Special Issue Information

Dear Colleagues,

Transcatheter aortic valve implantation (TAVI) is considered the default intervention for patients with severe symptomatic aortic stenosis in elderly populations or when surgery is not feasible, high risk, or prohibitive. Due to the rapid post-procedure recovery rates coupled with emerging medium- to long-term data, there has been a growing interest in expanding TAVI to low-risk populations. Moreover, TAVI using supra-annular valves may be better than surgery in patients with small aortic annulus at risk of prosthesis–patient mismatch. However, specific patient and anatomical characteristics limit the expansion of TAVI.

TAVI can be challenging in patients with bicuspid aortic valves and complex aortic anatomy, potentially leading to complications like annulus rupture and coronary artery occlusion. Growing operator expertise and newer TAVI valve iterations may mitigate those risks.

The direction of aortic valve surgery is undoubtedly moving towards the keyhole approach with emerging techniques, such as leaflet modification and better algorithms for lifetime management with redo TAV in TAV.

This Special Issue aims to focus on the current treatments and future considerations of TAVI. To this end, state-of-the-art reviews and original research articles focused on contemporary TAVI procedure planning and lifetime management will be warmly welcomed.
