns rich in fiber and polyphenols, these diets support microbiota-mediated immune education, mucosal barrier function, and immunological tolerance. When properly supervised, they represent a promising tool for allergy prevention and symptom management. Larger randomized trials and long-term population studies are needed to confirm and operationalize these findings in clinical and public health contexts. Full article

Background/Objectives: The EAT-Lancet Commission proposed a healthy and sustainable ‘planetary health diet’ (PHD) based on the nutritional needs of adults, but recommended for all population groups over the age of two years. This study investigated the extent to which UK adolescent diets meet these recommendations, and the association between meeting recommendations and nutritional intake. Methods: Dietary intake data collected from students aged 11–15 years between 2019 and 2022 as part of the Food provision, cUlture and Environment in secondary schooLs (FUEL) study was used. Specifically, 24 h intakes of food groups and key nutrients were summarised for each participant, and micronutrient intake was expressed as a percentage of the reference nutrient intake (RNI). The proportion of participants meeting each PHD recommendation was calculated. A PHD index score was developed to measure the overall adherence to recommendations. Multilevel linear regression models were fitted to explore the association between the PHD score and intake of each nutrient. Results: No participants fully met the recommendations. The mean PHD score was 14.2 (3.3) out of 39. Adherence was low for the emphasised foods, and lowest for nuts (1.3%) and unsaturated oils (0.2%). Higher PHD index scores were associated with lower intakes of all micronutrients, with the largest association for vitamin B12 [regression coefficient: −12.9% (95% CI: −16.3, −9.4) of RNI] and the smallest for vitamin D [regression coefficient: −0.4 (95% CI: −0.7, −0.2) of RNI]. Conclusions: Substantial dietary changes are needed for this population group to meet the PHD guidelines. Further research should focus on why these recommendations may lead to the inadequate intake of some nutrients in this age group. We propose that the PHD guidance needs to be reviewed and potentially adapted for this specific population, with consideration of the typical dietary behaviours of this age group. Full article

Current food systems pose serious threats to both human health and environmental sustainability. Market forces stimulate food production to such an extent that life on Earth is endangered, and an eating pattern based on calorie-dense, nutrient-poor, and ultra-processed foods is promoted. In terms of health, this situation has caused obesity to become a pandemic, which is a major risk factor for non-communicable diseases, one of the current leading causes of death. The EAT-Lancet Commission (EAT-LC) argues that providing a growing world population with healthy diets through sustainable food systems is an immediate challenge. Thus, we propose to compare: (a) the consumption patterns described by this commission for Latin America and the Caribbean (LA&C) with those determined for Argentina; (b) Argentine dietary habits with the healthy and sustainable dietary pattern proposed by the EAT-LC; and (c) the availability of each food group in Argentina with the pattern proposed by the EAT-LC. A total of 11 food groups proposed by the Commission were taken as a reference. The dietary patterns described by the EAT-LC for LA&C are not representative of Argentina, where meat consumption is much higher (200%) and legume consumption is lower (78%). Very marked differences are observed between the consumption pattern in Argentina and the recommendations made by the EAT-LC. Regarding food availability, Argentina exceeds the requirements for meat (13.4 times), starchy vegetables (2.4 times), and eggs (2.3 times); by contrast, it does not reach the levels proposed for other products such as vegetables, nuts, legumes, or fish. Promoting a change in the eating habits of the population from a human-rights perspective represents an enormous challenge. The main aim of this work is to take a first step towards that transformation by comparing international recommendations for health and sustainability—paying special attention to culture-specific aspects—and dietary patterns in Argentina. Introduction: The planetary health diet, introduced by the EAT–Lancet Commission, aims to address global health and sustainability challenges by promoting a plant-based diet with reduced consumption of animal-sourced foods. This diet not only contributes to environmental sustainability but also offers significant health benefits, including prevention and management of abdominal obesity, carbohydrate metabolism disorders, dyslipidemia, and elevated blood pressure. These metabolic disorders are components of metabolic syndrome, a cluster of risk factors for cardiovascular disease. Objectives: This narrative review aims to gather the most recent findings on the impact of the planetary diet on individual components of metabolic syndrome and on the prevention and treatment of cardiovascular disease. Methods: The available research on the topic was identified via searches in PubMed, Scopus, and Google Scholar. Results: Abdominal obesity, a major risk factor for a range of chronic diseases, can be effectively mitigated by adhering to plant-based dietary patterns, which have been shown to reduce waist circumference and improve overall cardiometabolic health. Furthermore, the planetary diet plays a crucial role in reducing the risk of type-2 diabetes and improving glycemic control, with specific dietary components such as whole grains and fiber, demonstrating positive effects on blood glucose levels. This diet is additionally associated with favorable lipid profiles, including lower levels of LDL cholesterol and total cholesterol, which are critical in the prevention of atherosclerosis and cardiovascular diseases. Conclusions: These multiple benefits highlight that the planetary diet may be an effective strategy for managing and preventing metabolic syndrome and cardiovascular disease. However, further research is needed to confirm its long-term efficacy and applicability across diverse populations. Full article

