Dear Colleagues,
Antimicrobial resistance is one of the most pressing global public health challenges of the 21st century, threatening the effectiveness of antibiotics, antifungals, and antivirals across human and veterinary medicine.
This Special Issue, “Paradigm Shifts in Antimicrobial Stewardship: From Innovation to Clinical Impact” seeks to highlight innovative approaches that challenge conventional paradigms. We welcome original research, reviews, perspectives, and methodological papers that introduce novel concepts, technologies, and analytical frameworks with the potential to transform stewardship practices.
Particular emphasis will be placed on advanced pharmacokinetic/pharmacodynamic (PK/PD) methodologies, artificial intelligence and machine learning applications, systems biology approaches, microbiome-informed stewardship strategies, and next-generation surveillance tools. Contributions exploring innovative approaches to cumulative antibiograms, dynamic resistance forecasting, real-time decision-support systems, and novel metrics for evaluating antimicrobial utilization and outcomes are highly encouraged.
We also invite submissions focused on assessing the ecological impact and selection pressure of antimicrobial agents. Studies developing predictive models for optimal treatment duration, individualized therapy, resistance emergence, and stewardship interventions are particularly welcome.
This Special Issue aims to stimulate interdisciplinary collaboration and identify the next generation of tools and approaches needed to preserve antimicrobial effectiveness.

Manuscripts can be submitted until the deadline. All submissions that pass pre-check are peer-reviewed. Accepted papers will be published continuously in the journal (as soon as accepted) and will be listed together on the special issue website. Research articles, review articles as well as short communications are invited. For planned papers, a title and short abstract (about 250 words) can be sent to the Editorial Office for assessment.
Submitted manuscripts should not have been published previously, nor be under consideration for publication elsewhere (except conference proceedings papers). All manuscripts are thoroughly refereed through a single-anonymized peer-review process. Antibiotics is an international peer-reviewed open access monthly journal published by MDPI.
