Special Issue Information

Dear Colleagues,

Valvular heart disease (VHD) is increasingly recognized as a systemic disorder in which outcomes are driven not only by the severity of the valvular lesion but also by the broader extravalvular burden. Although the management of VHD remains primarily focused on achieving diagnostic accuracy and correcting valve dysfunction, a growing body of evidence shows that long-term outcomes are often determined by both cardiac and extracardiac alterations. Atrial and ventricular remodeling, pulmonary hypertension, myocardial dysfunction, and fibrosis may persist—or continue to progress—despite technically successful valve intervention. Moreover, multi-organ involvement driven by congestion, impaired perfusion, and coexisting comorbidities increasingly requires a more integrative clinical perspective.

This Special Issue brings emerging insights into this expanding framework, highlighting advances in multimodality imaging, refined risk stratification, and targeted therapeutic strategies aimed at mitigating the extravalvular burden of VHD. By looking beyond the valve, we aim to accelerate the adoption of integrative approaches capable of altering disease progression and improving patient outcomes. We welcome contributions that challenge conventional paradigms and offer new tools for the contemporary management of VHD.

Journal of Clinical Medicine is an international peer-reviewed open access semimonthly journal published by MDPI.
