Lung Cancer Surgery: Where Are We Now?

Special Issue Information

Dear Colleagues,

Lung cancer surgery is currently entering a significant evolutionary era in several aspects. First, the development of minimally invasive surgery, such as robot-assisted thoracic surgery (RATS) and uniportal-video-assisted thoracic surgery (UVATS), has been remarkable, and several advances are ongoing, including the expansion of their indications and the fusion of them. Second, sublobar resection, such as segmentectomy or wedge resection, has attracted much attention. Although lobectomy has been the standard surgical procedure for more than 60 years, recent large randomized controlled trials have demonstrated that sublobar resection can be applied for the patient with peripheral tumor with 2 cm or less in size if sufficient surgical margins can be secured. Based on these results, sublobar resection is expected to be more frequently employed in the future, but unlike lobectomy, many issues remain such as optimization of resection line and indications for conversion to lobectomy. Third, the perioperative treatment of using immune checkpoint inhibitors (ICIs) and molecular-targeted drugs has been recently applied for the patients with resectable locally advanced lung cancer. However, there are still many unresolved issues, such as appropriate patient selection, the timing of treatment introduction (preoperative, postoperative, or both), and the possibility of multidisciplinary treatment combined with radiotherapy and chemotherapy. This Special Issue aims to address various issues and advances, including the above, and provide a perspective on lung cancer surgery.
