Dear Colleagues,
Candida represents a significant group of opportunistic pathogens responsible for a spectrum of infections that range from superficial mycoses to severe invasive candidiasis, which can result in considerable morbidity and mortality rates. While Candida albicans remains the most frequently encountered species, there is a growing incidence of non-albicans Candida, such as C. glabrata and C. auris, which are often associated with reduced susceptibility to antifungal agents.
Antifungal therapy typically involves the use of azoles, echinocandins, and polyenes. Azoles, including fluconazole, function by inhibiting ergosterol synthesis and are extensively utilized within clinical practice. In contrast, echinocandins are commonly recommended as the first-line treatment for invasive candidiasis due to their fungicidal properties and favorable safety profile. Polyenes, such as amphotericin B, are generally reserved for severe or refractory cases.
Therapeutic agent selection is contingent upon the severity of the infection, host-related factors, and local patterns of resistance. The rise in antifungal resistance underscores the necessity of accurate species identification and susceptibility testing. Therefore, it is imperative to optimize antifungal strategies in order to enhance clinical outcomes in patients suffering from Candida infections.
