Dear Colleagues,
Cholangiocarcinoma is an aggressive and heterogeneous malignancy of the biliary tract with increasing global incidence and persistently poor prognosis in advanced stages. Because most patients are diagnosed with unresectable or metastatic disease, systemic therapy remains the cornerstone of management. While gemcitabine plus cisplatin has long served as the standard first-line treatment, recent advances—most notably the integration of immune checkpoint inhibitors and the emergence of molecularly targeted agents—have begun to reshape the therapeutic landscape.
Despite these developments, major challenges remain in clinical practice. Treatment outcomes are still modest, optimal sequencing of therapies is not well defined, and resistance to both chemotherapy and targeted agents is increasingly recognized. At the same time, rapid progress in tumor biology, molecular profiling, and translational research has created new opportunities to refine systemic treatment strategies and improve patient selection. These advances make cholangiocarcinoma a timely and highly relevant area of investigation for both academic research and real-world oncology practice.
This Special Issue will present and disseminate recent advances in systemic therapy for cholangiocarcinoma. We welcome contributions addressing the biological basis of treatment response and resistance, evolving systemic treatment strategies, and the translation of emerging evidence into clinical practice.
Topics of interest for publication include, but are not limited to, the following:
- Advances in first-line and subsequent systemic therapies;
- Immunotherapy and combination strategies;
- Targeted therapies and biomarker-driven treatment approaches;
- Mechanisms of treatment resistance and therapeutic optimization;
- Translational research linking tumor biology to systemic treatment;
- Real-world evidence and clinical outcomes in cholangiocarcinoma.