Background/Objectives: Noncommunicable diseases, particularly cardiovascular disease (CVD), represent a significant global public health challenge, with unhealthy diets as a major risk factor. This study investigates the association between adherence to the Planetary Health Diet proposed by the EAT-Lancet Commission and CVD risk. Methods: Utilizing data from the Seguimiento Universidad de Navarra (SUN) cohort, which included 18,656 participants (mean age 38 years; 61% women), we assessed dietary intake using a validated food frequency questionnaire and the Planetary Health Diet Index to evaluate adherence (range 0–42). CVD was defined as new-onset stroke, myocardial infarction, or CVD death. Results: After a median follow-up time of 11.5 years, 220 cases of CVD were identified. Higher adherence to the Planetary Health Diet revealed no statistically significant reduction in CVD risk associated with the diet. Cox proportional hazard models indicated a trend towards lower CVD risk in the highest adherence quartile, but this did not reach significance (HR 0.77, 95% CI 0.51–1.18, p-trend = 0.127). Sensitivity analyses corroborated these results. Discrepancies in previous studies highlight the complexity of dietary assessments and underscore the need for standardized scoring systems. Conclusions: In a large Spanish cohort, adherence to the Planetary Health Diet showed no significant reduction in CVD risk. Further research is needed to reach a consensus on the operational definition of the Planetary Health Diet and to clarify the relationship between diet and CVD risk. Full article

Background/Objective: The relationship between food consumption and environmental sustainability is becoming increasingly evident. The aim of this study was to estimate the evolution of the environmental impact of food consumption in the Spanish population, assessed in terms of greenhouse gas (GHG) emissions. Methods: Data collected from the Household Budget Survey were included, from approximately 24,000 households for the period of 2006–2023. The environmental impact of diet, in terms of GHG emissions, was estimated from the EAT-Lancet Commission tables, and the adherence to the Mediterranean Diet (MedDiet) was calculated using the Dietary Score index. Results: The environmental impact of the Spanish diet, in terms of GHG, followed a downward trend over the years analysed, from 3978.1 g CO2-eq in 2006 to 3281.4 g CO2-eq in 2023, a decrease of 17.5%. The food groups with the largest decrease in consumption during this period were red meat (from 39.9 kg/year to 35.5 kg/year), fish (from 24.3 kg/year to 19.0 kg/year), and dairy products (from 113.4 kg/year to 99.7 kg/year). The level of adherence to the MedDiet increased slightly from 34 points in 2006 to 35 points in 2023 due to an increase in the amount of vegetables (42.7 kg/year vs. 44.3 kg/year) and grains consumed (53.1 kg/year vs. 72.6 kg/year) and a decrease in fish consumption (24.3 kg/year vs. 19.0 kg/year). Conclusions: In Spain, a reduction in GHG emissions associated with food consumption was observed due to a decrease in the consumption of red meat, fish, dairy products, and fats. National surveys are very useful tools to analyse the impact of food consumption on climate change and to assess the effect of the policies implemented to contain it. Full article

