Dear Colleagues,
The paradigm of prostate cancer treatment is evolving rapidly. Especially for patients in high-risk and oligometastatic stages, discussions on the utility of a multidisciplinary approach, including surgical intervention beyond conventional standard care, are more active than ever. Although diverse perspectives remain regarding the optimal combination of therapies, the attempt to improve patient prognosis by integrating local control with systemic treatment has become a pivotal theme in modern urological oncology.
The central challenge in managing high-risk and oligometastatic prostate cancer lies in understanding the complex interplay between intensive local and systemic therapies and the emerging molecular resistance mechanisms. While aggressive surgical cytoreduction and multidisciplinary efforts (radiotherapy, ADT, ARPI, and Docetaxel) are being pursued, certain cancer cells survive through sophisticated mechanisms such as the expression of molecular chaperones (e.g., Clusterin) or activation of autophagy. These adaptive responses are fundamental drivers of evolution into castration-resistant prostate cancer (CRPC).
This Special Issue aims to disseminate the most recent advances related to the clinical journey of high-risk prostate cancer and its underlying scientific evidence. We invite original research and review articles covering the spectrum from 'Diagnosis to Treatment and Overcoming Resistance'.
Topics of interest include the following:
- Innovations in robotic-assisted radical prostatectomy (RARP) for high-risk PCa;
- Multidisciplinary management of oligometastatic prostate cancer;
- Molecular mechanisms of Docetaxel and ARPI resistance;
- Role of molecular chaperones (Clusterin) and autophagy in tumor survival;
- Advanced imaging (PSMA PET/CT) in risk stratification;
- Therapeutic strategies to delay CRPC progression.
