Dear Colleagues,
Antimicrobial resistance is one of the most pressing challenges in modern healthcare, and its impact is particularly relevant in critical care medicine. Intensive care units represent a unique environment where severe infections, high antimicrobial consumption, invasive procedures, prolonged hospital stays, and the frequent presence of multidrug-resistant organisms converge. At the same time, delays in appropriate antimicrobial therapy can have major consequences for critically ill patients, making the balance between timely treatment and responsible antibiotic use especially complex. Antibiotic stewardship in the ICU therefore requires more than simply reducing antibiotic exposure. It involves selecting the appropriate antimicrobial agent, optimizing dose and route of administration, ensuring adequate timing, and defining the shortest effective duration of treatment. Advances in rapid diagnostics, pharmacokinetic/pharmadynamic optimization, biomarkers, therapeutic drug monitoring, and artificial intelligence may further support individualized antimicrobial strategies. Optimizing antibiotic stewardship allows the maintaining of patient safety and antimicrobial strategies.