Background: Childhood obesity is a critical public health challenge with a rising prevalence worldwide, contributing to numerous health risks and long-term societal burdens. Concurrently, climate change and environmental degradation demand sustainable approaches to dietary patterns. The Planetary Health Diet (PHD), initially designed for adults, emphasizes plant-based foods and sustainable practices. Objectives: This review explores the suitability of the PHD for addressing childhood obesity by assessing its nutritional adequacy and proposing necessary adaptations for pediatric populations. Methods: A narrative review methodology was employed, examining data from global and regional nutritional guidelines and evaluating the PHD’s bromatological composition against age-specific requirements. Results: The findings indicate that while the PHD aligns with environmental goals, it may not fully meet the energy and specific nutrient needs of children and adolescents without modifications. Key challenges include ensuring adequate intake of bioavailable protein, iron, calcium, vitamin B12, and vitamin D. Strategies such as incorporating fortified foods, optimizing food pairings, and gradual adaptation to high-fiber diets are critical for successful implementation. This review also highlights the importance of cultural adaptability, family involvement, and regional food systems in promoting adherence. Tailored interventions, such as school meal programs and educational initiatives, can bridge gaps in nutrition while fostering sustainable dietary behaviors. Conclusions: Adapting the PHD for pediatric needs presents an opportunity to integrate nutritional health with environmental stewardship, contributing to obesity prevention and a sustainable future. Further research is required to validate these adaptations and develop comprehensive frameworks for global implementation. Full article

Higher Adherence to the EAT-Lancet Diets After a Lifestyle Intervention in a Pediatric Population with Abdominal Obesity

Background/Objectives: The rising prevalence of pediatric obesity highlights the urgent need for effective lifestyle interventions that improve diet quality, in line with global health objectives. Tackling obesity through planetarian dietary practices not only enhances individual health but also mitigates the environmental impact of food systems. The EAT-Lancet Commission’s plant-based dietary recommendations underscore the dual benefit of promoting human health while supporting environmental sustainability. This study aims to assess changes in adherence to a planetarian diet, measured through planetary environmental impact indices, following a lifestyle intervention in a pediatric population. Methods: In this randomized controlled trial, 107 participants with abdominal obesity were assigned to either a usual care group or an intensive intervention group, the latter following a moderately hypocaloric Mediterranean diet combined with nutritional education. Adherence to the EAT-Lancet diet was evaluated using both the EAT-Lancet Diet Score and the EAT-Lancet Diet Index. Results: The intensive lifestyle intervention significantly improved adherence to the EAT-Lancet diet, leading to notable reductions in body mass index (BMI), weight, and waist circumference, alongside improvements in both anthropometric and clinical outcomes. Conclusions: This study demonstrates that intensive lifestyle interventions in children and adolescents with abdominal obesity can reduce BMI-SDS (BMI-standard deviation score) and improve adherence to planetarian dietary patterns, leading to enhanced health outcomes. Further research is needed to evaluate the long-term effects of such interventions and to determine their broader applicability across diverse pediatric populations. Full article

