Dear Colleagues,
Despite remarkable advancements in technologies and pharmacological therapies, cardiovascular disease remains the leading cause of morbidity and mortality worldwide. Over the past decades, significant efforts have been directed toward enhancing primary and secondary prevention, with pivotal clinical trials establishing new therapeutic paradigms and expanding the arsenal of pharmacological agents capable of improving long-term patient outcomes. Nevertheless, the decline in cardiovascular mortality appears to have plateaued in recent years. Moreover, a substantial number of patients who experience an acute myocardial infarction experience subsequent adverse cardiac events. In this context, emerging challenges call for a more nuanced approach to residual risk reduction. In particular, a dual focus on residual lipid and inflammatory risk is increasingly recognized as essential. Addressing these two interrelated yet distinct pathophysiological components—often incompletely targeted by standard therapies—represents a critical frontier for further improving outcomes in patients with established atherosclerotic disease. In this Special Issue, we present an overview of new technological and pharmacological opportunities, focusing on novel diagnostic approaches, risk stratification, and challenges related to residual risk management.
