Special Issue Information

Dear Colleagues,

Since the 1970s, despite substantial advances in the treatment of fungal infections, particularly those caused by Candida spp., crude mortality rates associated with candidemia have remained above 40% among high-risk patients. Furthermore, the positive predictive value of candidemia for disseminated candidiasis is only approximately 40%. These findings indicate that candidemia cannot be considered the gold standard for the diagnosis of invasive candidiasis. Although the European Organisation for Research and Treatment of Cancer (EORTC) definitions for patients with cancer, transplant recipients and other immunocompromised hosts—as well as the diagnostic criteria for multifocal candidiasis in critically ill patients—have facilitated earlier diagnosis and treatment with improved outcomes, a true gold standard for invasive candidiasis is still lacking. The search for this long-awaited gold standard should therefore focus on two complementary biomarker strategies. The first should identify biomarkers of immunodeficiency or immunoparalysis that accurately reflect the severity of immune dysfunction in high-risk patients. The second should identify circulating biomarkers demonstrating that Candida has invaded and damaged the vascular endothelium, thereby confirming invasive candidiasis even in the absence of positive blood cultures. Integrating these two biological approaches could provide the foundation for novel antifungal treatment strategies and facilitate the development of therapeutic interventions specifically targeting immunodeficiency or immunoparalysis.