Gestational diabetes mellitus (GDM) is a prevalent pregnancy complication that seriously endangers maternal and infant health, posing a medical and economic burden worldwide. Several dietary patterns have been recommended for women of childbearing age, demonstrating a positive role in preventing and managing GDM. However, these dietary patterns may not fully take environmental factors into account when addressing global food sustainability and planetary health. In this context, the EAT-Lancet Commission proposed a diet in 2019 aimed at both health improvement and environmental sustainability, which can potentially reduce the prevalence of diet-related diseases. Nevertheless, the role of the EAT-Lancet reference diet in preventing and managing GDM has not been fully evaluated. Therefore, we conducted a literature search to assess the existing evidence for the association between the EAT-Lancet reference diet components and GDM. Based on the current evidence available in the PubMed database from inception to 31 October 2024, women of childbearing age are recommended to consume whole grains, fish, soy products, olive oil, full-fat dairy products, nuts, and moderate amounts of fruits while reducing red meat and sugar-sweetened beverage intake to lower the risk of GDM. There remains inconsistency regarding the association between tubers or starchy vegetables, vegetables, eggs, and poultry and the risk of GDM. In conclusion, current research on the association between diet and GDM is limited and offers suggestions for methodologies to obtain robust evidence regarding the association between the EAT-Lancet reference diet and GDM. More consumers are replacing cow’s milk (CM) with plant-based drinks (PBD), but data indicating the nutritional impact are limited. This theoretical study aims to assess the effect of substituting CM with PBD sold in Italy on nutrient intake within two dietary patterns: one aligned with the EAT-Lancet Commission reference diet adapted to Italian food habits (EAT-IT) and another one in line with the Italian Dietary Guidelines (IDG). Nutrition information from 368 PBD were collected and categorized according to their descriptive name and their fortification or not with calcium (Ca- and nCa-fortified). The substitution of CM with each PBD category in both dietary patterns was conducted, and an analysis of nutrient content and adequacy was performed. Substituting CM with all PBD resulted in reduced protein intake, except for nCa-fortified soy drinks, decreased saturated fat and vitamins B2 and B12, and increased fiber intake. Replacing CM with nCa-fortified PBD within both diets decreased Ca intake. Following the substitution of CM with Ca-fortified PBD, variations in vitamin D intake depended on the PBD category. The main risk of nutritional inadequacy was observed in Ca and vitamin D levels, which may even be amplified considering the different bioavailability based on the source of nutrients. This study highlighted the important role of CM in meeting calcium requirements and the potential unintended consequences of substituting CM with PBD without considering their nutritional differences. The EAT-Lancet commission advocated a planetary health diet in 2019. Some have raised concerns about its nutrient adequacy. This study used data from recent Nutrition and Health Surveys in Taiwan—from 2017 to 2020 (n = 6538)—to assess food intake and nutrient adequacy among three red meat consumption levels (low/medium/high). The low red meat group, whose diet was similar to the EAT-Lancet reference, showed significantly higher/better levels of vitamins C and E, calcium, magnesium, sodium, dietary fiber, and the polyunsaturated to saturated fatty acids ratio. However, protein, B vitamins, phosphorus for females, and zinc were slightly compromised, but they were still near or above 100% of the Daily Reference Intakes (DRIs), except for zinc (74~75%). The intake levels of vitamin D, calcium, and dietary fiber in all three groups at times did not reach 70% of the DRIs, but this was more pronounced in the high red meat group compared to the low red meat group. Replacing ultra-processed foods (UPFs) with whole/healthy foods improved levels of zinc, calcium, and dietary fiber, but not vitamin D. Finally, a proposed local planetary health dietary construct was provided, suggesting maintaining the original distribution of the food groups recommended by the Taiwan Food Guide while specifying amounts of protein sources in line with the EAT-Lancet principles. The proposed diet, according to our estimation and comparison with Taiwanese DRIs, was nearly perfect in its nutrient composition. Full article

In 2019, the EAT-Lancet Commission proposed a Planetary Health Diet (PHD) to address challenges toward sustainable and healthy diets. However, its suitability within the Dutch context and a comparison with the Dutch Dietary Guidelines (DDG) needs investigation. Our study aimed to compare the PHD with DDG in terms of food groups, servings, nutritional content, and adequacy in adults. We modeled two theoretical diets, the PHD (PHD-NL) and another based on the DDG (DDG-NL), using the Dutch National Food Consumption Survey (FCS-2016) and Dutch Food Composition Database to calculate the nutritional content and compared it with the Dutch Dietary Reference Values (DRVs). The PHD included higher quantities of vegetables, fish, legumes, and nuts, while the DDG suggested more significant amounts of cereals, tubers, starchy vegetables, dairy, and red meat. We observed differences in macronutrient distribution; while both diets lacked sufficient vitamin D, calcium content was lower in the PHD-NL. The PHD-NL had higher levels of fiber, vegetable protein, unsaturated fats, and non-heme iron, while vitamins B2, B6, B12, and calcium were lower than the DDG-NL diet. The PHD-NL has nutritional adequacy in the Dutch context, except for vitamin D and calcium, although it is essential to be cautious with iron because of the bioavailability of non-heme iron in plant-based diets. These findings have implications for the adoption of a sustainable diet according to nutritional requirements, population health status, and sociocultural context, as well as compliance with specific dietary behaviors of populations.
